A survey, either general of partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this sub chapter. Vessel ran aground while transiting near Bolivar Ferry Terminal. A class (ABS) report attesting to the satisfactory structural integrity of the vessel is required. If a pipe, scuper, or electric cable passes through a main transverse watertight bulkhead, the opening through which it passes must be water tight. Marine Inspector found MCT installed improperly on the watertight bulkhead of A-End Engine Room. Provide evidence of proper installation. 16 AC code issued.

All portable fire extinguishers and semi-portable fire extinguishing systems must be maintained in accordance with NFPA 10, chapter 7. The CO2 portable fire extinguisher on the B-End Starboard side up forward has many cracks along the hose and needs to be replaced.. 46 CFR 71.25-20(a)(1)

Electric equipment must be arranged, as far as practicable, to prevent mechanical damage to the equipment from the accumulation of dust, oil vapors, steam, or dripping liquids. Electrical distribution and junction boxes in the center area of the engine room are excessively corroded having liquid dripping from a leaking pipe flange joint connection approximately 20ft. above, inside the exhaust trunk which needs to be repaired at the next dry dock (due 28OCT2024). The electrical box enclosures must be repaired or replaced as needed to ensure proper protection of internal connections. 46 CFR 111.01-3(a)

Metals in contact with each other must be either galvanically compatible or insulated with suitable non-porous materials. Provisions must also be made to prevent loosening or tightening resulting from differences of thermal expansion, freezing, buckling of parts, galvanic corrosion, or other incompatibilities. The rescue boat release mechanism has a shackle with a broken coder pin due to corrosion and the shackle bolt is completely corroded, which needs to be replaced with a bolt and coder pin of acceptable compatible material. 46 CFR 160.170-7(b)(5)

The material, design, construction, workmanship and arrangement of the main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be atleast equivalent to the standards established by the ABS Steel Vessel Rules. TXDOT reported an overspeed on the "B" end main engine. Vessel is to troubleshoot and affect repairs, provide technicians report and ABS report attesting to satisfactory repairs to USCG at [email withheld].

Each vessel under 12 meters must display an all-around white light. Rescue boat all-around white light was not working properly when tested. Replace light within 14 days.

Complete but simple instructions for the operation of the systems must be located in a conspicuous place at or near all pull boxes, stop valve controls and in the CO2 cylinder storage room. On systems in which the CO2 cylinders are not within the protected space, these instructions must also include a schematic diagram of the system and instructions detailing alternate methods of discharging the system should the manual release or stop valve controls fail to operate. Marine Inspector observed several labels were missing on parts of both, the Engine Room and Emergency Generator Room CO2 systems, and therefore not in accordance with the respective instructions and schematic diagrams. Additionally, Step 2 and Step 3 on the instructions for the Engine Room CO2 pull box station did not align with each pull handle labeled Step 1 and Step 2.

It shall be the duty of the owner, master, or person in charge to see that the vessel's firefighting equipment is at all times ready for use and that all such equipment required by the regulations in this subchapter is provided, maintained, and replaced as indicated. Marine Inspector observed posted instructions located in the engine room were for an older and different type of the semi-portable fire extinguisher than was present.

Prior to the carriage of passengers/cargo,make repairs and provide tech report to the satisfaction of the OCMI.

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician. If the ship passes the inspection, the technician will issue a Communications Act Safety Radiotelephony Certificate. Attending USCG Marine Inspectors found the vessel's Safety Radiotelephony Certificate had expired on 08/10/2021.

Each vessel must carry a certificate with a valid endorsement while subject to the Bridge-to-Bridge Act. Attending USCG Marine Inspectors found the vessel's Bridge-to-Bridge certificate had expired on 08/10/2021.

While transiting, one of the engines stopped producing forwward propulsion and began reversing. The captian un-synced the Voith engines and re-synced which resulted in a return of normal operation. See narrative for additional details.

Modifications were made to existing lube oil piping system during previous shipyard period by adding a lube oil preheater for both MDEs w/o plan approval or CG oversight. Modifications were made to existing bathroom arrangements in salon area during previous shipyard period by removing existing male and female bathrooms and installing multiple individual bathrooms w/o plan approval or CG oversight. Submit plans for approval, and upon receipt of approved plans, make vsl available for inspection to the satisfaction of the attending MI.(By next dry-dock & NLT 01JAN18)

2 existing pilothouse AC units were replaced with larger units and an additional AC unit was installed for the crew lounge w/o plan approval or CG oversight. Vessel had a loss of power on the #2 SSDG while at the galveston landing. See narratvie for futher details.

EGR automatic vent closure device "louvers" did not properly close during emergency generator shut down test or CO2 discharge test.

EGR CO2 system discharge control head failed pressure test (9.5psi, should read 6 psi). Control head was replaced and tested satisfactory, def corrected on the spot.

Electrical wires entering the side of the metal outlet box on control console are not protected from chaffing. Add stuffing tube or provide chaffing protection.

Port and Starboard navigation lights on either end of ferry have been improperly installed and are not providing proper arc of visibility or illumination.

Child life jacket donning instructions missing.
