Vessel is required to troubleshoot and affect permanent repairs to propulsion system and submit technicians report and class attestation of satisfactory repairs to USCG MI prior to movement.

At least one-half the total number of lifebuoys on the vessel must each be fitted with a self-igniting light approved under approval series 161.010. Float lights attached to applicable ring buoys were affixed by a quick disconnect clip. Float lights must be permanently attached to the ring buoy. Ensure float lights are permanently attached to the ring buoys. 01 float light located Port Side A-end was illuminating intermittently. Enact permanent repairs to the satisfaction of the attending marine inspector. CITE: 46 CFR 199.70(a)(3)(ii)

Each vessel must carry lifebuoys approved under approval series 160.150. Data plate for 01 ring buoy starboard side amidship was deteriorated; approval series could not be verified. Remove and replace defected ring buoy with an approved ring buoy. 05 ring buoys were fitted with unapproved retro reflective tape 'DOT-C2.' Enact permanent repairs to the satisfaction of the attending marine inspector. CITE: 46 CFR 199.70(a)

Each cable splice must be made in accordance with Section 6.11 of IEEE 45.8. Multiple cables improperly spliced in the horizontal wire bundle in the B-end #2 stern space. Enact permanent repairs to the satisfaction of the attending marine inspector. CITE: 46 CFR 111.60-19(b)

Each outlet or junction box must have a cover unless a fixture canopy, switch cover, receptacle cover, or other cover is used. Crank case sending unit junction box is missing a cover. Enact permanent repairs to the satisfaction of the attending marine inspector. CITE: 46 CFR 111.81-1(c)

Each power bilge pump must have the capacity to develop a suction velocity of not less than 400 feet per minute through the size of bilge main piping required by 56.50-50(d). MI's requested an op test of the engine room bilge suction utilizing the primary bilge pump. System was unable to pump out a 5 gallon bucket of water within a 5 minute time frame. Affect repairs to bilge system to the satisfaction of attending MI.

Test techniques must not simulate monitored system conditions by mis-adjustment, artificial signals, improper wiring, tampering or revision of the system unless the test would damage equipment or endanger personnel. Crew was unable to prove proper operation of generator low lube oil alarms without disconnecting the pressure sensor from the loop. Prove to MI proper operation of generator low lube oil alarms without tampering with the system.

All automatically or remotely controlled or monitored vital systems addressed by part 62 of this subchapter must be subjected to tests and inspections to evaluate the operation and reliability of controls, alarms, safety features and interlocks. Test procedures must be submitted to the Coast Guard for approval. Vessel is required to submit to local OCMI, periodic safety test procedures prior to next re-inspection.

All systems of a marine engineering nature shall be in accordance with the requirements of subchapter F including: Steering Systems While transiting eastbound to the Bolivar side Ferry Terminal, vessel reported a reduction in steering after the vessel's "B" end #02 steering pump became unresponsive. Vessel is authorized a one-time transit without passengers to the Galveston side Ferry Terminal. Verify the cause of the reduction in steering and enact permanent repairs to the satisfaction of a Coast Guard Marine Inspector. Cite: 46 CFR 77.03-1 Action Taken: 701C

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician. If the ship passes the inspection, the technician will issue a Communications Act Safety Radiotelephony Certificate. Attending USCG Marine Inspectors found the vessel's Safety Radiotelephony Certificate had expired on 08/10/2021.

Each vessel must carry a certificate with a valid endorsement while subject to the Bridge-to-Bridge Act. Attending USCG Marine Inspectors found the vessel's Bridge-to-Bridge certificate had expired on 08/10/2021.

Auxiliary machinery vital to the main propulsion system must be provided in duplicate unless the system is provided in independent duplicate. Vessel suffered a loss of 01 generator, which serves as part of the propulsion system. Cause of the casualty must be determined and repairs made to the satisfaction of a Coast Guard Marine Inspector prior to departure or carriage of passengers.

All portable fire extinguishers and semi-portable extinguishing systems must be maintained in accordance with NFPA 10. Portable extinguishers shall be installed in the bracket supplied by the extinguisher manufacturer. MI observed 03 portable CO2 extinguishers located in the engine room not in the approved brackets. They were secured into the bracket utilizing fabricated cage bolted to the extinguisher bracket. Replace existing brackets with approved brackets. 46 CFR 75.25-20(a) 16C

Reduction in RPM on the "A" side of the vessel to approximately 50%.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel. Vessel grounded with 79 passengers onboard and was stuck aground for approximately an hour and a half. It is unknown at this time if there is any damage to the underbody including shafts, propellers, or rudders. Have divers conduct assessment of the underside of the vessel and forward dive report for review.

Found three wtd in the engine room that did not properly seal. Failed light test. Possibly due to improperly sized rubber seal. Ensure all WTD meet minimum water tightness requirements.

The Gibb Gilchrest experienced a reduction in propulsion on the No. 2 Main. No. 2 Main will be serviced and fully operational prior to being placed back into service.

The "B" end steering tiller collar which secures to the rudder stock was found missing one nut and bolt assembly. Additionally, the two bolts were found missing on the cover plate to the same rudder stock. O/O given 14 days to installed approved securing bolts to the satisfaction of the attending marine inspector.

Noted ground detected on the E-Gen SB 120v ground detection indicator. Company electrician thinks is a fault with the circuit to the radar UPS units. Locate and correct ground fault to the satisfaction of the attending marine inspector or ABS surveyor.

The bulkhead deck separating the E-Gen room to the pilot house must be of a A-60 SFP. MI noted several pieces of A_60 insulation was missing from the overhead within the E-Gen room. Replace insulation to the satisfaction of the attending marine inspector or ABS surveyor.

Weather doors leading down into the machinery spaces and the watertight doors installed on the WTBH separating the below deck subdivision where found to not seal correctly. O/O to adjust and chalk test in the presence of a marine inspector the Watertight integrity of these doors.

Floating electric light attached to life rings on deck were found expired. Appropriate batteries have been order. Replace batteries with manufacture approved batteries as needed.

Loss of steering while switching from A end to B end.

Vessel experienced a failure of the MOV(resister) on the #1 MDG causing a loss of power to the #1 MDG.

Pilot house air condition was inoperable.

CO2 piping is not labeled or marked.

Vessel experienced loss of throttle control of "A" end engine. Restore full control of "A" end engine to the satisfaction of the vessel's classification society surveyor and the attending Coast Guard Marine Inspector.

Major malfunction of both main propulsion engines.

Failure of the engine overspeed device.

#2 MDE over-speed device inadvertently caused engine to come offline. Provide tech report attesting to the proper operation of the system.

Ships service generator trips off while underway, emergency generator comes online. Cannot shift load between ships service generators.

Vessel's main propulsion engines are not properly responding to throttle orders. Repair electronic throttle control and demonstrate proper operation to the satisfaction of the attending marine inspector.

Vessels Seawater cooling supply to the A & B end reduction gear was holed and leaking seawater.

Vessel reported an electrical issue on the #2 proplusion system due to a faulty SCR.

Vessel "B" end Audible alarms for bilge and doors not functional. Engine space must be manned at all times to monitor visual alarm indictors while operating.

Main propulsion engine failure. See activity for details.

Vessel grounded while approaching Bolivar Ferry Landing 4/5 on afternoon of 05APR. Vessel prohibited from carrying passsengers until CG MI attends and clears vessel.

#1 and #2 SSDGs failed to take and hold service load. #1 SSDG lost load causes unknown and the #2 SSDG over speed.

Main propulsion engine failure. See activity for details.

Time delay bottle (30 sec) for the system acutated at 26-27 seconds.

Shafts in motor rooms do not have adequate protection/guard in place to prevent personnel injury.

O/O reported a inoperative SSDG. Vessel is required to have two working SSDG and running in parallel mode while vessel is in ferry service. See act# 5182875 concerning parallel operations requirements.

A end Steer juction Box is missing the cover.

Found multiply pin hole leaks in the vessels reuduction gear and shaft cooling piping on both the A & B end that needs to be fixed. Vessel to make temp repairs by 21 SEP 14 until permanent repairs can be made in the yards.

Found the overboard piping at the skin of the hull for the Sea water cooling system for the reduction gear and shaft to be leaking. Issued CG-835 to make temp repairs to be completed by 21 SEP14 until pipping can be cropped and renewed in the yards.

Rudder was unable to maintain pressure at specific degree due to worn gasket. Vessel is to replace gasket and conduct sea trials prior to taking on passengers.

Vessel was found to have inoperable radar due to blown transformer.

Need to install IMO stickers IAW Fire Control Plan prior to next quarterly inspection.

Need to install sound powered and GIA-tronics phones IAW Fire Safety Plan by next credit drydock inspection.

Install the salon doors IAW 46 CFR 72.05(a)(2)

Complete periodic servicing of launching appliances and release gear for installed rescue boats iaw 46 CFR 199.190 (i)(1-5).

Due to loss of steering capability, prove proper operation of vessel's steering system by submitting a technicians report. Do this prior to operation.

Place one steering pump on the emergency switchboard prior to the end of the October 2011 drydock.

Open and inspect propulsion and generator prime mover fuel oil supply piping for restrictions Prior to the end of the OCT 2011 Dry Dock.

Covers on jackets were comprimised. (IAW 199.190)

Auxiliary generator aboard vessel flashed over while enroute to Bolivar Ferry Terminal. Problem did not effect imediate operation of vessel and was discovered while ferry was pierside in Bolivar. Vessel was sent to TxDot repair dock in Galveston, TX where tech rep determined that generator would need to be pulled and rebuilt or replaced. TxDot will submit action plan

Inspected fractured mooring block on the vessel's Bolivar end, port side. Discussed method of repair and met with ABS inspector while onscene.

Found engine room entrrance watertight door on Galveston end not properly sealed due to light around gasket when closed. Adjust and test water tight door.

Battery charger for engine room alarms is inoperable. Repair or replace battery charger.

Found sewage alarm inoperable. Repair or replace.

Found emergency lightbulb is out in L.O. compartment. Repair or replace.

Found no prevention slide bar that prevents shore power and ships power from being tied together. Install.

Found control panel lights on Bolliver side inoperable. Repair or replace.

Found nav light panel alarm/buzzer inoperable on Bolliver side. Repair or replace.

Found semi-portable extinguisher out of date for hydro test. Repair or replace prior to getting underway.

Combustible debris in bilge. Remove prior to getting underway.

MAKE PERMANENT REPAIRS TO THE #1 MAIN SEA CHEST PRIOR TO RETURNING TO PASSENGER SERVICE. THESE REPAIRS SHALL BE MADE TO THE SATISFACTION OF THE ATTENDING USCG MARINE INSPECTOR.

CONDUCT SATISFACTORY EMERGENCY DRILLS IN THE PRESENCE OF A COAST GUARD MARINE INSPECTOR. DRILLS TO INCLUDE; FIRE, MAN OVERBOARD, DEPLOYMENT OF ANCHOR, AND ABANDON SHIP. ALL DRILLS ARE TO BE HELD WITH A REGULAR U/W CREW.

PROVE WATERTIGHT INTEGRITY OF ALL WATERTIGHT DOORS, SCUTTLES, AND HATCHES.

PROVE WATERTIGHT INTEGRITY OF BOLTED ACCESS PLATES IN WAY OF THE FOLLOWING WATERTIGHT BULKHEADS; A. 44A, B. 33A, C. 33B, AND D. 44B

REPLACE FOUR RING LIFE BUOYS ON MAIN DECK.

REPAIR OR REPLACE TWO RING LIFE BUOY LIGHTS

REPLACE DOGGING WRENCHES FOR MAIN DECK SCUTTLES AND HATCHES.

REPLACE DETERIORATED HOSE ON CO2 HOSE REEL IN ENGINEROOM.

REPLACE SHORT BOLS IN SPLIT FLANGE COUPLINGS FOR HYDRAULIC HOSES TO HYDRAULIC RAMS IN STEERING SYSTEM.(TWO COUPLINGS EACH END)
