Machinery systems must be designed and maintained to provide for safe operation of the towing vessel and people onboard under normal and emergency conditions. Each towing vessel must have a means to monitor rudder angle at the operating station. Inspectors found the steering system to only be working in non-followup with multiple failure alarms. The rudder angle indicator was noted to be 7 degrees off. Steering system must be repaired and tested to the satisfaction of the Coast Guard. 46 CFR 143.205 46 CFR 143.225

Any vessel of at least five net tons which engages in the fisheries on the navigable waters of the United States or in the EEZ or coastwise trade, unless exempt under 67.9 must have a Certificate of Documentation bearing a valid endorsement appropriate for the activity in which engaged. Vessel owner changed the name of the vessel on the 12 of April 2024 and invalidated all documents. Must update FCC documents to include Bridge to Bridge, EPIRB, FCC license, markings on vessel, vessel security plan, MMSI number, AIS listings, names on PFD's and life buoys. Finally any company plans that include the name Sea Eagle must be updated to include DAPI testing lists. All must be corrected prior to operation. 46 CFR 67.7 Code 60c

Towing vessels must be equipped with illuminated magnetic compass. Vessels compass light is not operable. Must keep flash light on bridge until corrected to the satisfaction of the attending MI. 46 CFR 140.725 (d) Code 50c Code

Each towing vessel must have a reliable means to provide notification when an emergency condition exists: high bilge alarms. Vessel has a fwd and aft engine room bilge alarm, the fwd alarm is not working. Repair to the satisfaction of the attending MI. 46 CFR 143.230 (a)(5) Code 50c

Each towing vessel in oceans and coastwise service must have a line throwing appliance approved under series 160.040: 46 CFR 160.040-4 requires four rocket projectiles. Vessel currently has two and will need to provide two more to the satisfaction of the attending MI. 46 CFR 141.385 Code 50c

Each towing vessel must be maintained and operated so as to minimize fire hazards and to ensure the following: all bilges and void spaces are kept free from accumulation of combustible and flammable materials... Port side lube oil tank and winch engine fuel oil tank both require sigh glass protection to prevent accidental discharge of liquids into the engine room. Repair to the satisfaction of the attending MI. 46 CFR 142.220 (a) Code 50c

Pressure vessel relief valves must be changed twice every 5 years with no more than three elapsing since last test. Vessel is unable to provide documentation to the last time the port and stbd air receivers had the safety relief valves changed out. Provide new reliefs to the satisfaction of the attending MI. 46 CFR 143.245 (b) Code 50c

Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system requires attention, alarms must be audible and visual at the operating station. Vessel is missing alarms for the port and stbd generators at the operating stations for low lube oil pressure and high j/w temp. Repair to the satisfaction of the attending MI. 46 CFR 143.230 Code 50c

The cognizant OCMI may require review of a vessel's watertight integrity. Forward hold has a 1/4" gap at the top of the WTD gasket, replace to the satisfaction of the attending MI. 46 CFR 144.330 Code 50c

Towing vessel must meet the communications requirements of 33 CFR part 26 and 33 CFR 164.72 as applicable. Vessels HF radio is not set up with MMSI or lat/long GPS coordinates. It must be corrected for distress alert to properly notify USCG when more than 20NM from shore. Repair to the satisfaction of the attending MI. 46 CFR 140.715 (a) Code 50c

The Coast Guard may issue the permit either on form CG-948, "Permit to proceed to Another Port for Repairs," or in letter form, and will state the conditions under which the vessel may proceed to another port for repair. This office rcvd a request for the subj vessel to proceed to MSU Port Canaveral's AOR, to perform investigation/repairs to the STBD shaft due to a possible fracture in the shaft. The vessel is authorized a one-time permit to proceed to MSU Port Canaveral's AOR under the following conditions; 1) Vessel will proceed from Fort Pierce, FL directly to 99 Dover Ave. Merritt Island, FL. 2)Vessel is prohibited from pulling, pushing or towing of any kind, including any other type of commercial service. 3) Vessel must ensure that STBD prop and shaft is properly secured and affixed to the vessel. 4) Vessel will notify MSU Port Canaveral once vessel has been made ready for damage survey.

Survival craft provide a means for survival when evacuation from the towing vessel is necessary, life raft painter line was incorrectly attached to the framing of the life raft cradle which prohibits the hydrostatic release from releasing the liferaft, once inflated, should the vessel sink. Corrected while onboard.

Pressure vessels over 5 cubic feet and over 15 PSI MAWP must be equipped with one or more spring loaded relief vavles with a total releiving capacity to prevent the pressure from exceeding the MAWP by more than 10%. A valve was installed between the releif valve and the pressure vessel, which could prohibit the releif vavle from activating if the valve was closed or malfunctioned.

Relief valves must be tested in accordance with 46 CFR 143.245. Pressure relief valves on pressure vessels must be tested twice every 5 years with no more than 3 years elapsing since the last test. Relief valves not tested in accordance with those intervals.

Towing vessels must meet the requirements of 33CFR part 26 and 33 CFR 164.72, as applicable. Both vessel's VHF radios do not display lat and long nor display the vessel's MMSI number for the distress call feature.

Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention. The following alarms must be provided: M/E low lubricating pressure; M/E high cooling water temp; aux generator engine low lube oil pressure; aux generator high colling water temp; All alarms were unavailable for inspection.

Essential systems or equipment must be regularly tested and examined. Overspeed shutdown on main engines and gen-sets were unable to be tested during inspection.

Fuel shutoff must be operable from a safe place outside the space where the valve is installed. Fuel shut off cable attached to fuel line valve, frayed, damaged and not properly attached.

Each life buoy must be approved under approval series 160.050 or 160.150. port side life bouy retro tape is deteriorated.

Each towing vessel operating north of lat 32 north or south of lat 32 south must carry a minimum number of immersion suits, the immerision suit has to be personally assigned to each crew member and be of a proper size to fit the crew member, in addition each watch station must have enough immerission suits to equal the number of person normally on watch, Immerision suits storage location must be marked "anti exposure suits" as appropriate, immersion suits must be marked in such a way to identify person or vessel to which it belongs. Vessel did not have the minimum required immersion suits, did not have to proper size suits for crew onboard at the time, were not marked in accordance with this regulation and location of suit was not properly marked.

Each life jacket must be marked in capital block letters with the name of the vessel. All lifejackets did not have the vessel name marked on them.

Each towing vessel must have a fire-detection system installed and the owner or managing operator must ensure the following;the design of the system/install is certified and inspected by a registered professional engineer. Vessel does not have the PE approval onboard.

A towing vessel may not be operated without having onboard a valid COI issued by the Coast Guard as required by 136.202. Vessel's exceeded its annual inspection which was due by March 10, 2022, therefore rendering the COI invalid. Vessel must receive a satisfactory COI inspection prior to movement.

46 CFR 144.200 The vessel is built, equipped, and maintained to conform to the rules of a recognized classification society appropriate for the intended service and routes. Vessel rep's reported a pin hole 1/8 inch and 2 Lack of fusion cracks 1/2 inch, located By ultra sonic inspection. Repair to the satisfaction of CG Marine Inspector.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and saferty of persons onboard under normal and emergency conditions. SEA EAGLE reported excessive smoke from exhaust and a loss of propulsion on the starboard engine. Provide repair plan and tech report to the satisfaction of CG inspector.

The vessel is an inspected towing vessel for which the Owner has chosen the Coast Guard option and is therefore not required to have a Safety Management System per Regulation. Deficiencies [2 through 4] are objective evidence that a serious substandard condition has not been proactively managed by the vessel’s Owner/Operator, leading to a lack of safety culture onboard the vessel.

Each towing vessel must have a fire-detection system installed to detect engine room fires that is certified and inspected by a registered professional engineer with experience in fire-detection system design, by a technician with qualifications as a National Institute for Certification in Engineering Technologies (NICET) level IV fire alarm engineering technician, or by an authorized classification society with equivalent experience. Detection system is overdue for servicing.

Lined commercial fire hose 15 meters (50 feet) in length, at least 40 millimeters (1.5 inches) in diameter, must be capable of providing a 80 GPM. The starboard fire hose was holed and not able to provide effective stream.

Towing vessels must carry the minimum number of fire extinguishers in Table 142.230(a). All portable and semi-portable fire extinguishers were overdue for annual servicing.

Each towing vessel must be equipped with an industrial type first aid cabinet or kit, appropriate to the size of the crew and operating conditions. First aid kits contain expired contents.

Each lifebuoy on a towing vessel must: be approved under approval series 160.050 or 160.150. Retro-reflective material on lifebuoys weathered and requires renewal.

Electrical connections must be suitably installed to prevent them from coming loose through vibration. Install hex nuts on battery terminals.

Piping and machinery components that exceed 428 °F, including fittings, flanges, valves, exhaust manifolds, and turbochargers, must be insulated. Measures must be in place to prevent flammable or combustible liquid piping leaks from coming into contact with these components. Properly install lagging / blankets on the port main diesel engine and winch engine where missing or deteriorated.

The owner, master or officer in charge of a navigational watch of a towing vessel must ensure the strength of each component used for securing the towing vessel to the tow and for making up the tow is adequate for its intended service. The aft winch support assembly has excessively deteriorated structure on the port side which requires permanent repair.

Vessel not in compliance with the VGP.

Prove all crewmembers are enrolled in a drug and alcohol testing program.

Exercise all dogs and adjust landings as needed.

Renew shackles securing guide wires for the mast.

Prove proper operation of port side main deck fire main valve.

Remove or repair improperly installed valves in fire main.

Provide day shapes.

Provide required navigation publications.

Remove or properly stow flammables in the E/R.

Service port shaft packing to reduce leak.

The exhaust lagging on the port generator is excessively soaked. Identify cause of leak and renew lagging. Vessel to utilize the starboard generator until repairs are completed.

Bench test pressure relief valves to prevent pressure from exceding MAWP by more than 10 per cent.

Provide FCC Safety Certiicate and Marine Operator Permit. 47 CFR 80. 903

Renew retro-reflective tape on life buoys and provide waterlights.

Provide 12 VDS.

Provide three type I PFD's.

Repair leak in fire main and prove satisfactory water pressure.

Provide logbook for fire fighting equipment and testing of engines.

Provide one 40 B:C fire extinguisher.

Provide one line throwing gun.

Clean data plate on UPV for inspection.

Provide permanent connector on wire in E/R.
