All vessels except non-self-propelled vessels shall be provided with suitable steering apparatus. MI observed the vessel's hydraulic steering ram leaking steering fluid at an approximate rate of 50 drips per minute. The leakage is active and substantial enough to degrade the performance and reliability of the steering system. 46CFR 182.30-1(a)

Gauges to indicate engine cooling water discharge temperature and lubricating oil pressure shall be provided for all propulsion engines and located so to be in view of the operator. The starboard main engine cooling water gauge was inoperable.

Vessel must comply with applicable requirements for any radio installation. MF/SSB radio installation on board vessel does not have Digital Selective Calling (DSC) capability IAW 47 CFR 80.905(a)(2). Vessel is restricted from operating more than 20 nm from shore until a MF-DSC transmitter and receiver is installed.

Each survival craft on a vessel must be in good working order and ready for immediate use before the vessel leaves port and at all times when the vessel is underway. The starboard life float was in disrepair with significant wastage to the webbing attaching the grab lines to the body of the life float. Deficiency rectified prior to departure; vessel procured new life float prior to completion of inspection.

All cable and wire must be installed with metal supports spaced not more than 24" apart, and in such a manner as to avoid chafing and other damage. The use of plastic tie wraps must be limited to bundling or retention of multiple cable installations. Multiple cable runs throughout the engine room were hanging loose and minimally secured by plastic zip ties.

Shutoff valves shall be fitted in the fuel supply lines and shall be operable from outside the tank compartment. The shutoff valve for the aft fuel tank was inoperable. Prior to completion of the inspection, the operator was able to troubleshoot and inspector witnessed proper remote operation of the valve.

All vessels except non-self-propelled vessels shall be provided with suitable steering apparatus. MI observed hydraulic leaks in the steering system to include engine driven pump housing and associated piping from the pump to the steering ram. Operator to repair all leaks to the satisfaction of the attending MI. 46CFR182.30-1 60c.

It shall be the duty of the master of any vessel to assure that all exposed hatches are properly secured before getting underway for a voyage on other than protected waters. MI observed that both main engine hatch covers and engine room entry hatch, in addition to steering gear space hatch cannot be secured due to failed/missing hardware. Operator to replace and or repair to the satisfaction of the attending MI. 46CFR185.20-15 60c.

Examine all accessible parts of the exterior and interior of the hull, watertight bulkheads: MI observed multiple unsealed bulkhead penetrations on the forward engine room bulkhead. Operator to reseal penetrations to the satisfaction of the attending MI. 46CFR176.25-5 16c.

Protective gratings shall be provided at such locations where persons or gear might come in contact with the exhaust pipe. MI observed exhaust lagging not properly installed and secured on both main propulsion engines and generator. Operator to reinstall or replace exhaust lagging to the satisfaction of the attending MI 46CFR182.15-20 16c.

Examine all accessible parts of the exterior and interior of the hull, watertight bulkheads: MI observed a cracked weld aft of the port main propulsion engine and a gouged/cut weld in the vertical seam below the failed weld. Operator to make repairs to the satisfaction of the attending MI 46CFR176.25-5 16c.

This part is intended to provide a minimum standard of regulations for electrical installations and equipment that will result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment, and reasonably safe from electrical shock of personnel. MI observed multiple dead ended wires, exposed conductors, improperly secured connectors and, unlabeled disconnects. Operator is required to remove all abandoned cable and wiring, secure wire connections, and label electrical disconnects and panels as cited in this reference to the satisfaction of the MI. 46CFR183.01-5 16c.

Protection from mechanical damage shall be specially considered. Unsupported piping in regards to the bilge pump riser in the vicinity of the port main propulsion engine was noted by the [name withheld] to properly secure all loose piping. 46CFR182.40-1 16c.

The fixed power bilge pump shall be self-priming and may be driven off the main engine or other source of power. MI observed the electrically powered bilge pump housing dripping from the pump case while pump was secured. Operator to identify the source of the leak and make necessary repairs to the satisfaction of the MI. 46CFR182.25-5 16c.

Engine control mechanisms. Such mechanisms shall be tested by operation and examined, visually, through-out their length. MI observed port main propulsion engine transmission control cable severely chaffed due to contact with the engine driven bilge pump belt. Operator to replace cable and reroute to avoid future damage. 46CFR176.25-10 16c.

Batteries shall be located as high above the bilge as practicable and secured against shifting with motion of the boat. MI observed engine room batteries not secured. Operator to secure batteries to the satisfaction of the attending MI. 46CFR183.05-20 16c.

Suitable covers, guards, or rails shall be installed in way of all exposed and hazardous places such as gears, machinery, etc. MI observed generator guard not installed satisfactorily. Operator to reinstall guard to the satisfaction of the attending MI. 46CFR177.35-15 16c.

All vessels shall be provided with an approved adult type life preserver for each person carried. Vessel is certified for 30 life preservers and was missing 02. Add two approved adult life preservers to the satisfaction of attending MI. 46 CFR 180.25-5

Except as further modified in this section, where items of life-saving equipment is not required, but are installed, such equipment and its installation shall meet the requirements of this part. Vessel has rescue platform and did not have a harness. Provide a harness and a means of securing the rescue platform to the vessel to the satisfaction of the MI. 46 CFR 180.05-5

The life line shall be of a size and strength not less than 3/8-inch diameter. Life line has began to deteriorate. Replace life lines on life floats to the satisfaction of the attending MI. 46 CFR 180.15-5

The inspection shall be such as to insure that the arrangement, materials and scantlings of the structure. Starboard window on pilot house was broken. Replace starboard window to the satisfaction of attending MI. 46 CFR 176.05-5

Interconnected detection and alarm system. (a) An interconnected detection and alarm system must: (1) Consist of multiple-station smoke detectors listed by an NRTL, or independent laboratory accepted by the Commandant according to 46 CFR subpart 159.010, as meeting UL 217 (incorporated by reference, see 46 CFR 175.600); (2) Be installed such that the actuation of alarm in one area results in both audible and visual alarms in all areas required by 46 CFR 181.405(c) or 118.400(d) to be protected by the interconnected detection and alarm system; (3) Contain an independent power source; and (4) Alarm on low power. All vessels regulated by Subchapter T (this includes OSV-T boats) that have overnight accommodations for passengers or are operating on a Coastwise or Oceans route must have an interconnected detection and alarm system that complies with 46 CFR 181.450. Provide to the satisfaction of the attending MI. 46 CFR 181.450

Accessories, such as switches, fuses, and sockets shall be standard National Electrical Code types for the loads to be carried. The 12V system on the vessel was malfunctioning causing VHF radios, AIS, radars, engine room smoke detection system, and ship whistle to be inoperable. 46 CFR 183.05-35

Propulsion machinery shall be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load. Marine Inspector observed significant oil leaks on the starboard main diesel engine mechanical governor box and aft end of the crank case causing a fire hazard. Vessel must determine the cause of the oil leakage and rectify. 46 CFR 182.05-5

All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspections. The ship service diesel generator had significant oil leakage from the exhaust manifold flange causing a fire hazard. Vessel must determine the cause of the oil leakage and rectify. 46 CFR 183.01-15

At each initial and subsequent inspection for certification and reinspection the marine inspector shall examine the bilges and other spaces to see that there is no accumulation of oil or other matter which might create a fire hazard. Marine Inspector observed excess amounts of oily water in the Engine Room bilge. 46 CFR 176.25-50

Each vessel must meet the applicable requirements in Subchapter S of this chapter. MI observed three bulkhead penetrations in the forward bulkhead of the engine room that were not water tight. 46 CFR 177.13-1

The construction and arrangement of the vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection. While transiting through mm 1 AHP, vessel ran aground. Vessel suffered damage to the port propeller, wheelhouse windows and wheelhouse switch panel. Prior to further operation owner shall make vessel available for inspection to verify proper installment of propeller, proper operation of switch panel, and weather test wheelhouse windows. 46 CFR 177.100
