Founds numerous missed pick ups that had been identified during the initial survey, Two Side shell repairs in the port no.3 void one 26 x 28 x 1/2" was incomplete with open pits and poor fusion in the weld, the second 24 x 26 x 1/2" was also incomplete and missing weld continuity, in both welds the welder failed to remove built up slag and complete a final inspection prior to presenting for inspection.

Action required: 50 - Rectify deficiencies w/in 30 days

Except when otherwise considered necessary for inspection or pumping, all hatch covers and other hull closure devices for void spaces and hull compartments other than cargo spaces shall be closed and secured at all times. Deck of aft rake damaged on starboard quarter allowing void space to be compromised. Repair in accordance with NVIC 7-68.
Action required: 17 - Rectify deficiencies prior to departure

Resolution: MSU Huntington MI attened MRF in Catlettsburg, KY to verify repairs of the aft starboard timberhead. Repairs were satisfactory and 1 PSI soapy water test was also satisfactory. Cleared outstanding Def from MSU Pittsburgh.

During the pressure test of the bulkhead repairs between the No. 1 Starboard and box rake the weld repair was satisfactory but an additional crack was discovered via an air leak in the starboard centerline bulkhead under the no 1 cargo tank.
