No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection.[b] Drawings of alterations shall be approved before work is started unless deemed unnecessary by the OCMI. Independent tanks supported by three or more saddles contribute to the strength and stiffness of the hull. In such case, the hull stress shall not exceed the percentage stress. Marine Inspectors attended vessel for ISE and Periodic and noted significant crushing of three (3) saddles. Renew/repair as original to the satisfaction of the attending Marine Inspector.

The respective piping systems installed on ships and barges shall have the necessary pumps, valves, regulation valves, safety valves, relief valves, flanges, fittings, pressure gages, liquid level indicators, thermometers, etc., for safe and efficient operation of the vessel.Marine Inspectors attended vessel and noted flange for transfer manifold had no gasket and was only secured by two bolts. Renew/repair as original to the satisfaction of the attending Marine Inspector.

Scantlings, material, and workmanship, subdivision of cargo spaces, fitting of cofferdams, and testing of tanks shall be at least equivalent to the requirements of the American Bureau of Shipping or other recognized classification society. Marine Inspectors attended vessel and noticed at least 7 broken welds within the stern rake of the vessel. Renew/repair as original to the satisfaction of the attending Marine Inspector.

Scantlings, material, and workmanship, subdivision of cargo spaces, fitting of cofferdams, and testing of tanks shall be at least equivalent to the requirements of the American Bureau of Shipping or other recognized classification society. Marine Inspectors attended vessel and noticed at a portion of the deck plating was bubbling when pressure was applied. Renew/repair as original to the satisfaction of the attending Marine Inspector.

The respective piping systems installed on ships and barges shall have the necessary pumps, valves, regulation valves, safety valves, relief valves, flanges, fittings, pressure gages, liquid level indicators, thermometers, etc., for safe and efficient operation of the vessel. Marine Inspectors attended vessel and noted excessive corrosion on the drainage piping. Piping must be hydro statically tested to ensure compliance with CG regulations.

Internal structural examination means an examination of the vessel while afloat or in drydock and consists of a complete examination of the vessel's main strength members, including the major internal framing, the hull plating, voids, and ballast tanks, but not including cargo or fuel oil tanks. Marine Inspectors attended vessel to conduct ISE but could not complete inspection due to thick layer of mud and debris covering the bottom plating and lower portion of collision bulkhead of the vessel. Vessel must be cleaned and ready for inspection to the satisfaction of the CG.

MAKE PERMANENT REPAIRS TO CENTER STARBOARD CARGO TRANSFER MANIFOLD PRIOR_x001C_ TO LOADING NEXT CARGO BUT NOT LATER THAN 06MAY94.

REINSTALL THE DRIP PAN FOR THE STARBOARD SIDE LOADING HEADER.

MAKE PERMANENT REPAIRS TO THE CARGO PIPING ON THE STARBOARD SIDE AMIDSHIPS.

PRIOR TO CONDUCTING ANY CARGO OPERATIONS BUT NOT LATER THAN COMPLIANCE DATEHAVE VESSEL'S CARGO PIPING INSPECTED BY A COAST GUARD MARINE INSPECTOR AND PROVIDE EVIDENCE OF A 1 1/2 MAWP HYDROSTATIC TEST OF SAME.

PRIOR TO CONDUCTING CARGO OPERATIONS BUT NOT LATER THAN COMPLIANCE DATE HAVE VESSEL'S ELECTRIC CARGO PUMP INSTALLATION AND ASSOCIATED WIRING INSPECTED BY A COAST GUARD MARINE INSPECTOR.

PRIOR TO CONDUCTING CARGO OPERATIONS BUT NOT LATER THAN COMPLIANCE DATE PROVE PROPER OPERATION OF THE CLOSED GAUGING DEVICES ON EACH CARGO TANK TO THE SATISFACTION OF THE COGNIZANT OCMI.

MAAKE PERMANENT REPAIRS TO #1 STBD WING TANK AND STERN BOX. MAINTAIN TEMP. REPAIRS UNTIL COMPLETE PERMANENT REPAIRS ARE MADE. REPAIRS TO BE COMPLETEDTO THE SATISFACTION OF THE ATTENDING CG MARINE INSPECTOR, NOT LATER THAN COMPLIANCE DATE.

MAKE PERMANENT REPAIRS TO WASTED AREAS IN SIDE SHELL AT DECK KNUCKLE IN WAYOF STBD CARGO HEADER DRIP PAN. TO BE COMPLETED TO THE SATISFACTION OF THE ATTENDING U.S. COAST GUARD MARINE INSPECTOR.

MAKE PERMANENT REPAIRS TO SHORE TIE SUPPLY RECEPTACLE AND INSTALL PROPER WATERTIGHT COVER WHICH IS TO BE UTILIZED WHEN RECEPTACLE IS NOT IN USE I.A.W. 46 CFR 111.79-5.

UPON INSTALLATION OF PORT & STBD CARGO PUMPS AND ELECTRIC DRIVE MOTORS AND PRIOR TO UTILIZING FOR CARGO TRANSFER OPERATIONS CONTACT COAST GUARD FOR INSPECTION OF INSTALLATION.

PRIOR TO UTILIZING CARGO TRANSFER SYSTEM MAKE PROPER REPAIRS TO ELECTRICAL SYSTEM CONDUIT CONNECTIONS. ALL CONNECTIONS SHALL BE MADE WATERTIGHT I.A.W. ELECTRICAL REGULATIONS.
