Identify the cause of the starboard MDE/transmission failure and enact an appropriate repair. A satisfactory sea trial must be completed in the pressence of a USCG marine inspector.

Observed expired alcohol swabs. Vessel is to replace swabs.

Compass light was not operational. Vessel is to ensure compass light is operational.

Observed Aft portside terminal box on weather deck was missing a cover. Vessel is to replace cover or terminal box.

Repair/replace aft door on second deck due to corrosion.

The general construction of the vessel must be as such as to minimize the fire hazards in so far as it is reasonable and practicable. MI identified missing insulation in laz on portside overhead. 46CFR116.405(a)

Each window, port hole and its means of attachments to the hull or deckhouse must be capable of withstanding the maximum load from wave or wind conditions expected due to its location on the vessel and the authorized rout of the vessel. MI identified cracked glass in LAZ exit door on main deck. 46CFR116.1020

A door must not be undercut more than .5 inches above the door sill. MI identified bottom of door on main deck exiting to back deck/laz chipping/rusting away on bottom portion. 46CFR116.435(c)(5)

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other person and the vessel from electrical hazards, including fire caused by originating in electrical equipment. MI observed outlet in galley space to be taped and secured in the off position. 46CFR120.200

On 15MAY2023, It was reported that the ADVENTURE HORNBLOWER lost propulsion on their approach to the dock. Vessel operators must identify the cause of the loss of propulsion and submit a report/description to the OCMI detailing the events. USCG must witness satisfactory propulsion prior to the carriage of passengers and request permission to shift within the port.

Junction box on no.3 deck port side IWO aft stairwell found to be not secured. Secure junction box to prevent movement.

A means of escape that is not readily apparent to a person from both inside and outside the space must be adequately marked in accordance with 46 CFR 122.606.

Tubular gauge glasses must be adequately protected from mechanical damage. Need to add two (02) more protection rods to the gauge glass.

Nonconducting mat must be provided in front of the engine space distribution box next to the switchboard.

Must provide bilge high level alarm in the fuel tank/DPF space.

Multiple exposed wires were found in the wheelhouse under the console, adjacent to the windshield wiper switches and on the port side overhead while leaving the wheelhouse. All wires in use need to be properly terminated.

The remote start for the vessel's starboard MDE is inoperable. Repair the remote start and test in the presence of a USCG marine inspector.

A failed gasket on the bow thruster is leaking water into the bow thruster void. Enact permanent repairs to the bow thruster leak. Hourly rounds of the space must be conducted until the vessel is repaired or hauled out to verify that pumping arrangements are keeping up with the leak.

Vessel suffered a 6ft x 1ft gash along the port quarter into the steering gear room.

Due to the nature of the allision, the vessel will be required to demonstrate proper transfer of controls to and from the bridge wings in accordance with manufacturer requirements.

Non-metallic conduit on the aft end of Port Diesel Particulate Filter melting during operation. Verify cause of the melting and implement a manufacturer approved repair/correction. Deficiency must be rectified prior to departure.

Vessel operator notified USCG that all I.B.A.s will be removed from vessel for annual servicing. To continue to operate, the vessel must: a). Be restricted to Lakes, Bays and Sounds route inside the San Diego Bay and not cross the boundary line in accordance with 46 CFR 7.120(a). b). While transiting inside San Diego Bay, cannot be more than one (1) mile from land in accordance with 46 CFR 117.207. Vessel may return to normal routes in accordance with their C.O.I. upon return of all I.B.A.s from the service facility.

The exhaust system on the starboard auxiliary engine including the DPF ingested a significant amount of sea water. As a result, the filter system is no longer functional and will need to be replaced to the satisfaction of the Coast Guard. The ship operator must continue to monitor the back pressure monitoring system and maintain engine operation within the manufacturers recommendation while permanent repairs are made to the system. All failures/safety infractions of the system must be immediately reported to the Coast Guard.

Non-metallic conduit on the aft ends of both Diesel Particulate Filters (DPF) began melting during operation. Verify cause of the melting and implement a manufacturer approved repair/correction. Deficiency must be rectified prior to operation of either engine fitted with a DPF. Since the vessel will not be able to power vital systems from a secondary source of power, this deficiency must be rectified prior to departure. Emergency lighting failed in the following locations: in the vicinity of (IVO) Fire Stations #2,#3, #4, #5, and the galley. Repair or replace the identified emergency lighting installations. Vessel may only operate during daylight hours until this deficiency is rectified. Cite: 46 CFR 184.30-5 (Old T)

Raw water overboard pipe on forward generator had a leaking patch. Complete permanent repairs to this pipe.

Several improperly terminated wires found above the ceiling tiles IVO the port cargo door. Properly terminate these wires either with wire nuts in a junction box or with pressure type connectors. Cite: 46 CFR 183.05-50(b) (Old T)

Electrical equipment and installations must be suitable for the roll, pitch, and vibration of the vessel underway. 03 junction boxes identified during the inspection not secured. Mount J-boxes to prevent movement. Cite: 46 CFR 183.01-15(a) (Old T)

Junction box on upper deck aft of food service area found to be missing cover. Install new weather tight cover. Cite: 46 CFR 183.01-15(a) (Old T)

First aid kit found to be missing Ammonia inhalants and Aspirin. Re-stock first aid kits.

The aft-most air receiver in E/R was leaking air from the relief valve. Replace valve and perform a new test in the presence of a USCG marine inspector. Cites: 46 CFR 119.330 46 CFR 54.15-10(g)

A deficiency requiring the development and approval of a Qualitative Failure Analysis and Design Verification Test Procedures in accordance with 46 CFR 121.100(b), first issued in December of 2021, has not yet been satisfied. Additionally, the DPF system onboard suffered a casualty through an unanticipated failure mode which resulted in overheating components melting an electrical conduit and the filter media being damaged. Prior to movement of the vessel, DVTP's must be completed based off of a procedure approved by the Marine Safety Center to the satisfaction of the attending Marine Inspector. The DVTP's must be developed using a QFA which considers the failure modes outlined in MSC letters E1-2103519, comment 2, as well as the failure which was observed onboard during the recent casualty. Additionally, all outstanding comments from letter E2-2201566 must be resolved prior to shipboard testing.

Sector San [name withheld] has determined that the control system for the DPFs aboard ADVENTURE HORNBLOWER warrant additional review. Prior to operating the DPF system in the San Diego [name withheld], a QFA and DVTP must be developed and submitted to the MSC for review and approval, and DVTPs conducted to the satisfaction of the attending Marine Inspector.

Machinery Exhaust Gas must not leak. Forward generator exhaust leaks at the engine to piping system (near turbo).

A Class fire boundary. Port aft passenger cabin (02 level) fire door labeled "Smoke and Draft control requires class H gasket" but half the fire door gasket is missing, and appears to be weather strip.

Protection against hot piping. Piping over 150 degree must be insulated. Following sections need insulation: 3' on aft gen exhaust, 1' on foward gen elbox (in exhaust space), 1' on forward gen exhaust (through hull), and both Port and Stbd MDE at the engine room bulkhead penetration.

All cable and wire must be secured with metal supports every 24". Starboard DPF thermocouple wire 8' run is hanging in the DPF space.

Electrical junction box on top of pilot house found to not be secured to a stationary surface. All electrical equipment and installations must be suitable for the roll, pitch, and vibration of the vessel underway.

The Shutoff valve at the tank shall be operable from outside the tank compartment. Remote fuel shutoff was inoperable. Repair remote fuel shutoff valve. Cite OLD T 46 CFR 182.20-40 (b)(3).

All distress flares were expired. Provide 03 valid red hand flares and 03 valid orange smoke flares. 46 CFR 117.68

Bilge alarms indicated the incorrect spaces. Either rewire or relabel the bilge alarm panel so that the correct space is identified when a bilge alarm sounds. 46 CFR 119.530

Replace the bracket for the floating water-light tied to the ring-buoy on the port side of the pilothouse. 46 CFR 117.71(d)

Make identified bulkhead penetration in machinery space water tight. Wire run from sewage tank space to machinery space needs to be sealed to watertight standards with approved sealant. Penetrations of required watertight decks/bulkheads by any rigid plastic pipe systems are prohibited, except when using an acceptable metallic fitting, welded or otherwise attached to the bulkhead or deck by an accepted method and an acceptable metallic shutoff valve is installed adjacent to the through deck or through bulkhead fitting.

Plug identified hole in forward bulkhead of stair tower to machinery space. 46 CFR 116.415

Install a chain on the hatch entering the Lazzarette.

Remove debris in containment on aft deck. 46 CFR 147.45

Clean port side steering pump hydraulic containment. 46 CFR 147.45

Install proper lighting fixture guards for emergency lighting in liquor locker. 46 CFR 120.410

Open air receivers for internal inspection by USCG marine inspectors. 46 CFR 61.10-5

Replace the aft air receiver pressure gauge. 46 CFR 54.15-5

Remove dead end wiring under bridge in battery compartment and properly mount loose wiring. 46 CFR 111.60-5

Remove Port Propeller and aft section of Tailshaft for inspection.

Conduct Dye Penetrant testing of all Port shaft coupling bolts and keys.

Vessel suffered a loss of steering due to an unforseen failure. Make proper repairs and provide technician report indicating cause of failure, repairs completed and specific testing conducted to demonstrate unit is returned to proper operating conditions. Vessel is restricted from carrying passengers until deficiency is rectified. 46 CFR 58.25

Repair all leaks in the steering hydrulic system. Until the leaks are corrected, rounds of the hydrualic steering system must be performed hourly while the vessel is operating.

Repair structural fire protection in machinery spaces as identified.

Replace port and starboard rudder bearing races.

Repair bulkhead seals and deck penetrations as identified.

Complete satisfactory sea trials in the presence of a CG Marine Inspector.

Provide durable marking of at least 1" numbers and letters for upper deck passengers permitted IAW COI and currently stability letter. Ensure marking is posted at the entranceway to that deck.

Found one 50 person IBA expired for servicing. Vessel is restricted to 450 persons for limited coast wise route. Properly service.

Replace 02 shackles on 02 IBA's.

Provide appendices for current alternate security plan.

Prove proper operation on 04 fire doors.

Provide retro-reflective material on 05 adult sized life jackets.

Properly label #4 fire station.

Repair damaged coolant hoses on port MDE.

Prove proper operation of F/O shutoff valve.

Install captive chain on steering gear space hatch.

Replace broken window on starboard side main deck bow compartment.

Install approved window in accordance with vessel plans.

Properly install door in frame on main deck bow compartment.

Install handle on valve at fire station # 4.

Replace expired Hydrostatic Release Unit (HRU) on IBA Ser# L020300.

Properly install HRU for the following IBAs Ser# 11013910, P3725, P3724 by etching in accordance with manufacturer's instructions.

Post Employee Assistance Program hotline numbers in crew space.

Replace fire hose at station #3.

Install non-conductive mats or grating on deck in front of each switchboard.

Engine room: Properly install cable entering light switch box located at STBD side of engine room door.

Engine room: Properly hang/secure wires IVO starboard side FWD BLKHD under electrical panels.

Properly hang/secure engine control wire bundles above each engine.

Install missing fire insulation on overhead where required and missing. Primarily engine room and galley space.

STBD main engine raw water discharge pipe/hose: Renew section of rubber hose approved J1942 hose and fittings; additionally, the hose section shall be less than 30 inches. In the interim double hose clamp hose.

STBD shaft safety guard: Configure guard to provide complete protection from rotating shaft.

Port reduction gear is missing a ventilation grating: renew with proper grating.

Galley space: Repair broke wire hanger bracket (broke weld at blkhd.)

Galley range hood vent ducting needs thorough cleaning.

There is an electric bilge pump, located in STBD aft galley space, that needs to be permanently wired utilizing proper marine cable/wire.

Properly install compass light.

Remove oven in snack bar.

Improve general welfare in E/R. 46 CFR 115.800.

Replace missing valve on E/R fuel manifold.

Remove (2) two space heaters from bridge, and all other spaces. 46 CFR 121.200.

Cover exposed GFCI in pantry.

Replace light guards in pantry.

Label mission circuits in snack bar.

Replace missiong auto closure on door leading to E/R.

Properly mount fire extinguisher in bar area.

Replace line connecting lifering light to lifering.

Prove proper operation of port MDE coolant system.

Replace missing IBA. Vessel restricted to 450 total people on limited coastwise route.

Make repairs and demonstrate proper operation of the vessel generators to the satisfaction of the attending marine inspector.Generator operation failed due to cooling system sensor failing.
