The machinery, boilers, and other pressure vessels, associated piping systems and fittings shall be of design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. The master of the vessel reported that the vessel experienced a marine casualty after an alarm sensor shut down Main Engine #1A. Engineers discovered five exhaust valves leaking jacket water into the engine oil which triggered an oil mist sensor to alarm and shut down the engine. Provide an ABS survey report to MSU Saipan once repairs are completed to the satisfaction of the attending surveyor. SOLAS 74 cons. (2004 ed.) Ch. II-1 Reg 26.1

1. Frame is wasted on the A-15 Fire boundary door C-Deck in the forward stair tower. Make permanent repairs to restore integrity of the boundary to the satisfaction of the attending ACP ACS.

2. Galley firefighting roller door does not operate properly and is supported to remain in the open position. Make permanent repairs to restore integrity of the boundary to the satisfaction of the attending ACP ACS.

Watertight Door 1-20-0 is missing a section of gasket. Make permanent repairs to restore integrity of the watertight boundary to the satisfaction of the attending ACP ACS.

Fire Station #140 valve is leaking. Make permanent repairs to the satisfaction of the attending ACP ACS.

Replace retro-reflection on all 4 life boats and bridge ring buoys as needed to the satisfaction of the attending ACP ACS.

Planned Maintenance Program onboard documentation is not available as required by ABS Rules Part 7.14.13.5. Provide required documentation to the satisfaction of the attending ACP ACS. ABS Rules Part 7.14.13.5

The company should ensure that each ship is provided with an integrated system of contingency plans for shipboard emergencies. Safety Management System identified grounding as an emergency situation but did not provide an integrated response plan. The Recognized Organization (ABS) is to attest to the verification of conformance to the ISM Code.

#4 SSDG Reverse Power Relay does not operate. Do not use generator in parallel until properly fixed.

Total persons allowed on board is restricted to 100 until port and starboard marine evacuation systems are fitted with US Coast Guard approved inflatable life rafts.

Provide written proposal to OCMI regarding alterations made to port MES by manufactures techinicians following failed deployment. Proposal shall address any necessity to apply alterations to stbd MES and whether use of cable ties are sufficient corrective action to retain the trim/list cable.

The vessel's OWS did not operate due to a solenoid failure during warm up phase. ABS did sat check during annuals previous to annual coi inspection. Prove proper operation as designed before departure from dry dock.

Both first aid kits in engineering spaces need to be updated and inventoried properly. Locations are 1) in ECC and 2) 5-99-2 eng shop. Repair before leaving dry dock.

The #4 SSDG has an exhaust leak. Make perm repairs to elimate hazard of leaking exhaust. Repair before leaving dry dock.

The bleach/chlorine tank for the ship's MSD has a small leak and needs repair or replacment of tank to prevent possible large spill of bleach onto personnel. Repair before leaving dry dock.

The vessel's Fire Detection System shows a constant trouble alarm. Ship's crew says trouble alarm is due to small grounds in the system electrical system that can not be repaired. Provide tech report to attest to proper operation of detection system and whether grounds in system are normal/accepted. Repair before leaving dry dock.

Provide alarm for all refrigerated spaces capable of being locked from the outside. Complete before leaving dry dock.

Repair crack in elbow of cold potable water line at the forepeak entrance. Repair before leaving dry dock.

Provide electrical cover for heater box in the #3 lifeboat. Repair before leaving dry dock.

Repair/replace broken glass in A60 fire door at 4-90-2on 5c deck. Repair before leaving dry dock.

Prior to activation from ROS to FOS provide to attending MI; all crew licenses amd crew documents, demonstrate fire and boat drills, updated nav charts, and hospital medicines.

Maintain fire detection system to eliminate two detector heads found in "alarm" status.

Renew cracked window at coxian position, portside, lifeboat #1.

Sewage system lift station in C5 wasted, renew as original.

Renew seal for emergency generator jacket water heater pump.

Remove standing oil in vicinity of main engines to provide clean dry bilges.

Prove proper operation/servicing and maintenance of MSD H2S atmospheric alarm and LP CO2 atmosphere detector alarm.

As per MOU, upon being recalled/change of status vessel must demonstrate satisfactory fire drills with entire crew on board.

PROVE CREW'S PROFICIENCY IN ALL ASPECTS OF SHIPBOARD OPERATIONS INCLUDING MACHINERY AUTOMATION AND FIRE AND BOAT DRILLS PRIOR TO DEPARTING AVONDALE SHIP YARD.
