Individual circuit breakers on switchboards and distribution panels must be labeled with a description of the loads they serve. Observed port circuit breakers on port distribution panel, in engine room, not labeled.

All vessel equipment must be used in accordance with the manufacturer's recommended practice and in a manner that minimizes risk of injury or death. Observed hot water heater relief valve not positioned to drain directly into bilge. Position relief valve to drain directly into bilge in order to prevent personnel injury.

To stop the flow of fuel in the event of a fire or break in the fuel line, a remote fuel shutoff valve must be fitted on any fuel line that supplies fuel directly to a propulsion engine or generator prime mover. Observed port and starboard fuel shutoff valves inoperable.

The following list of items must be recorded in the TVR, official logbook, or in accordance with the TSMS applicable to the vessel: (1) Personnel records, in accordance with § 140.400; (2) Safety orientation, in accordance with § 140.410; (3) Record of drills and instruction, in accordance with § 140.420; (4) Examinations and tests, in accordance with § 140.615; (5) Operative navigational safety equipment, in accordance with § 140.620; (6) Navigation assessment, in accordance with § 140.635; (7) Navigation safety training, in accordance with § 140.645; (8) Oil residue discharges and disposals, in accordance with § 140.655; (9) Record of inspection of towing gear, in accordance with § 140.820; and (10) Fire-detection and fixed fire-extinguishing, in accordance with § 142.240. The vessel had not maintained TVR logs during the month of March. 46 CFR 140.915

The officer in charge of a navigational watch must conduct a navigation assessment for the intended route and operations prior to getting underway. The vessel had not completed a navigational watch during the month of March. 46 CFR 140.635(a)

The safety of the towing vessel is the responsibility of the master and includes: (1) Adherence to the provisions of the COI; (2) Compliance with the applicable provisions of this subchapter; (3) Compliance with the TSMS, if one is applicable to the vessel; and (4) Supervision of all persons onboard in carrying out their assigned duties. The master was not familiar with the recording requirements for the TVR or navigational watch assessments. Furthermore, the master was unaware of the company's Employee Assistance Program (EAP). Company must conduct training with all vessel masters regarding proper logging procedures and ensure that all persons have sufficient knowledge of the EAP. 46 CFR 140.210(a)

Heat detector above stbd main with loose cover and water inside, crew members attempted to test in manner not as per manufaturers instructions.

General alarm back up source of power didn't work.

JW head tank for center MDE had a busted level guage, fuel guage hose was an innapropriate type for the main service fuel tank, the clothes dryer back panel was left opened and wiring exposed, hot water tank needs to have it's relief valve drain routed into the bilge in a safe manner, and the steering electrical box cover wasn't secured.

Vessel couldn't demonstrate that the version of their electronic charts (Coastal Explorer) was updated with the LNMs.
