All failures/safety infractions of the system must be immediately reported to the Coast Guard.

A deficiency requiring the development and approval of a Qualitative Failure Analysis and Design Verification Test Procedures in accordance with 46 CFR 121.100(b), first issued in December of 2021, has not yet been satisfied. Additionally, the DPF system onboard suffered a casualty through an unanticipated failure mode which resulted in overheating components melting an electrical conduit and the filter media being damaged. Prior to movement of the vessel, DVTP's must be completed based off of a procedure approved by the Marine Safety Center to the satisfaction of the attending Marine Inspector. The DVTP's must be developed using a QFA which considers the failure modes outlined in MSC letters E1-2103519, comment 2, as well as the failure which was observed onboard during the recent casualty. Additionally, all outstanding comments from letter E2-2201566 must be resolved prior to shipboard testing.
