All portable fire extinguishers, semi-portable fire extinguishing systems, fire-detection systems… must be inspected or tested at least once every 12 months… There was no evidence onboard that the fire detection system had been inspected within the past 12 months. Hourly rounds must be conducted while the vessel is operating to check for fire until the fire detection system is inspected by an approved fire inspection agency. 46 CFR 142.240(a)

Towing vessels must be equipped with the following equipment…Illuminated magnetic compass or an illuminated swing-meter (Western Rivers vessels only). The compass or swing-meter must be readable from each operating station. Compass not installed at operating station. 46 CFR 140.725 (d)

Each lifejacket must have a lifejacket light approved under approval series 161.012 or 161.112 securely attached to the front shoulder area of the lifejacket. The lights on all the lifejackets were expired. 46 CFR 141.340 (g)(1)

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel… Coolant was leaking from the starboard aft coolant expansion tank located above the starboard main diesel engine. 46 CFR 143.205 (a)

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Portable, non-marine-grade space heaters and a power strip were in the pilothouse. Remove these items. 46 CFR 143.205 (a)

Means must be provided…for closing, in case of fire, each doorway, ventilator, and annular space around funnels… The closures on the outside of the funnels were not airtight when closed. 46 CFR 144.610

The emergency duties and duty stations…must be posted at each operating station and in a conspicuous location in a space commonly visited by crewmembers. The muster list was not posted in the pilothouse. 46 CFR 140.405 (c)

Sufficient lighting suitable for the marine environment must be provided within crew working and living areas. Multiple light fixtures were missing their protective covers in the engine room. 46 CFR 143.410 (a)

Regardless of the option chosen to obtain a COI, upon obtaining a COI each towing vessel must then undergo a dry dock and internal structural examination... A. In forward rake, replace all vertical truss frames that are bent. B. In starboard side forward rake, weld upper transverse frames 1, 2, and 3, that are not welded to deck. C. In center fuel tank on the port aft side, crop and renew inset. D. In port and starboard fuel tanks on the outboard aft section on bilge turn, grind and re-weld insets. E. In starboard aft lazarette, re-weld two gussets. F. In port aft lazarette, replace stuffing tube packing. G. Install shut off valve on all through hull keel cooler penetrations. H. Fire main sea suction, remove overhead drain piping and cap to prevent water intrusion. I. Replace all pressure regulator valves on air compressor. J. Signs of exhaust leak on port and starboard generators. Repair to satisfaction of marine inspectors.

Vessel has not received annual inspection as required for the Coast Guard option.

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Port gen was not working. Repair and test for proper operation as per 46cfr143.400

A towing vessel must carry lifebuoys and be marked in block capital letters with the name of the vessel. The name of vessel is fading on lifebuoys. Correct as needed per 46cfr141.360(4)(b)(4)

Closure devices must be provided for deckhouse or hull penetrations, which open to the exterior of the vessel and which may allow water to enter the vessel. The door on STBD side had a cracked window. Repair as needed per 46cfr144.320

Individual circuit breakers on switchboards and distribution panels must be labeled with a description of the loads they serve. Label missing or faded on some electrical panel breakers. Label each breaker with correct description. As per 46cfr143.400(d)

Each device must have a placard suitable for posting on which is printed the operating instructions, safety precautions, and warnings pertinent to the device. The size of the letters printed on the placard must be one-eighth of an inch or larger. MSD was missing this placard, add per 33cfr159.59

Each towing vessel must be maintained and operated so as to minimize fire hazards. Exhaust fans in engine room are dirty with black soot, clean as necessary. Per 46cfr142.220

Vessel is missing hailing port on stern. Label hailing port on stern as per 46cfr67.123.

Means must be provided for stopping each fan in a ventilation system serving machinery spaces and for closing, in case of fire, each doorway, ventilator, and annular space around funnels and other openings into such spaces. Shut off is located in engine room and not easy to shut off during engine room fire. Install a emergency shut off for ventalation fans to eliminate possible injury to person in event of fire. Per 46cfr140.510

Fire detection system did not have PE letter on board. Vessel missing PE letter. Owner/operator locate and provide approval PE letter for installed fire detection panel. Per 46cfr142.330(a)(1)

PORTABLE FIRE EXTINGUISHER REQUIRED IN MACHINERY SPACE: ONE TYPE B-II SHALLBE CARRIED FOR EACH 1000 B. H. P. OF THE MAIN ENGINE. NEED AT LEAST ONE B- II PLACED IN THE ENGINE ROOM FOR 1200 B. H. P. ON THE MAVRICK.

TWO ELECTRICAL BOXES LOCATED IN THE ENGINE ROOM ARE MISSING THE DOORS THAT COVER THE FUZE SWITCHES. JUST NEED TO ADD THE COVERS TO THE BOXES. THIS ISA CAPTAIN OF THE PORT (COTP) REQUIREMENT.
