The Internal survey process results illustrate failure to capture vessel discrepancies. During the survey completed on 14 May 2018, the surveyor, acting as an independent surveyor hired by Southern Illinois Transfer Company, failed to identify the vessel's compromised watertight integrity. The owner and managing operator failed to ensure that the towing vessel is in compliance with this sub-chapter and other applicable laws and regulations at all times. Based on the failure of the surveyor to identify the above deficiency (at a minimum) and the owner and managing operator failing to ensure compliance, Towing Vessel Inspection Bureau (TVIB) shall complete an external management audit of Southern Illinois Transfer Company's TSMS in accordance with (IAW) 46 CFR 138.510 and provide evidence addressing the TSMS elements IAW 138.220(a), 138.220(b)(1), 138.220(c)(1) and 138.220(d); specifically a review of the Southern Illinois Transfer Company's Internal Survey Program and surveyor hiring process IAW 137.210. Additionally, the survey dated 14 May 2018 of M/V JAKE HUFFTY is considered invalid and shall be completed by a qualified surveyor.
