Each lifebuoy must be marked in block capital letters with the name of the vessel and the name of the port required. One lifebuoy was missing the required markings and was missing retroreflective tape.

Where fitted, it shall be the duty of the master to see that the emergency lighting and power systems are operated and inspected at least once in each week. 04 emergency lights were found inoperable. Conduct necessary repairs.

Each life raft must be arranged for float-free launching. The arrangement must ensure that the life raft or life rafts, when released and inflated, are not dragged under by the sinking vessel. Hydrostatic Release Unit was installed incorrectly which had the painter line secured to the cradle. Training was provided to the crew and the HRU was installed correctly during the inspection.

It shall be the duty of the owner, master, or person in charge to see that the vessel's firefighting equipment is at all times ready for use and that all such equipment is provided, maintained, and replaced as indicated. CO2 system for the bow thruster room was recently serviced; however, 01 hoses was found split in numerous places. Conduct necessary servicing of the CO2 system.

Vessel that operates in fresh water at least six months in every 12 month period since the last drydock must be examined in accordance with intervals set inside the table. Vessels COI shows ISE next exam date 20APR2023. Vessel is authorized to off load at St. Marys Cement dock and is free to transit. No further cargo operations are authorized intil ISE is completed or an ISE extension request is approved.

Approved portable fire extinguishers must be installed in accordance with Table 95.50-10(a) of this section. Several extingushers were not on board due to them currently being serviced.

All safety and relief valves for use on pressure vessels or piping systems shall be designed to meet the protection and service requirements for which they are intended and shall be set to relieve at a pressure which does not exceed the "maximum allowable working pressure" of the pressure vessel or piping system. The SRV for the pressure vessel in the bow thruster space relieved at 10 psi lower than set point and the SRV for the main deck tank relieved at 10 psi higher than set point.

CO2 portable extinguisher in Green Bay Deck Gen enclosure needs annual service.

Mount liferaft on forecastle deck for automatic release.

Gen Deck House port and stbd WTD entry need adjustments.

Dust collector M/C amp metter needs to be replaced.

In stbd fwd cement tunnel area outside of control room, repair or replace EM light fixture.

Green Bay Deck Gen enclosure ventilation hatch is missing hardware on studs to secure properly.

In cement control room, remove stowed items behind the SWBD.

Provide 1 Type I lifejacket to meet COI.

Cummins 6.7 service Genset is currently being overhauled. Complete servicing with CAT/Cummins. Provide evidence of L/O psi, High temp, Overspeed, reverse power relay testing or test for local OCMI prior to putting Genset back in normal service.

Green Bay unloading Genset cat 3512 is currently being serviced with CAT. Provide evidence of L/O psi, High Temp, Over speed, and reverse power relay testing or test for local OCMI prior to putting Genset back in normal service.

Remote valves for High and Low suction of main fuel oil tank are not operational. Repair both valves and test remote operation or both to satisfaction of local OCMI.

Multiple Battle lantern/emergency lights not working. Make repairs and provide MI’s with total count of DC lights onboard.

Life ring up forward on STBD side needs retro tape and water light is not operational.

PORT side running light has issue with lamp changing device. Make repairs and verify STBD side is operating as intended prior to vessel movement.

On 11AUG20 the vessel grounded just outside the navigation channel IVO the Port of Green Bay. The vessel's bow section grounded and the vessel became stuck and unable to free itself. As a result of the grounding the vessel is to have a damage survey conducted by it's Recognized Classification Society. Results of this survey and any resulting conditions of class are to be reported immediately to MSD Sturgeon Bay. If the survey reveals that there is no damage and there are no conditions of class the vessel may have this 835 cleared and signed off on by the attending Class Surveyor. Please contact the below Inspector with the damage survey findings.

Bilges in machinery spaces must be free of oil accumulation which might create a fire hazard. Bowthruster space bilge has 3" of oil present. Remove oil from bilge to the satisfaction of attending Marine Inspector prior to departure.

Load line markings and reference line shall be painted white or yellow on a dark background or in black on a light background. Load line as reference line are same color as hull. Paint markings a contrasting color to hull to the satisfaction of attending Marine Inspector.

If a vessels COI is renewed less often than annually, the Marine Inspector must check the settings of the safety or relief valves on all pressure vessels twice every 5 years, no more than 3 years elapse. Relief valve on pressure vessel located on tool deck would not operate properly. Prove operation of relief valve or work with ABS Classification to remove pressure vessel to the satisfaction of attending Marine Inspectors.

Each light fixture must have a globe, lens, or diffuser of high strength guard or be made of high strength material. Forward store room by anchor on the port side of vessel is missing diffuser. Repair/replace diffuser/fixture to satisfaction of attending Marine Inspector.

Emergency lighting and power systems shall be in proper operating condition. Emergency lighting in tool deck space, at aft of cargo tunnel, and forward of cargo tunnel found inoperable. Repair/replace to the satisfaction of the attending marine inspector.

As noted in CVC Policy Letter 16-04, vessel required to carry logbook. Logbook not found on vessel. Provide documentation to the satisfaction of the attending marine inspector.

All lifesaving equipment must be arranged IAW sub chapter W. Life raft not arranged to float free with current hydrostatic release installment. Repair/replace to the satisfaction of the attending marine inspector.

Machine Aux Controller Group has 2 motor controllers cabinets not labeled and one is missing cover for run light indicator.

Electrical cord for mounted drill press was chaffed and exposing cable conductors.

Hydraulic oil head tank for Bowthruster has loose pipe fittings and is leaking down assembly and onto the deck.

Machine Space [name withheld] P101 had breaker #15 completed missing from panel exposing buss work.

Found dead ended cable hanging in workspace/shop area.

A motor controller cabinet located at MCC #3 is not labeld.

On deck generator shed light fixture in overhead is missing its globe, cage, diffuser, cover.

Reattach pipe hanger for sounding tubes in #3 Stbd, #4 P & S, #5 P & S ballast tanks.

Several emergency lights (automatic battery operated lanterns) not illuminating: Control room, center landing between ballast pumps, cargo tunnel slide locations 16, 18, 37, & 47. Prove proper operation of all emergency lighting.

Emergency light missing at emergency escape trunk aft port side of cargo tunnel. Install and prove proper operation of emergency lamp at escape trunk.

Properly label remote operated fuel shutoff valve for on deck cargo unloading generator.

Suitable shields are to be fitted in way of the flanged joints in the fuel fill piping in vicinity of valve above "at sea" generator.

Prove proper operation of forward bilge high level alarm.

The weekly and quarterly inspection checklists were improperly filled out. They were copies of a previously filled out checklist; there were no original signatures. Properly fill out each checklist and sign with original signature.

Renew hydrostatic release attached to the liferaft located on the bow. Release expired on Jan2015.

03 Lifebuoys do not have proper retro-reflective tape, name of the vessel and homeport as required. 46 CFR 199.70

Install a remote fuel oil valve outside main deck generator fuel oil tank which serves the temp main deck engine/air compressor IAW 46 CFR 50-60.

Submit Equipment Specifications for the engine/air compressor removed from within the forward portside cabin.

Install glass globes and light fixtures cages on the lighting fixtures within and above the main deck generator (box).

Submit proposal which addresses installing a new engine/air compressor unit that serves mooring, deck and anchor winches.

Install a shaft guard for the bow thruster propulsion shaft in the bow thruster space where crew is required to step over the propulsion shaft. 46 CFR 58.01-20 and 46 CFR 92.25-15.

Provide a protective guard over the engine flywheel of the stbd side generator. Currently this engine has no protective guard. 46 CFR 58.01-20 and 46 CFR 92.25-15.

Install a hand rail where the hand rail was removed at the stbd ladder in the unloading tunnel just forward of the cargo control room. 46 CFR 92.25-15

Service the Portable B-II fire extinguisher in the Generator Conex Box installed on deck. The current Fire Extinguisher was serviced in February 2007. NFPA 10, 46 CFR 95.25-20 and 46 CFR97.15-60.

Replace the fuel hoses to and from the racor filters in the deck generator box (house). Current hoses do not meet SAE J-1942 standards. As an alternative, approve fire sleeves can be installed over the current hose to meet SAE fire standards. Fire sleeves are to be installed in accordance with the manufactures instructions. 46 CFR 56.60-25 (b).

Replace all fuel and oil hoses on the bow thruster that do not meet SAE J-1942 stanards. Current hoses do not meet the SAE J-1942 standard. As an alternative, approve fire sleeves can be installed over the current hoses to meet SAE fire standards. Fire sleeves are to be installed in accordance with the manufactures instructions. 46 CFR 56.60-25 (b).

Replace all fuel and oil hoses on all generators and machinery located in the forward machinery space. Current hoses on the generators and hydro pump do not meet SAE J-1942 standards. As an alternative, approve fire sleeves can be installed over the current hoses to meet the fire standard. Fire sleeves are to be installed in accordance with the manufactures instructions. 46 CFR 56.60-25 (b).

Stow all Oxygen, Acetylene, Propane bottles in approved upright racks in an approved stowage locker. 46 CFR 147.60(b). Currently these cylinders are stored in machinery spaces and in the cargo unloading tunnel.

Install non-conductive mating behind the forward main electrical switchboard/panel. 46 CFR 111.30-11

Repair or replace the leaking fuel valve on the bow thruster fuel day tank. 46 CFR 56.50-75 and 46 CFR 56.50-60.

Provide proof of satisfactory air & floatation testing of the five (5) immersion suits that are older than 1996. 46 CFR 199.273 and NVIC 01-08

Provide embarkation ladders for the forward 6 man liferaft. 46 CFR 199.30 and 46 CFR 199.110

Provide hand rail or chain for the portside forward deck just aft of the bulwarks. 1966 Load Lines and 46 CFR 92.25-5.

Remove all flammable liquids in excess of 5 gallons (e.g. Kerosene, propane, oxygen and acetylene) from the forward machinery space and the cargo unloading tunnel. Remove all combustible liquids in excess of 55 gallons from the forward machinery space. Flammables and combustibles to be removed prior to any vessel operations. 46 CFR 147.45

Make barge available for Credit Drydock and Internal Structural exam. O/O failed to comply with the provisions of the Drydock extension letter issued by the Ninth Coast Guard District dated March 7, 2007. As such vessel was operated out of compliance with its certificate of inspection as of July 1, 2007. See MISLE activity 2903018 and scaned documents for further details.

Renew all fuel and oil line hoses with approved hose of no more than 30" inlength.

Renew fractured air compressor lube oil temperature regulator housing on diesel air compressor.

Re-route fuel and lube oil lines in vicinity of exhaust system on both bow thruster engine and air compressor diesel engines.

Prior to sailing, and until a fire protected flammable storage locker is installed, remove all paint, thinner, and flammable liquids from the vessel.

Provide evidence of ABS approval of required vessel plans as discussed withOCMI Chicago, IL.

Prove proper operation of remote automated release of STBD anchor.

MAKE REPAIRS TO #2 BULKHEAD, PORT SIDE, WHERE CRACKED (CORNER BRACKET AND BULKHEAD BELOW 5TH LONGITUDINAL FROM MAIN DECK). REPAIRS TO BE MADE IAW PROPOSAL APPROVED BY ABS, AND TO THE SATISFACTION OF THE ATTENDING MARINE INSPECTOR.
