The specimen collection and shipping kits need not be carried on board each vessel if obtaining the kits and collecting the specimen can be completed within 32 hours from the time of the occurrence of the SMI. Vessel did not have specimen collection kits on board and conducts oceans route up to 100 miles. Add specimen kits on board to the satisfaction of Marine Inspector. 46 CFR 4.06-15(b)(2)

Each vessel must meet the applicable requirements in subchapter s of this chapter. MI discovered gasket on port door of wheel house was bad. Renew gasket to the satisfaction of the Marine Inspector.. 46 CFR 177.13-1

Each life jacket carried on a vessel on oceans, coastwise, or Great Lakes route, must have a life jacket light approved in accordance with § 161.012 in subchapter Q of this chapter. MI discovered all life jackets lights missing expiration dates. Add dates to lights to the satisfaction of the Marine Inspector. 46 CFR 180.75(a)

Each rescue boat and ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018. MI observed Retro tape worn on stbd life float. Replace retro tape on stbd life float to the satisfaction of the Marine Inspector. 46 CFR 185.604

Electric able for installation in damp and wet location must be impervious sheathed. Marine Inspector identified that the cables on the port batteries in the steering room were corroded. along the wire and at the terminal connection. 46 CFR 183.10-20(b)(1)(i)

Interconnected detection and alarm system. (a) An interconnected detection and alarm system must: (1) Consist of multiple-station smoke detectors listed by an NRTL, or independent laboratory accepted by the Commandant according to 46 CFR subpart 159.010, as meeting UL 217 (incorporated by reference, see 46 CFR 175.600); (2) Be installed such that the actuation of alarm in one area results in both audible and visual alarms in all areas required by 46 CFR 181.405(c) or 118.400(d) to be protected by the interconnected detection and alarm system; (3) Contain an independent power source; and (4) Alarm on low power. All vessels regulated by Subchapter T (this includes OSV-T boats) that have overnight accommodations for passengers or are operating on a Coastwise or Oceans route must have an interconnected detection and alarm system that complies with 46 CFR 181.450. Provide to the satisfaction of the attending MI. 46 CFR 181.450

On a vessel of at lease 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level. Vessel bilge alarm is inoperable. Repair to the satisfaction of the marine inspector.

This part contains requirements for the design, construction, installation, and operation of propulsion and auxiliary machinery, piping and pressure systems, steering apparatus, and associated safety systems. Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the intended purpose. Port rudder hardware is loose. Repair to the satisfaction of the marine inspector.

Each lighting fixture, globe, lens, or diffuser must have a guard or be made of high strength material, except in an accomodation space, radio room, galley or similar space where it is not subject to damage. Galley and head light fixture broken or missing. Install new cover for lights to satifaction of the marine inspector.

Electrical equipment on a vessel must be installed and maintained to: (b) protect passengers, crew and other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment and electrical shock. Observed water intrusion from search lights; make watertight to satisfaction of the marine inspector.

Vessel control systems and other miscellaneous systems and equipment required by this part must be suitable for the intended purpose. Vessel GPS controls are damaged/missing. Repair GPS to the satisfaction of the marine inspector.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure and personnel injury. Helm is leaking behind control station. Repair to satisfaction of the marine inspector.

This part contains requirements for the design, construction, installation, and operation of electrical equipment and systems including power sources, lighting, motors, miscellaneous equipment, and safety systems. Observed daisy chain of temporary extension cords. Disconnect daisy chain and remove temporary cords as permanent power source.

A vessel must comply with the applicable requirements for any radio and EPIRB installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR Part 80. Radiotelephony Safety Cert is expired. Have certificate renewed to the satisfaction of the marine inspector.

An adult lifejacket must be provided for each person on board a vessel. Vessel was missing 02 lifejackets.

Each life jacket, immersion suti, and ring life buoy must be marked in clearly legible block capital letters with the vessel's name. Re-label life buoy to the satisafaction of the marine inspector.

The inspection and maintenance of the equipment listed in paragraph (a) of this section shall be logged or otherwise documented for review by the Coast Guard upon request. Records have not been logged since December.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: (1) charts of large enough scale to make safe navigation possible; (2) US Coast Pilot or similar publication; (3) Coast Guard Light List (4) Tide tables; and (5) current tables, or a river current publication issued by the US Army Corps of engineers or a river authority. Charts and pubs are to remain on the vessel.

Gauges to indicate engine revolutions per minute (RPM), jacket water discharge temperature, and lubricating oil pressure must be provided for all propulsion engines installed in the vessel. The gauges must be readily visible at the operating station. Engine temperature gauge was not reading. Provide proof of the gauges operating properly to the marine inspector.

Electrical equipment on a vessel must be installed and maintained to: (a) provide services necessary for safety under normal and emergency conditions; Starboard search light extinguished. Repair search light to the satisfaction of the Marine Inspector.

Each distribution panel and switchboard must be in as dry a location as practicable, adequately ventilated, and protected from falling debris and dripping or splashing water. Panel door cannot secure due to broken latch. Repair panel to the satisfaction of the marine inspector.

At each inspection for certification and at every other vessel inspection, quarters, toilet and watshing spaces, galley, serving pantries, lockers, and similar spaces may be examined to determine that they are servicabe and in a sanitary condition. Sewage line had flexible hose improperly connected to rigid pipe. Install hose to the satisfaction of the marine inspector.

46 CFR 180.71(a) - An adult life jacket must be provided for each person carried on board a vessel. Vessel had 30 life jackets on board, but 25 of them are no longer approved. Vessel is limited to carrying 2 passengers until they can provide evidence of having the appropriate number of approved life jackets to match COI requirements.

46 CFR 180.75(a) - Each life jacket carried on a vessel on oceans, coastwise, or Great Lakes route, must have a life jacket light approved in accordance with §161.012 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.112. Vessel was using unapproved lights on 25 of their life jackets. Vessel is limited to carrying 2 passengers until they can provide evidence of having the appropriate number of approved life jacket lights to match COI requirements.

46 CFR 185.516(a) - Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Vessel did not have placard that matched style of life jackets on board.

46 CFR 182.25-1 - All vessel shall be provided with a satisfactory arrangement for draining any watertight compartment other than small buoyancy compartments, under all practical conditions. Flex hose being used to connect electrical bilge/fire pump to motor driven pump was collapsed, rendering it ineffective.

46 CFR 177.35-15 - Suitable covers, guards, or rails shall be installed in way of all exposed and hazardous places. Both guards covering vessels shafts were not bolted in place or providing adequate coverage of the rotating equipment, creating a hazard inside engine room.

46 CFR 183.01-15(a) - All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspections. Vessel had 1 power strip installed on the bridge and 1 in the main cabin.

46 CFR 170.005(a)(1) - Alterations and repairs to inspected vessels must be done under the direction of the Officer in Charge, Marine Inspection. Inspector observed cracked welds where port and starboard exhaust piping meets the stern bulkhead on the vessel.

Fuel water separator "Racor" was found to have a leak on the bowl on both Main diesel Engines. Complete repair in order to stop fuel leak.

Bilge Alarm is not working, no audible no visual alarm for any of the spaces. Conduct troubleshooting and repair as needed.

Batteries on Lazarette have loose connections. Adjust all connections on batteries as needed.

Bilge pump wiring not secured with a tension clamp/Stuffing tube leading into pump casing.

Starboard side engine room lighting fixture cover/guard broken.

Dead wire was located in the engine room. Remove or replace dead ended wire. Old "T" cite 183.05-50(2).

Vessel did not have the required three 2 and 1/2 gallon buckets on board. Fire buckets are required to sail. Vessel rep brought fire buckets to vessel at the end of the inspection and deficiency was cleared on the spot.
