All access openings in bulkheads at ends of enclosed superstructures shall be fitted with doors of steel or other equivalent material, permanently and strongly attached to the bulkhead, and framed, stiffened and fitted so that the whole structure is of equivalent strength to the unpierced bulkhead and weathertight when closed. MI observed a crack at bottom clear opening lip on the "B" end watertight door leading to the engine room. Effect permanent repairs to RO. Submit RO report to USCG.

The respective piping systems installed on ships and barges must have the necessary valves for safe and efficient operation of the vessel. MI observed the bilge overboard discharge check valve installed backwards. Re-install valve correctly to the satisfaction of USCG MI.

A nonmetallic expansion joint must be replaced 10 years after it has been placed into service if it is located in a system which penetrates the side of the vessel and both the penetration and the nonmetallic expansion joint are located below the deepest load waterline. The Officer in Charge, Marine Inspection may grant an extension of the ten-year replacement to coincide with the vessel's next drydocking. MI observed a deterorated expansion joint on the A-end main engine jacket water expansion joint. Replace expansion joint to the satisfaction of USCG MI with joints meeting 46 CFR 56.60-1(b). MI could not determine age of all other expansion joints. Vessel rep is required to provide documentation of the age of all remaining expansion joints or propose plan to replace following visual inspection of all joints.

Retro-reflective material. The lifebuoys shall be fitted with retro-reflective material (tape) with a minimum of 5 cm (2 inches) width around the lifebuoy at four locations spaced equally around the circumference of the lifebuoy. Retro-reflective material shall be Type II material that is approved under 46 CFR 164.018. Marine Inspector observed red retro-reflective tape not approved under 46 CFR 164.018 on multiple life rings on deck. Further, the existing reflective material contained DOT-2 markings on it signifying Motor Vehicle Department of Transportation approved.

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Steel Vessel Rules (incorporated by reference, see 46 CFR 58.03-1), except as otherwise provided by this sub chapter. Vessel experienced reduction in propulsion due to over-speed sensor activation on B end Voith Unit. Affect permanent repairs and provide final classification Society attestation of completed repairs to USCG.

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Steel Vessel Rules (incorporated by reference, see 46 CFR 58.03-1), except as otherwise provided by this subchapter. Vessel experienced loss of propulsion due to over-speed sensor activation.

The bilge pumping system shall be capable of operation under all practicable conditions after a casualty. MI's observed the bilge manifold valve serving the "A" end compartment #2 difficult to operate by hand. The chief engineer had to utilize a pipe wrench to operate the valve. Vessel is to service all bilge valves to ensure valves operate freely by hand.

Each liferaft or group of liferafts must be arranged for float-free launching. The arrangement must ensure that the liferaft or liferafts, when released and inflated, are not dragged under by the sinking vessel. All seven (7) inflatable buoyant apparatuses' (IBA) painter and weak-links were shackled to the IBA bands and not connected to the IBA cradle or the vessel.

Each section of firehose must be a lined commercial firehose that conforms to UL 19. Identified 01 fire hose ruptured after testing of fire main. Replace hose.

Each vessel must carry lifejackets approved under approval series 160.155, 160.176 or 160.177. Each vessel must carry a lifejacket for each person on board, and in addition - (i) A number of lifejackets suitable for children equal to at least 10 percent of the total number of passengers on board must be provided, or such greater number as may be required to provide a lifejacket of suitable size for each person smaller than the lower size limit of the adult-size lifejacket. Identified 06 adult and 05 child life jackets in unsatisfactory condition. Replace lifejackets of an approved type and quantity.

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician. If the ship passes the inspection, the technician will issue a Communications Act Safety Radiotelephony Certificate. Attending USCG Marine Inspectors found the vessel's Safety Radiotelephony Certificate had expired on 08/10/2021.

Each vessel must carry a certificate with a valid endorsement while subject to the Bridge-to-Bridge Act. Attending USCG Marine Inspectors found the vessel's Bridge-to-Bridge certificate had expired on 08/10/2021.

All vessels except unmanned barges shall be provided with a satisfactory bilge pumping plant capable of pumping from and draining any watertight compartment. Vessel was not able to demonstrate satisfactory operation of the installed bilge pumping system. Demonstrate to the attending Marine Inspectors satisfactory operation of the bilge pumping system as designed.

All vessels except barges shall carry at least the minimum number of fire axes as set forth in Table 76.60-5(a). The vessel did not have two operational fire axes as required, due to the top of the handle separating from the heads of the axes. Add 02 operational fire axes to the vessel to the satisfaction of the attending Marine Inspectors.

Shutoff valves of a suitable type shall be installed in the fuel supply lines, one as close to each tank as practicable. Where fuel tanks are installed below the weather deck, arrangements shall be provided for the operating of all shutoff valves at the tanks from outside the compartments in where they are located,preferably from an accessible position on the weather deck. The t-handles used to secure the shutoff valves on the weather deck were not able to secure the valves. The surrounding rails/guards/bulwarks/bulkheads prevented the full movement of the t-handles from closing the valves. Furthermore, the t-handles were not labeled for specific valves, and were not universal in size. Provide a corrective action plan to provide appropriate sized t-handles for all weather deck shutoff valves, and have the t-handles suitably marked so crew can easily determine which t-handle can secure which specific emergency shutoff valve.

Doors in stairway enclosures and Main Vertical Zone bulkheads shall meet the following requirements: hold back hooks, or other means of permanently holding the door open, not subject to control station release, will not be permitted. Marine Inspectors observed installed metal hold backs (requiring manual release) on two A-60 doors at the entrance/exit to the salon on the A/B end of the vessel. Remove the metal hold backs so that the doors shall continue to operate as A-60 boundary self-closing doors.

Electric installations on vessels must ensure protection of passengers, crew, other persons, and the vessel from electrical hazards. Marine Inspectors observed the cables running into the anchor windlass junction box had damaged metal sheathing exposing the rubberized outer layer of the cables, as well as the conducting wires. Effect permanent repairs to the effected system to the satisfaction of the attending Marine Inspectors.

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Steel Vessel Rules. Marine Inspectors observed a jacket water leak off of the main diesel engine on the A-end of the vessel. Effect permanent repairs to the satisfaction of the attending Marine Inspectors.

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalant to the standards established by ABS. MI's arrived to conducted quarterly inspection. As part of the inspection, MI's requrested to get underway to observe operation of machinery and conduct drills. Due to malfunctions within the Voith system, the vessel could not get underway to demonstrate operations and conduct drills. Prior to carriage of passengers, vessel must prove proper system operation and conduct drills to the satisfaction of the attending MI.

Due to numerous recent failures of the Voith sytem rendering the vessel inoperable, provide repair proposal including timeline to rectify Voith steering actuator issue.

All piping shall be securely supported. MI's observed a broken pipe support on the firemain overboard piping allowing the piping to vibrate significantly at idle. Repair firemain pipe hanger to satisfaction of attending MI.

The installation of all lifesaving appliances and arrangements must be in accordance with the requirements of subchapter W of this chapter. Each rescue boat must be capable of being launched with the vessel making headway of 5 knots in calm water. Vessel experienced damage to the rescue boat davit, launching equipment, guard rails, and other critical equipment rendering the equipment non operational. All repairs are to be made to the satisfaction of a Coast Guard Marine Inspector. 46 CFR 77.06-01 46 CFR 199.160(a)

The material, design, construction, workmanship, and arrangement of main propulsion machinery and each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Steel Vessel Rules. Vessel reported an oil leak in the A end voith main seal. Vessel is to proceed to dry dock for repairs. Repairs are to be made to the satisfaction of a USCG Marine Inspector. 46 CFR 58-05.1

Noted two life rings did not have adequate solas retro reflective tape. Remove damaged existing tape and replace with solas tape.

One e-light(battle lantern type) was installed in the second deck passenger lounge did not work and required a new battery. Replace battery and test to the satisfaction of the attending marine inspector.

Marking / Labeling of fixed fire fighting system instructions missing, the inspection shall as to insure that the vessel, as regards to fire detecting and extingishing equipment is in satisfactory condition. 46 CFR 71.25-10

"B" end Voith system hydraulic leak. Possible cause a failed system seal. Make final repairs and provide tech report attesting to properly repaired and functioning voith system.

Modifications were made to existing PA system without plan approval. Submit plans for approval, and upon receipt of approved plans, make vessel available for inspection to the satisfaction of the attending Marine Inspector. [46 CFR 110.25-1(c)(9)]

Modifications were made to existing lube oil piping system (addition of preheaters for both MDEs) without plan approval. Submit plans for approval, and upon receipt of approved plans, make vessel available for inspection to the satisfaction of the attending MI. [46 CFR 56.01-10(c)(1)(xvi)]

Modifications were made to existing bathroom arrangement in salon area by removing existing male and female bathrooms and installing multiple individual bathrooms without plan approval. Plans have been submitted to MSC (hull branch) but have not been approved at this time. Obtain approved plans, and upon receipt of approved plans, make vessel available for inspection to the satisfaction of MI.

Two existing pilothuse AC units were replaced with larger units and an additional AC unit was installed for the crew lounge without plan approval. Submit plans for approval (to include at a minimum updated load analysis and 1 line diagrams). Upon receipt of approved plans, make vessel vailable for inspection to satisfaction of attending MI. In course of yard period inspection, attending MIs noted that existing insulation in overhead of both A & B end VSP rooms did not meet A-60 reqts as indicated on FCP. As part of agreement b/t USCG, ABS, & TXDOT, center section of insulation was replaced w/ A-60 insulation this shipyard period, & the remaining insulation will be replaced @ next scheduled dry dock & NLT 01OCT18.[46 CFR 72.05-10(g)]

Vessel reported issued with ship service generator #2 and has been taken out of service pending troubleshooting and repairs. Vessel is not authorized to carry passengers until repairs are made to the satisfaction of the attending CG Marine Inspector.
