Vessels must be equipped with independently driven fire pumps in accordance with Table 95.10-5(a). Vessels over 1000 GT shall have two fire pumps. While discharging cargo in Marysville, MI the main fire pump failed. Vessel shall diagnose, repair, main fire pump and provide attestation from the Chief engineer the pump is fully operational. Notify the local OCMI of any change in condition.

All repairs shall be conducted to the satisfaction of the local OCMI.

A ship 1600 GT or more must have fixed containment around oil tank vents and fill vents with a capacity of at least one barrel. Containment provided was not adequate.

Vessel shall enact permanent repairs to the satisfaction of the local OCMI prior January 31st, 2026.

Abandon-ship drills must include lowering of at least one lifeboat after any necessary preparation for launching. Vessel was unable to lower open lifeboat due to the mooring arrangement of the vessel. Vessel is to perform a lowering of the lifeboat in the presence of a marine inspector at the next available port of call. 46 CFR 199.180(c)

Whenever a vessel is deprived of the service of a member of its complement, and the master or person in charge is unable to find appropriate credentialed personnel to man the vessel […] a report of sailing short must be filed with the OCMI having cognizance for inspection in the area in which the vessel is operating, or the OCMI within whose jurisdiction the voyage is completed. Vessel master made sailing short notification for the position of First Assistant Engineer from Detroit, MI to Sault Ste. Marine, ON. Vessel is sailing with an additional Second Engineer. Master shall ensure a 3-watch system is maintained, work/rest requirements are met, and all maintenance is performed in accordance with SMS. Master attests to be sufficiently manned and that work/rest requirements will be met for the voyage to Sault Ste. Marie, ON. Prior to next U.S. port, a replacement with the same or greater credential shall be found for the vacancy. If no replacement is found, the master/operator must provide a new request to the cognizant OCMI.

After a survey any fitting or structure found in an unsatisfactory condition must be repaired to the satisfaction of the attending Marine Inspector or Class Surveyor. The #1 Port ballast tank is leaking into the #1 Cargo Hold by-way of the port-side slope. Drill stop and fill crack, and clad weld affected area to the satisfaction of Coast Guard or Class.

An inspection either general or partial depending upon the circumstances shall be made whenever repairs are undertaken. Vessel suffered structural failure from frames 18-25 in port ballast tank #2. The failure caused 2 cracks in the bottom shell plating in the vicinity of frame 21. Company has made permanent repairs to affected frames and shell plating to the satisfaction of the [name withheld] to the nature of the failure, company must provide OCMI with failure analysis conducted by 3rd party to determine the cause. Failure analysis must be provided prior to drydock. Vessel may continue to operate until 2025 winter layup, provided that crew monitors the condition of the #2 port and starboard ballast tanks at least once every 14 days, or as operations allow.

Vessels must be equipped with independently driven fire pumps in accordance with Table 95.10-5(a). Vessels over 1000 GT shall have two fire pumps. While underway in Lake Huron the aux fire pump/tunnel wash down pump shaft broke. Vessel shall diagnose, repair, aux fire pump and provide attestation from the Chief engineer the pump is fully operational.

Automatic propulsion systems, automated electric power management systems, and all associated subsystems and equipment must be capable of meeting load demands from standby to full system rated load, under steady state and maneuvering conditions, without need for manual adjustment or manipulation. While underway, vessel's pitch increased to 65% and lost maneuverability. Vessel is to submit a technician's report attesting to the proper operation of the vessel from each of the three operating stations and from engine room controls prior to movement.

During and after an survey...those items are found to be in an unsatisfactory condition by the surveyor shall be repaired or renewed in order to place the vessel in a satisfactory condition. A box patch was satisfactorily installed on the hull penetration noted in Sector Detroit's Activity #7822381, in the #4 Port Ballast Tank. Permanent repairs are to be completed at next scheduled dry-dock.

Vessel is cleared to depart for Ashtabula, OH, conducting hourly soundings of port #4 ballast tank and surrounding areas. The following is to be completed upon arrival: 1. Conduct temporary repair for the hull penetration in port #4 ballast tank. Permanent repairs to be affected during winter lay-up 2023-2024. 2. Conduct permanent repairs to damaged frames in the port #4 ballast tank in vicinity of hull penetration. 3. De-ballast starboard tanks and make available for inspection by class surveyor, post grounding.

The 2nd Assistant Engineer, departed the vessel in Cleveland, OH on 25JUL2023, for vacation relief, a consent based discharge. Replacement due to be onboard 23AUG2023. Vessel is conducting shuttle runs in the Cuyahoga River in Cleveland, OH. Vessel is to find a replacement for the 2nd Assistant Engineer with the same or greater credential within 7 days. The vessel is only permitted to conduct shuttle runs in the Cuyahoga River - Cleveland, OH. The vessel is permitted to operate (cargo operations, transiting, and vessel maintenance)- THIS EXCLUDES ANY EMERGENCY RESPONSE- no more than 12 hours in a 24 hour period without a 2nd Assistant Engineer. The vessel shall notify MSU Cleveland Marine Inspectors 24 hours prior to completing shuttle runs, with the intent to depart Cleveland.

THE LIFEBOAT IS PAST DUE FOR A WEIGHT TEST (LAST DONE IN 2019) AND AN OPERATIONAL TEST. THE LIFEBOAT WAS UNABLE TO BE TESTED SINCE IT WAS DOCKSIDE TO THE PIER. MUST CONDUCT OPERATIONAL TEST AND WEIGHT TEST PRIOR TO LEAVING THE CLEVELAND [name withheld], BUT MAY CONDUCT COMMERCIAL OPERATIONS IN THE CLEVELAND AREA OF RESPONSIBILITY.

THE FIRE MAIN, ON THE MAIN DECK, HAS MULTIPLE LEAKS WHICH MUST BE REPAIRED TO THE SATISFACTION OF THE COAST GUARD, PRIOR TO DEPARTURE.

TO MAINTAIN STRUCTURAL FIRE PROTECTION, PAINT LOCKERS SHALL BE WHOLLY LINED WITH METAL. THE PAINT LOCKER IN THE FORECASTLE HAS A HOLE DRILLED IN THE BULKHEAD WITH AN EXTENSION CORD MUST BE REMOVED AND THE HOLE SEALED WITH AN APPROPRIATE STEEL INSERT.

All piping, valves, and fittings shall meet the applicable requirements of subchapter F (Marine Engineering) of 46 CFR. Fire main has pinhole, starboard side, at the wash down deck on the main deck. Vessel is permitted to transit up river while permanent repairs are being made. Once vessel arrives at Osborne Dock, repairs to be verified and tested to the satisfaction of Class/Coast Guard prior to vessel movement from Osborne Dock.

In the Aft Peak / steering gear void, along the Portside, between Frames 77-79, two frames are deformed due to an allision with a dock. The hull is not compromised but the frames must be cropped and renewed during the 2023 winter maintenance period, to the satisfaction of the local CG Marine Inspector.

The bow thruster has experienced multiple failures, most recently resulting in a marine casualty on 13DEC2022. The current status of the bow thruster creates a hazardous condition which is unacceptable. The bow thruster shall not be used until proper operation is proven to the satisfaction of the attending CG Marine Inspector.

Installed navigational control system, "Smart Syatem", has yet to successfully complete the required Design Verification Test Procedures (DVTP). The ship shall not use this system until the DVTP is witnessed, the results reviewed and the system approved by the Coast Guard.

The assignment of load lines is conditioned upon the structural efficiency and satisfactory stability of the vessel, and upon the provisions provided on the vessel for her protection and that of the crew. The AMERICAN COURAGE ran aground and potentially sustained unknown damage to the hull. Vessel is to have a hull survey following grounding, once it transits to Fairport, OH. The AMERICAN COURAGE is to receive hull survey from either Class or Coast Guard prior to departure from Fairport, OH.

The assignment of load lines is conditioned upon the structural efficiency and satisfactory stability of the vessel, and upon provisions provided on the vessel for her protection and that of the crew. The AMERICAN COURAGE ran aground and sustained damage to the STBD # 3, 4 and 5 ballast tanks. Vessel is permitted to receive temporary repairs in Ashtabula, OH. Permanent repairs are to be made at next dry dock. Inspection of repairs to be by the Coast Guard or Class.

All fire main piping, calves and fittings shall meet the applicable requirements of subchapter F (Marine Engineering) of 46 CFR. Starboard deck fire main has two pin hole leaks in branch of the fire main. Make permanent repairs and test to the satisfaction of Class. Satisfactory Class report required to be sent OCMI listed on this CG-835v to clear deficiency.

Abandon ship drills must include the lowering of at least one lifeboat after any preparation for launching; starting and operating the lifeboat engine and operating davits used for launching liferafts. Vessel has not been able to lower starboard lifeboat due to mooring arrangement in port. Vessel is to conduct an abandon ship drill to include the lowering of starboard lifeboat and operating the lifeboat engine when permitted to moor port side to.

Controlable Pitch Propeller locked in forward propulsion at about 40%. Make repairs to CPP controls and prove proper operation of the same, prior to departure.

Both Auxiliary boilers are required to undergo Hydrostatic testing, automation testing, safety valve inpsection, mounts opened, and fusible plug inspection, in the presence of a CG MI, prior to placing either boiler in service.

Crew was unable to lower and release lifeboat during the inspection due to being pier-side directly over the mooring wires/gangway. Provide objective evidence of satisfactory life boat drill per 46CFR199.180.

Label all General Alarm Bells in tunnel per 46CFR113.25-20 within 30 days.

Provide blanks in open breaker slots in the engine control room and house panels/implement lock out tag out to ensure protection of crew from electrical hazardous.

Vessel reported the air line for the clutch to the starboard engine broke. Vessel had to shut down starboard engine and operate on port engine. Vessel shall make repairs to the starboard engine to the satisfaction of Coast Guard.

Vessel unable to conduct Fire drill at the time of inspection. Vessel is required to conduct a fire drill within 90 days to the satisfaction of the Coast Guard.

Vessel Was unable to lower the life boat, as well as get the life boat underway during the inspection due to location at the dock. Vessel is required to lower the life boat into the water and have crew maneuver the life boat in the water in the next 90 days.

Approx. 24" crack was discovered in the #5 Port ballast tank aft bulkhead near knuckle. Repair according to repair proposal.

Take appropriate actions repair or replace malfunctioning components of the propulsion control system and provide technician's report attesting to the proper operation of the same, prior to departure.

Propulsion control froze in reverse. Prove proper operation of control unit prior to departure.

Counterbalance solenoid on stbd steering pump must be replaced as it had burn marks and smelled like it was burning.

Vessel does not have 12 rocket parachute flares approved under approval series 160.136. Vessel must supply 12 approved paracute flares on-board the vessel stowed on or near the vessels navigating bridge.

Vessel does not have the required hand flares and parachute flares on-board the lifeboat. Vessel must supply approved flares.

Vessels life jacket's and immersion suit stowage positions are not labled as such, or with the appropriate symbol from IMO Resolution A.760(a18). Label stowage locations to the satisfaction of the attending Coast Guard Marine Inspector.

Vessels time delay for the carbon dioxide extinguishing system installed in the forward and aft paint lockers releases at 25 seconds. Manufactures specifications on the current time delay states release at 30 seconds. Replace/service time delay to release at manufactures specifications. Servicing company needs to demonstrate time delay testing in the presence of CG Marine Inspector.

Cable penetrations were found to not meet structural fire protection requirements. Decks separating the accommodations and control stations from machinery spaces shall be of "A" class construction. Some penetrations were found to be lacking an approved sealing system such as the penetrations in the ECC on the port side which share their boundary with the engine room. Install properly listed and approved systems to ensure class "A" boundaries are properly maintained.

Lighting fixtures were found within accommodation spaces such as heads and berthing areas where lights were missing guards. Exposed bulbs and electrical connections pose an electrical and personnel hazard to crew members. All lighting fixtures are to have properly installed guards/covers to prevent electrical hazards.

Plumbing damage on board has resulted in tilework in several accommodation spaces to be damaged causing tilework in the heads to break away from the deck and lower bulkheads. The damaged tilework presents hazards as well as sanitary condition concerns. The damanged deck coverings are to be properly repaired to remove hazards to personnel and allow these spaces to be maintained in the sanitary condiction and suited for their intended purpose.

Both auxiliary boilers hydrostatic test, fireside inspection, waterside inspection, safety-valve inspection, anf fusible plug inspection is overdue. Boilers must be made available for examination by CG MI prior to placing either boiler in service.

Both auxiliary boilers must have their automation systems tested prior to placing either boiler in service. Automation testing is to be conducted in the presence of an attending CG MI.
