At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following: Inspection of the condition of the superstructure, masts, and similar arrangements constructed on the hull. MI discovered 03 4"x4" wooden support posts for upper deck excessively wasted on the base of posts; 02 posts port and starboard below upper deck staircase and 01 port forward. Upper deck fiberglass coating is delaminating and exposing raw plywood deck to the weather. Owner is to submit plan of repair to OCMI for approval and complete repair prior to carrying any passengers on the upper deck.

Vessels must undergo a drydock and internal structural examination as follows: A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. Owner is trying to sell the vessel due to financial constraints in continuing with the drydock. Vessel cannot carry passengers until drydock is completed.

All cylinders used for storing carbon dioxide must be fabricated, tested and marked in accordance with the requirements of 147.60 and 147.65 of this chapter. One of three CO2 bottles on the vessel was condemned by Fire Protective Services (FPS) during the inspection of the system due to excessive rust. Replace to the satisfaction of the attending Marine Inspector.

The minimum number of portable fire extinguishers required on each vessel shall be as determined by the Officer in Charge, Marine Inspection in accordance with Table 181.30-1(a). Two(2) of the required four (4) portable fire extinguishers were inoperable, one was condemned by FPS due to excessive rust and one is due for a hydrostatic test. Replace to the satisfaction of the attending Marine Inspector.

A power-driven fire pump shall be installed on each mechanically propelled vessel which is a ferry vessel or which is permitted to carry more than 49 passengers. Vessel's designated fire pump panel is malfunctioning, vessel can use bilge pump but has to stop vessel for it to operate properly. Repair to the satisfaction of MI.

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Vessel's Ships Radio Telephony Safety Certificate expired on 20FEB2024. Correct to the satisfaction of the attending Marine Inspector.

At each initial and subsequent inspection for certification, the marine inspector shall view the vessel afloat and shall conduct the following test and inspections of the hull structure and its appurtenances: Examine all accessible parts of the exterior and interior of the hull. Stern decking has two corrosion holes approximately 1/4" allowing water to enter the stern Rudder room. Repair to the satisfaction of the attending Marine Inspector.

It shall be the duty of the master of any vessel to assure that all exposed hatches are properly secured before getting underway for a voyage. The number two void flush hatch is missing the "T" bar on the bottom that secures it to the deck. Correct to the satisfaction of the attending Marine Inspector.

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level. The engine room bilge high level alarm visually indicated but did not produce an audible alarm, all other spaces provided both. Repair to the satisfaction of the attending Marine Inspector.

Flexible hose designed for use with hose clamps may be installed with two clamps, at both ends of the hose. The Flexible hoses on the generator only have one clamp on each end. Repair to the satisfaction of the attending Marine Inspector.

Each applicant for an original endorsement or renewal of license must present a current radiotelegraph operator license (T) issued by the Federal Communications Commission. Vessel Captain did not have his FCC radiotelepgraph operator license available. Correct to the satisfaction of the attending Marine Inspector.

First-aid and cardiopulmonary resuscitation (CPR) course certificates: A currently valid certificate of completion of a CPR course. Vessel Captain did not have a valid CPR course certificate and only one deckhand had a current certificate. Correct to the satisfaction of the attending Marine Inspector.

The marine inspector must check the settings of the safety or relief valves on all pressure vessels, except cargo tanks, twice every 5 years. No more than 3 years may elapse between any check and its immediate predecessor. The safety relief valve installed on the air receiver did not have any means to verify the proper operation of the device. Provide attending Marine Inspector with satisfactory means to verify the proper operation of Safety Relief Valve.

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil. Rear main engine has excessive oil accumulation in valve channel underneath the blower leading to oily water accumulation in the bilge. Clean excess oil, determine and correct the cause of the leak to the satisfaction of the attending Marine Inspector.

ll uninsulated current carrying parts must be mounted on noncombustible, nonabsorbent, high dielectric insulating material. Buss bars in engine room were found mounted directly to wooden backers.

Individual bilge suction lines must be led to a central control point or manifold and provided with a stop valve at the control point or manifold and a check valve at some accessible point in the bilge line. Bilge control valve for #2 void is located inside the space, you are unable to access it if the space was flooded.

Flexible hose must meet the requirements of § 182.720(e)Where flexible nonmetallic hose is permitted for use in piping systems by this section, it must meet SAE J–1942,Hose end fittings must comply with SAE J–1475. Flexible hoses for forward engine does not meet above requirement, Aft engine fuel line is degraded and needs replaced, flexible hose in fuel supply line in between the fore and aft engine fuel shutoff does not meet the above requirement.

The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located. The reach rod for the valve was improperly connected and unable to properly close the valve, crew repair before the MI departed.

A hatch exposed to the weather must be watertight. The door leading to the engine room has a wastage hole in the frame at the bottom of the door.

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Unknown alarm activated in OS and audible in ER, identify cause.

Each watertight bulkhead must be of sufficient strength to be capable of remaining watertight with a head of water to the top of the bulkhead. Bulk head boundary in the void space forward of the engine room was found not to be water tight with an approximately ½ inch x 3 inch gap. Repair watertight boundary to the satisfaction of the attending marine inspector.

Bulk head boundary in the void space aft of the engine room was found not to be water tight. Repair electrical wire penetration into the engine room to the satisfaction of the attending marine inspector.

The design of all exhaust systems must ensure minimum risk of injury to personnel at such locations where persons or equipment might come in contact with an exhaust pipe. Exhaust manifold in the engine room and the exhaust pipe by the operating station need insulation to the satisfaction of the attending marine inspector.

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. all closure devices must be readily available and mounted in the vicinity of the vent. Dampers for the engine room vents did not provide adequate closure in relation to the vent opening on the aft side of the operating station. Provide proper damper for the engine room vent to the satisfaction of the attending Marine Inspector.

Electrical equipment on a vessel must be installed and maintained to: protect passengers, crew, other persons, and the vessel from electrical hazards. Lighting fixture in the engine room was hanging from the overhead and resting on the engine. Repair light to the satisfaction of the attending marine inspector.

Fuel lines must be accessible, protected from mechanical injury, and effectively secured against excessive movement and vibration by the use of soft nonferrous metal straps which have no sharp edges and are insulated to protect against corrosion. Fuel line on the forward diesel fuel tank has no structural support and is resting on another hose. Repair fuel line to the satisfaction of the attending marine inspector.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Noted oil leak under both operating stations descending along pipes from operating station to the engine room. Identify, secure and clean source of leak to the satisfaction of the attending marine inspector.

A drydock examination conducted in compliance with 176.600 must be conducted while the vessel is hauled out of the water or placed in a drydock or slipway. During the examination all accessible parts of the vessel's underwater body and all through hull fittings, including the hull plating and planking, appendages, propellers, shafts, bearings, rudders, sea chests, sea valves, and sea strainers shall be made available for examination. Sea chests, sea valves, and sea strainers must be opened for examination. On wooden vessels, fastenings may be required to be pulled for examination. The accuracy of draft or loading marks, if required by 185.602 of this chapter, must be verified if not verified at a previous dry dock examination. Vessel needs to complete final exam to verify work list items and seaworthiness of the vessel.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. A fathometer was found installed on board the vessel without approval from the cognizant OCMI with through hole fitting on the starboard side of the engine room approximately 3 feet above the water line. Properly install fathometer in accordance with the manufacturer’s instructions to the satisfaction of the attending Marine Inspector.

On a vessel of at least 7.9 meters in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces: a machinery space bilge, bilge well, shaft alley bilge, or other spaces subject to flooding from sea water piping within the space. Inoperable audible and visual bilge alarm in the aft most compartment. Repair audible and visual alarm to the satisfaction of the attending Marine Inspector.

Each distribution panel and switchboard must be in as dry a location as practicable, adequately ventilated, and protected from falling debris. Engine room 12 VDC power distribution panel has excessive oil leaking on top of the enclosure and seeping inside around circuit breakers. Clean excess oil and maintain oil contamination efforts to the satisfaction of the attending marine inspector. 46 CFR 183.330(a)

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. The vessel’s generator was found without a sufficient guard installed to protect personnel against rotating machinery belts. Install effective machinery guard to the satisfaction of the attending Marine Inspector. 46 CFR 182.100

All electrical equipment and circuits must be clearly marked and identified. Breaker panels located on the bridge and in the engine room are not properly labeled. In addition, the specific amperage ratings are no longer visible on multiple switches. Ensure that each switch is properly labeled for its intended use to the satisfaction of the attending Marine Inspector.

Electrical equipment on a vessel must be installed and maintained to: protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Exposed wires on aft pole by escape ladder. Remove or cap wires to the satisfaction of the attending Marine Inspector.

Electrical equipment on a vessel must be installed and maintained to: protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Outlet on the port bridge bulkhead is reverse wired. Wire outlet appropriately to the satisfaction of the attending Marine Inspector.

Electrical equipment on a vessel must be installed and maintained to: protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Marine Inspector identified multiple exposed and/or dead end wires under the starboard side of the forward bridge console. Remove or cap wires to the satisfaction of the attending Marine Inspector.

A vessel must be provided with a main steering gear that is: of adequate strength and capable of steering the vessel at all service speeds. Aft steering ram found to have a hydraulic oil leak with moderate hydraulic oil accumulation in the bilge. Clean bilge and repair aft steering to satisfaction of attending Marine Inspector.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. First-aid kit was missing required contents to include Ammonia inhalants. Obtain all required first aid contents as per 46 CFR 160.041-4 to the satisfaction of the attending Marine Inspector.

Each watertight bulkhead must be of sufficient strength to be capable of remaining watertight with a head of water to the top of the bulkhead. Bulk head boundary in the void space aft of the engine room was found not to be water tight with an approximately ½ inch gap observed along the overhead interfacing with the deck plate. Repair watertight boundary to the satisfaction of the attending marine inspector by the vessels next scheduled dry-dock.

The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies once each month, if reasonable and practicable; but at least once within a 3 month period before the vessel gets underway with passengers. Drill intervals exceeded the one month and three month interval. Complete drills at required intervals.

Annual Inspection. Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel lapsed on the required annual inspection date interval. Receive credit annual inspection from a USCG Marine Inspector prior to the carriage of passengers.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessel's Forward Main Propulsion Engine is inoperable and unable to maintain constant marine load. Prior to carrying passengers. 46 CFR 182.200

A device must be provided to automatically shutdown power ventilation serving the protected space and engines that draw intake air from the protected space prior to release of the extinguishing agent into the space. Control device is severely deteriorated. Repair and replace with proof of proper operation. 46 CFR 181.410(b)(10)

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. Stern navigation light is inoperable. Vessel is restricted to daytime operations until stern light is repaired. 46 CFR 183.420

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Stowage areas need proper labeling. 46 CFR 180.78(a)(5)

Employers must ensure that they and their crew members meet the requirements of this part. Crew hasn't been randomly drug tested in over a year. Crew promptly went 46 CFR 16.203(a)(1) 10, c

A submersible electric pump may be used if a positive shutoff valve is installed at the hull penetration. Vessel does not have a positive shutoff valve installed.

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located. Aft Fuel tank emergency shutoff valve handle is broken thus making the reach rod inoperable.

Centerline by the keel, approximately 15 feet fwd of aft draft marks there was water leaking from unknown source due to boats hull was located in the dry-dock support points. Vessel will adjust the dry-dock location in order to properly access any damage and submit a repair proposal to the attending Marine Inspector for approval.

FLEXIBLE NON METALLIC PIPING MUST BE PROTECTED FROM MECHANICAL DAMAGE AND ALL PROTECTIVE COVERING OR SHIELDS MUST BE INSTALLED. HYDRAULIC HOSES SERVICING AFT END MDE CLUTCH WORN, PROTECTIVE COATING WASTED

SHUT OFF VALVES, INSTALLED SO AS TO CLOSE AGAINST THE FUEL FLOW, MUST BE FITTED IN THE FUEL SUPPLY LINE. THE SHUT OFF VALVE AT THE TANK MUST BE MANUALLY OPERABLE FROM OUTSIDE THE COMPARTMENT. WHILE OPERATING THE BOW END FUEL TANK EMERGENCY SHUTOFF VALVE THE PIPING FRACTURED AT THE THREADED CONNECTION LEADING INTO THE TANK CAUSING DIESEL FUEL TO LEAK UNCONTROLLED.

SHUT OFF VALVES, INSTALLED SO AS TO CLOSE AGAINST THE FUEL FLOW, MUST BE FITTED IN THE FUEL SUPPLY LINE. THE SHUT OFF VALVE AT THE TANK MUST BE MANUALLY OPERABLE FROM OUTSIDE THE COMPARTMENT. WHILE OPERATING THE STERN END FUEL TANK EMERGENCY SHUTOFF VALVE THE REACH ROD BROKE.

A VESSEL MUST BE PROVIDED WITH A SATISFACTORY ARRANGEMENT FOR DRAINING ANY WATERTIGHT COMPARTMENT. ENGINE ROOM (BOW END) BILGE INOPERABLE, WOULD NOT PROVIDE SUCTION.

04102-Emergency fire pump and its pipes: A fire pump must be capable of both remote operation from the oerating station and local operations at the pump. During annual inspection, remote operation control for the fire pump at the operating station not labeled. 46 CFR 181.300 (e)

A SPACE CONTAINING PROPULSION MACHINERY MUST BE EQUIPPED WITH A FIRE DETECTION AND ALARM SYSTEM, EXCEPT WHEN A FIXED GAS FIRE EXTINGUISHING SYSTEM THAT IS CAPABLE OF AUTOMATIC DISCHARGE UPON HEAT DETECTION IS INSTALLED OR WHEN THE SPACE IS MANNED. VESSELS AUTOMATIC HEAT DETECTORS FAILED ANNUAL INSPECTION.

Vessel inspection of the bilges may be examined to see that there is no excessive accumulation of oil that might create a fire hazard. Prior to carrying passengers, remove and clean excess oil from engine room bilge to the satisfaction of Coast Guard Marine Inspector.

Vessel reported a catastrophic aft engine failure while carrying passengers that occured two days prior on 18JUN2017 at 1715hrs. All machinery & equipment must be installed and maintained in such a manner to afford adequate protection from machinery failure. Vessel rep shall identify cause of failure and effect permanent repairs.

Aft E/R BLKHD has numerous holes in surrounding shaft log. 179.320(a) Each watertight BLKHD must be of sufficient strength to be capable of remaining watertight.

No audible or visual verification on bridge of high level alarm in lazerette. 182.530(a) Visual and audible alarm must be provided at the operating station to indicate high level.

No remote means of operation for fire pump in pilot house. 181.300(e) A fire pump must be capable of both remote operation from the operators station and local operations at the pump.

Nonmetallic drain hose penetrating from steering space thru Blkhd. 179.320(c) A penetration thru a watertight blkhd must be made watertight.

Hole in deck aft of E/R. 178.430(a). The weatherdeck on a well deck vessel must be watertight.

Missing and inoperable emergency lighting along egress route. 183.432(a) Each vessel must have adequate emergency lighting fitted along the line of escape to the main deck.

Lighting and dehumidifier systems are on same circuit. 183.410 (b) Lighting fixture may not be used as a connection box for a circuit other than the branch circuit.

Temporary repair on F/O vent line. 176.700 Repairs and alterations to the Hull Machinery or equipment that affect the safety of the vessel must not be made without approval.

E/R emergency escape not labeled. 185.606 All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters" Emergency Exit Keep Clear" in 2" lettering.

Electrical wire conduit incorrect type and style. 183.340 (a) Individual wires, rather than cable greater than 50V must be in conduit.

Incorrect heads on CO2 bottles. Service tech is unable to test. 176.810 Inspection and testing of all smoke and fire detection systems including sensors and alarms.

Vessel operator has not met the dry dock date of 11 JAN 2017. Vessel is to not be used for commercial purposes or carry passengers until credit dry dock has been completed.

Prior to getting u/w, correct outstanding CG-835 issued 08APR03 - Renew and reinstall shaft collar flanges for intermediate shaft penetrations through watertight bulkheads between e/r and big voids. Collars must be installed on both sides of the bulkheads per original construction arrangements.

Renew #3 starboard bracket forward in forward big void.

Renew #2 port overhead deck beam and aft bracket. #4 port overhead deck beam in forward big void. LEGACY COMPONENT DESC: Stiffener

REnew and Reinstall shaft collar flanges for intermediate shaft penetrations through watertight bulkheads between e/r and big voids. Collars must be installed on both sides of bulkhead per original construction arrangements.

provide gauging report to assess condition of shell plating along keel in areas where conctete permanent ballast has been installed. Gauging shots to be taken at 24" by 24" grid.

Renew all holed and Thinning outboard deck supports. LEGACY COMPONENT DESC: Stiffener

Thoroughly scrape/clean interior of sea chest to make spool pieces availabe for insepection.
