At the U.S. Coast Guard's internal structural examination, all longitudinal watertight bulkheads in way of voids 1 through 6 were found altered and not watertight. In addition, concrete material was added to the cargo deck which modifies the vessel's lightship condition. Both alterations invalidated the vessel's February 17, 2011 stability approval issued by the USCG. This CG-835 supersedes the prior to departure (code 17c) issued on 26FEB2021.

If water or cargo is not present, these spaces need not be gas freed, ventilated, cleaned or otherwise prepared for personnel entry. The ladders in the forward rakes were found to be waisted. A few ladder rungs broke off during use. Safe access to the spaces/ voids must be provided. Voids 5 and 6 have not been inspected yet, due to the space not being certified by a Marine Chemist.
