A fixed fire pump must be capable of being energized remotely from a safe place outside the engine room and at the pump. The "High" mode on the remote control panel located for the fire pump, was inoperable. Make necessary repairs to ensure operation of "Low" and "High" modes.

Storage of flammable or combustible products must be stored in a designated storage room or cabinet when not in use. If a storage room is provided, it may be any room or compartment that is free of ignition sources. The storage room used to store paint cannisters contained non-explosionproof installations of wires, including the ship-to-shore power receptacle.

Each towing vessel must be maintained and operated so as to minimize fire hazards and to ensure the following: (a) All bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquids insofar as practicable. The vessel's bilge space in the engine room had an excessive accumulation of fuel oil.

Pressure vessels over 5 cubic feet in volume and over 15 pounds per square inch maximum allowable working pressure (MAWP) must be equipped with one or more spring-loaded relief valves. The total relieving capacity of such relief valves must prevent pressure from exceeding the MAWP, as established by the manufacturer, by more than 10 percent. The relief valves installed on the port and starboard side pressure vessels were found coated with paint. Vessel replaced with new relief valves.

PT01- A towing vessel's survival craft must meet the functional requirements of paragraphs (b)(1) through (b)(5) of this section. Survival craft must have an aggregate capacity sufficient to accommodate the total number of individuals on board. During the Initial COI inspection, it was determined that the vessel only has a four person survival craft. The [name withheld] rivers route ends at the line of demarcation; the vessel route allows it to transit out to Buoy "1" for vessel escort work. A survival craft which accommodates all persons on-board is required when the vessel operates outside the line of demarcation. Vessel may not transit outside the line of demarcation with more than four person.
