Propulsion machinery shall be suitable in type and design for propulsion requirements of the hull in which it is installed. STBD engine in inoperable due to maintenance and repairs. Repairs to the STBD engine and sea trials required prior to carriage of passengers.

All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspection. (1)The light fixture in the port side engine space is not properly secured. Secure the light fixture. (2) An electrical connection in the engine room was secured using a wire nut. Repair the wire to the satisfaction of the Marine Inspector. (3) An open enclosure with exposed wire was identified in the starboard wheelhouse. Install a cover on the enclosure.

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. Dead end wire found in port outboard side of switchboard. Repair or remove wire.

Electrical equipment on a vessel must be installed and maintained to: (b) Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Insulation on the shore tie receptacle has been removed, and a combustible line was attached to it. Insulation must be continuous up to the fitting.

Acid batteries shall be located in a tray of lead or other suitable material resistant to deteriorating action by the electrolyte. Excessive corrosion is present on the port side battery box, and the inboard support has been cut, rendering the support ineffective. Clean the battery space and repair or replace the battery box as necessary.

In systems and applications where flexible hoses are permitted to be clamped. Two clamps must be used on each end of the hose, or one hose clamp can be used if the pipe ends are expanded or beaded to provide a positive stop against hose slippage. Install double hose clamps on all required hoses on the port engine to comply with applicable standards and ensure safe operation.

Piping, including valves, pipe fittings and flanges, conveying vapor, gas, or liquid, the temperature of which exceeds 150 F, must be suitably insulated where necessary to prevent injuries. Install appropriate exhaust lagging on port and stbd side aft bulkhead of engine room and port side of shaft compartment.

All vessels shall be provided with a satisfactory arrangement for draining any watertight compartment other than small buoyancy compartments under all practical conditions. Secure the bilge pump to the deck to ensure safe and reliable operation as per manufacturer.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date:(1) Charts of large enough scale to make safe navigation possible. Vessel did not have updated navigation charts.

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Placard for model # 8830A missing, post placards.

Where life preservers are stowed so that they are not readily visible to passengers, the containers in which they are stowed shall be marked "Life Preservers" and with the number contained therein, in at least 1- inch letters and figures. This legend should indicate the separate stowage of children's life preservers. Label child lifejackets storage space.

The general construction of the vessel shall be such as to minimize fire hazards insofar as reasonable and practicable. MI identified several areas overhead in the engine room missing essential Structural Fire Protection. Adequately address the SFP. 46 CFR 177.10-5 OLD T

...each life jacket must be approved in accordance with either §§ 160.002, 160.005, or 160.055 in subchapter Q of this chapter, or other standard. MI observed approximately 73 life-jackets with unsatisfactory retro-reflective tape. Repair/replace. 46 CFR 117.71 (c)

The intent of this part is to insure that each vessel is provided with safe, readily available and sufficient firefighting equipment of the type necessary to extinguish any fires which are likely to be encountered in normal operation.” No firefighting equipment service records were on board to review. Provide records. 46 CFR 181.01-5(a)OLD T

A vessel of more than 19.8 meters (65 feet) in length must carry a minimum of three life buoys of not less than 610 millimeters (24 inches) in diameter. MI observed 01 life buoy missing. Replace within 7 days. 46 CFR 117.70(a)(3)

A vessel on a lakes, bays, and sounds, or rivers route must carry:(1) Three hand red flare distress signals approved in accordance with § 160.021 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.121; and(2) Three hand orange smoke distress signals approved in accordance with § 160.037 in subchapter Q of this chapter, or other standard specified by the Commandant. Vessel had 03 expired orange flares w/o any other viable options onboard. Ensure vessel had required amount of flares on board within 7 days. 46 CFR 117.68 (b)(1)(2)

On a vessel on which the number of passengers permitted on upper decks is limited by stability criteria, as indicated by the vessel's stability letter, the maximum number of passengers allowed on an upper deck must be indicated by a durable marking of at least 25 millimeters (1 inch) numbers and letters at the entranceway to that deck. Max pax of 288 was not marked on 2nd deck. Rectify with signage w/in 7 days. 46 CFR 122.602 (f)

An exposed hazard, such as gears or rotating machinery, must be protected by a cover, guard, or rail. MI observed exposed gears/rotating machinery on port side generator. Replace within 7 days. 46 CFR 116.960

All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspection.” MI identified several lights on the switchboard in the engine room are extinguished. Ensure all lights work as intended. 46 CFR 183.01-15 OLD T

All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspection. During high level bilge alarm tests, the shaft void and engine room high level alarms failed to illuminate or alarm at the bridge. Ensure all high level bilge alarms are functioning within 30 days. 46 CFR 183.01-15 OLD T

Each life jacket carried on board the vessel must be marked in accordance with 122.604 of this chapter. There were no signs or labels posted at stowage location for child life jackets. Ensure child life jackets are adequately labeled within 30 days. 46 CFR 117.71 (e)

The intent of this part is to insure that each vessel is provided with safe, readily available and sufficient fire fighting equipment of the type necessary to extinguish any fires which are likely to be encountered in normal operation.. MI observed noticeable leak on fire pump in engine room. Address leak within 30 days. 46 CFR 181.01-5 (a)OLD T

All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspection. MI observed a junction box for the ventilation fan located in sewage tank space missing cover. Additionally MI identified an obstruction in the ventilation fan. Address both the cover and obstruction within 30 days. 46 CFR 183.01-15 OLD T

The intent of this part is to insure that the main propulsion machinery installed on each vessel is suitable for the vessel and its operation…” During testing, the starboard main engine was unable to be secured from the pilot house. Ensure the starboard ME can be secured as designed. 46 CFR 182.01-5 (a)OLD T

Switches other than those mounted on the switchboard shall be of the enclosed type.” MI identified start batteries for the Main Engine located outside of a switch box or panel in the engine room. Ensure the batteries are adequately addressed. 46 CFR 183.05-15 OLD T

The intent of this part is to insure that the scantlings, material, fastenings, workmanship and arrangement of each vessel are such as to provide for safe operation in accordance with the terms of its certificate of inspection.” MI identified chunks of missing fixed ballast underneath the sewages takes, with liquid in the bilge. Remove the liquid for further CG inspection. 46 CFR 177.01-5 (a)OLD T

Vessel is required one mate per COI and allowed to drop to a senior deckhand per routes and conditions; Senior deckhand list was not current and person identified as the "senior deckhand" during inspection does not have the required training and knowledge to serve that position currently as stated to Coast Guard Marine Inspector (CGMI).

Vessel must maintain current and up to date charts for intended voyage; vessel's charts were from Mar 2022, must get up to date charts.

Vessel navigational lights must meet UL 1104 and must meet proper arch of viability and distance; vessel's side light were of an unknown type & mounted wrong, the stern light has an unknown new LED bulb.

The maximum number of passengers permitted must not be more than that allowed by regulation, vessel has folding chairs throughout the main deck and second deck which does not meet fixed seating criterion; must have CGMI conduct measurements with vessel operator to determine correct passengers allowed by regulation.

The general construction of the vessel shall be such as to minimize fire hazards insofar as reasonable and practicable. Vessel must rectify the following electrical hazards. -Light switch with wasted cover and frayed wire in exhaust void. -Worn wiring in exhaust void must be replaced and secured with metallic clips IAW regulation. -All wire penetrations through bulkheads must be vapor tight

Seating accommodations for passengers are required to be provided when the maximum number of passengers has been determined using the fixed seating criteria in 176.01-25(b) of this sub chapter. Provide updated General Arrangement plan from Naval Architect to determine maximum passenger count. Vessel is restricted to carrying a maximum of 412 passengers until GA plan is provided and accurate passenger count is determined.

A power driven fire pump shall be installed on each mechanically propelled "L" vessel. Fire pump was not operational. 46 CFR 181.10-1(a) OLD T.

Firehose nozzle must be bronze or another material that has strength and corrosion resistance properties equal to those of bronze. Vessel's nozzle was plastic. 46 CFR 181.15-10(d)(2) OLD T

Portable fire extinguishers shall be installed and located to the satisfaction of Officer in Charge of Marine Inspections. 03 portable fire extinguishers were not installed properly. 46 CFR 181.30-10(b) OLD T

All piping, valves, and fittings shall be in accordance with good marine practices and suitable for the purpose intended. The main distribution pipe for the fire main downstream from the sea strainer was damaged. 46 cfr 181.15-5(c)

Repair windows as laid out in proposal. Clips required on widows being glued in without a metal frame.

Audio Gage engine room and aft void along the centerline to the third longitudinal frame both port and stbd. Request USCG attendance on day of audio gaging.

C/R 02 transverse web frames and 03 longitudinal frames in fwd void identified during inspection on 11Apr2023.

MI discovered a hole in the bulkhead between the machinary space and aft void space

Remove unused pipe that runs through the bulkhead between the tank room and the eng. room.

Emergency fuel shut off valve is easily accessible to passengers. Provide a mean to prevent passengers for pulling valve

MI received an anonymous tip that the vessel's hull was damaged while mooring to the pier. After responding, the MI found a hole in the hull of the vessel.

Machinery must be maintained in such a manner as to afford adequate protection from machinery failure. Starboard steering hydraulic cylinder leaking. Repair or replace hydraulic cylinder. Rectify w/in 7 days

Machinery must be maintained in such a manner as to afford adequate protection from personnel injury. Marine Inspector's gas meter alarmed upon entering shaft alley space. Rectify issue and provide a Marine Chemist Certificate certifying the space as gas free along with a casual analysis. Inform all crew of hazard.

Remove scaling from forepeak to permit safe operation of bilge intake

OWNERS AND OPERATORS MUST MAKE THE VESSEL READY FOR INSPECTION IN ACCORDANCE WITH THIS SUBCHAPTER IF ISSUANCE OF A COI IS DESIRED. PRESENTLY THIS VESSEL HAS MISSED ITS ANNUAL COI INSPECTION AND IS RESTRICTED FROM CARRYING >6 PASSENGERS FOR HIRE. THIS INFORMATION WAS BRIEFED TO THE VESSEL REPRESENTATIVE JAMES SILECCHIA WITH THE UNDERSTANDING THAT THE VESSEL WILL DISCONTIUE COMMERCIAL OPERATIONS UNTIL READY FOR INSPECTION. WHEN READY FOR INSPECTION, THE OWNER/OPERATOR SHALL SUBMIT AN APPLICATION FOR INSPECTION (FORM CG-3752) TO THE EMAIL PROVIDED BELOW PRIOR TO RETURNING TO COMMERICAL OPERATIONS.THE CONDITION OF THE VESSEL, ITS EQUIPMENT AND ANY ASSOCIATED DRY DOCK INSPECTIONS AS NEEDED MUST BE ACCEPTABLE TO THE COGNIZANT OCMI.

Adequate life jackets must be provided for all passengers.
