Towing vessels must be equipped with the following equipment, as applicable to the area of operation: (a) Fathometer Vessel's fathometer was inop due to possible fault within the hull mounted sensor. Company plans to replace at drydock in OCT 2024. Diagnose cause and effect any necessary repairs at drydock. Repairs are made to the satisfaction of the US Coast Guard.

Emergency lighting must be provided for all internal crew working and living areas. Emergency lighting sources must provide for sufficient illumination under emergency conditions to facilitate egress from each space and must be either: (1) Automatic, battery-operated with a duration of no less than 2 hours; or (2) Non-electric, phosphorescent adhesive lighting strips that are installed along escape routes and sufficiently visible to enable egress with no power. Insufficient lighting or phosphorescent adhesive lighting strips present in berthing areas.

The master or person in charge of a ship to which this section applies shall ensure that the ship is not operated unless a garbage management plan meeting paragraph (b) of this section is on the ship and that each person handling garbage follows the plan. Prior to getting underway for the first time on a particular towing vessel, each crew member must receive a safety orientation. Safety orientation being conducted does not meet the requirements of 46CFR140.410(b)1 -(b)10.

A record of the type, size, and service of each towline, face wire, and spring line, used to make the towing vessel fast to her tow, must be available to the Coast Guard or third-party auditor for review. The following minimum information is required in the record: The dates when examinations were performed, the identification of each item of towing gear examined, and the name(s) of the person(s) conducting the examinations. Towing gear inspections could not be verified to certs or what towing gear was inspected.

Stowage positions for lifejackets stowed in a berthing space or stateroom and all lifejacket containers must be marked in block capital letters and numbers with the minimum quantity, identity, and, if sizes other than adult or universal sizes are used on the vessel, the size of the lifejackets stowed inside the container. Each Marine sanitation device must have a placard suitable for posting on which is printed the operating instructions, safety precautions, and warnings pertinent to the device. The size of the letters printed on the placard must be one-eighth of an inch or larger.

Electrical connections must be suitably installed to prevent them from coming loose through vibration or accidental contact. multi plug extension cord was being utilized under bridge console.

Charts, including electronic charts acceptable to the Coast Guard, of appropriate scale to make safe navigation possible. Electronic chart did not have power back up, vessel loaded charting program on lap top.

All seals and gaskets must be properly maintained to prevent leaks of flammable or combustible liquid, as those terms are defined in 46 CFR subpart 30.10, into the machinery space. Fuel pipe was leaking in the bilge, pipe was removed and capped, correcting the leak.

Each part of the Marine sanitation device that is required by the manufacturer's instructions to be serviced routinely must be readily accessible in the installed position of the device recommended by the manufacturer. Cover to MSD was wasted.

The radio requirements of the Vessel Bridge-to-Bridge Radiotelephone Act, 33 U.S.C. 1201-1208, part 26 of this chapter, and 47 CFR part 80. Vessel was missing Bridge-to-Bridge certificate, FCC issued prior to completion of exam.

A 40-B portable fire extinguisher must be located near the flammable storage room or cabinet. This is in addition to the portable fire extinguishers required by tables 142.230(a) and 142.230(b) of this part. 40-B was not in the area and accessible. 40-b was moved to be readily accesible.

Electrical equipment, piping for flammable or combustible liquid, seawater cooling, or fire-fighting systems must be provided with isolation devices and markings as follows: Piping for flammable or combustible liquid, seawater cooling, or firefighting systems must be fitted with isolation valves that are clearly marked by labeling or color coding that enables the crew to identify its function. Currently, the vessel is using an emergency fuel shut-off valve to prevent the cross flow of fuel between tanks. During inspection vessel list was induced due to the valve being left open and fuel inadvertently transferring to the other tank. When testing the emergency shut-off valve, it was not reset and would not operate as designed. Owner / Operator has proposed to install a cross flow isolation valve at the next dry-dock, since tanks will need to be emptied.

All owners and operators of an existing towing vessel must have a valid COI prior to the specified phase-in period. This vessel is one of the associated fleet that has not obtained a valid COI. The vessel shall be made available for a Coast Guard COI inspection within 90 days. If an inspection is not completed prior to this date, the action for this deficiency shall be changed to a 60 (Rectify deficiencies prior to movement) and the vessel may no long be permitted to operate.
