crew were unaware that the 2 fuel valves were unable to
be closed by the remote closure devices rendering the
vessel a major safety risk if a fire occurred. Repair
immediately and prior to movement. 46 CFR 143.260(d)

Resolution: Vessel had 01 fuel valve that was installed inproperly and seized. Vessel replaced it and Marine inspectors witnessed test of both fuel valves.

narrow catwalks in way of deck-house sides, hand grabs
may be substituted." No railing or hand grabs alongside
the side deck superstructure, bulwark only 2 feet high,
creating an unsafe condition for crew members to
potentially fall overboard. Within two weeks, coordinate
