A towing vessel with overnight accommodations and alternating watches (shift work), when pulling, pushing or hauling alongside one or more barges, must have a system to detect when its master or mate (pilot) becomes incapacitated. Alerter system was installed however the system was not operating properly. Develop and implement two man bridge team , provide tech report and video evidence of proper operation.

Any vessel of at least five net tons which engages in coastwise trade must have a Certificate of Documentation bearing a valid endorsement appropriate for the activity in which engaged. The Certificate of Documentation for the HAILEY NICOLE is expired. Renew HAILEY NICOLE Certificate of Documentation.

Each towing vessel must perform the applicable tests in Table 143.245(b) of this section. The tests required by this section must be recorded in accordance with part 140 of this subchapter. Alarm setpoints have not been tested. Conduct all alarm setpoint testing.

Essential systems or equipment must be regularly tested and examined. Tests and examinations must verify that the system or equipment functions as designed. If a component is found unsatisfactory, it must be repaired or replaced. Starboard engine crank case engine protection device bypassed. Repair equipment as required.

The TSMS, through policies, procedures, and documentation, must: (a) Demonstrate management responsibility. The management must demonstrate that they implemented the policies and procedures as contained in the TSMS and the entire organization is adhering to their safety management program. (b) Document management procedures. A TSMS must describe and document the owner or managing operator's organizational structure, responsibilities, procedures, and resources which ensure quality monitoring. (c) Ensure document and data control. There must be clear identification of what types of documents and data are to be controlled, and who is responsible for controlling activities, including approval, issue, distribution, modification, removal of obsolete materials, and other related administrative functions. (d) Provide a process and criteria for selection of third parties. Procedures for selection of TPOs must exist that include how third parties are evaluated, including selection criteria. (e) Establish a system of recordkeeping. Records must be maintained to demonstrate effective implementation of the TSMS. This must include audit records, non-conformity reports and corrective actions, auditor qualifications, auditor training, and other records as considered necessary. (f) Identify and meet training needs. The owner or operator must establish and maintain documented procedures for identifying training needs and providing training. (g) Ensure adequate resources. Identify adequate resources and procedures necessary to comply with the TSMS. Vessel found to not be within compliance with TSMS by not completing required inspections and making repairs to essential systems. Vessel is required to undergo external survey demonstrating understanding and compliance with vessel TSMS to the satisfaction of the TPO and schedule a follow-on vessel audit with the TPO.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons on-board under normal and emergency conditions. Starboard propulsion engine experienced apparent crankcase explosion. Identified broken fuel injector line at the fuel rail on the outboard side. Troubleshoot and repair IAW manufacturer's direction or replace as required.

All bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquids insofar as practicable. Engine room bilges has oil/fuel/water mix resulting from crankcase explosion and subsequent fire extinguishing efforts. pump and clean bilge as required.

Towing vessels of more than 65 feet in length must carry— (1) At least the minimum number of portable fire extinguishers set forth in table 142.230(b); and (2) One 40–B portable fire extinguisher fitted in the engine room for each 1,000 brake horsepower of the main engines or fraction thereof. (03) extinguishers expended combating engine room fire. Replace to bring into compliance with COI. Each towing vessel must be operated in compliance with applicable sections of the Federal Water Pollution Control Act, including section 311 of the Federal Water Pollution Control Act, as amended (33 U.S.C. 1321). Grey water discharge pipe transiting through fuel oil tank cracked and leaking fuel oil overboard. Pump down and make tanks certified for entry/hot work by a certified marine chemist and make repairs to original condition.

Air receivers were not equipped with pressure relief valves.
