The owner and managing operator must ensure that the towing vessel is in compliance with this subchapter and other applicable laws and regulations at all times. Vessel's remaining voids all have a significant amount of water in them. Due to known water entering the bow and port side voids the condition the vessel's hull structure is in question. Prove to the attending marine inspector the hull has not been compromised and not taking on water and meets the minimum requirements for hull structure.

The cognizant OCMI may require additional examinations of the vessel whenever he or she discovers or suspects damage or deterioration to hull plating or structural members that may affect the seaworthiness or fitness for the route or service of a vessel. Due to multiple instances of hull damage found, the vessel's structural and watertight integrity cannot be accepted as adequate for issuance of a COI. Vessel is to be dry-docked to verify condition of hull and make sufficient repairs.

Each towing vessel must be equipped with a means to detect smoke in the berthing spaces and lounges that alerts individuals in those spaces. Install missing smoke detectors in all lounges and berthing spaces.

Electrical systems and equipment must function properly, minimize system failures and fire and shock hazards. Distribution panels are missing breakers, provide breakers or blanks to cover open blanks reducing the chance of shock.

Prior to USCG visit crew found engine room high bilge level alarm inoperable. Prove proper operation of high bilge level alarm.

The Internal survey process results illustrate failure to capture vessel discrepancies. During the survey completed on 14 May 2018, the surveyor, acting as an independent surveyor hired by Southern Illinois Transfer Company, failed to identify the vessel's compromised watertight integrity. The owner and managing operator failed to ensure that the towing vessel is in compliance with this sub-chapter and other applicable laws and regulations at all times. Based on the failure of the surveyor to identify the above deficiency (at a minimum) and the owner and managing operator failing to ensure compliance, Towing Vessel Inspection Bureau (TVIB) shall complete an external management audit of Southern Illinois Transfer Company's TSMS in accordance with (IAW) 46 CFR 138.510 and provide evidence addressing the TSMS elements IAW 138.220(a), 138.220(b)(1), 138.220(c)(1) and 138.220(d); specifically a review of the Southern Illinois Transfer Company's Internal Survey Program and surveyor hiring process IAW 137.210. Additionally, the survey dated 14 May 2018 of M/V JAKE HUFFTY is considered invalid and shall be completed by a qualified surveyor.

Vessels General Alarm did not operate and supplemental flashing light for the engine room did not flash. Ensure vessels general alarm operates IAW 46 CFR 27.201.

Vessels fixed fire pump did not produce 60 PSI and 80 GPM and did not have remote start controls. Ensure vessels fixed fire pump meets the requirements of 46 CFR 27.301.

The anchor light on the top of the mast was inoperable.
