The officer in charge of a navigational watch must conduct a navigation assessment for the intended route and operations prior to getting underway. The navigation assessment must incorporate the requirements of pilothouse resource management of 46 CFR 140.640, assess operational risks, and anticipate and manage workload demands. Vessel had not completed a navigation assessment since 29APR25. 46 CFR 140.635

The design of the system and its installation on the towing vessel is certified and inspected by a registered professional engineer with experience in fire-detection system design, by a technician with qualifications as a National Institute for Certification in Engineering Technologies (NICET) level. Vessel's fire detection panel is not certified.

Each towing vessel must perform the applicable tests in Table 143.245(b) of this section. The tests required by this section must be recorded in accordance with part 140 of this subchapter. Vessel has no proof of testing machinery alarms.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Port main engine secondary oil pressure sensor was inop.

Pressure vessels over 5 cubic feet in volume and over 15 pounds per square inch maximum allowable working pressure (MAWP) must be equipped with an indicating pressure gauge (in a readily visible location) and with one or more spring-loaded relief valves. The total relieving capacity of such relief valves must prevent pressure from exceeding the MAWP, as established by the manufacturer, by more than 10 percent. The relief valves were not set at the proper pressure.

The owner or managing operator must implement procedures to identify and mitigate health and safety hazards. The hot water heater was not secured to the deck.

Each distribution panel or switchboard on a towing vessel must be totally enclosed and of the dead-front type. The distribution panel was missing a blank on the engine room panel.

The CG will issue a medical certificate to a mariner meeting the medical and physical standards for merchant mariners. Crew member's medical certificate was expired.

Piping and machinery components that exceed 220 °C (428 °F), including fittings, flanges, valves, exhaust manifolds, and turbochargers, must be insulated. Measures must be in place to prevent flammable or combustible liquid piping leaks from coming into contact with these components. The port and stbd generator lagging is loose and must be secured.

Means must be provided for stopping each fan in a ventilation system serving machinery spaces and for closing, in case of fire, each doorway, ventilator, and annular space around funnels and other openings into such spaces. Engine room built-in ventilation is insufficient for securing air flow into the engine room.

All owners and operators of an existing towing vessel must have a valid COI on board no later than July 19, 2022. This vessel has not obtained a valid COI and is no longer permitted to operate.

46 CFR 136.212(b) Inspection for certification. Nature of inspection. The inspection will ensure that the vessel is in satisfactory condition and fit for the service for which it is intended, and that it complies with the applicable statutes and regulations for such vessels. The inspection will include inspections of the structure, pressure vessels and their appurtenances, piping, main and auxiliary machinery, electrical installations, lifesaving appliances, fire detecting and extinguishing equipment, pilot boarding equipment, and other equipment. While conducting the vessel's initial COI inspection, excessive structural steel wastage/holed areas were discovered throughout watertight wing tank bulkheads and structural frames in multiple voids and wing tanks, and excessive scaling throughout all voids and wing tanks. Prior to issuance of the initial COI, vessel must complete the remainder of the COI inspection and all void and wing tanks must be dewatered and descaled in order to assess extent of wastage. Hull and structural areas where wastage is in question to be in excess of 25% of original thickness must gauged by a Non-Destructive Testing (NDT) level II technician. All hull and structural areas in excess of 25% wastage of original thickness must be cropped and renewed. Owner/Operator has been informed that a dry dock may be required once assessment has been completed. Vessel may not be used for towing operations.

All owners and operators of an existing towing vessel must have a valid COI prior to the specified phase-in period. This vessel is one of the associated fleet that has not obtained a valid COI. The vessel shall be made available for a Coast Guard COI inspection within 90 days. If an inspection is not completed prior to this date, the action for this deficiency shall be changed to a 60 (Rectify deficiencies prior to movement) and the vessel may no long be permitted to operate.
