All lifesaving appliances and arrangements must be in accordance with subchapter W (Lifesaving Appliances and Arrangements) of this chapter. Markings. Each lifejacket must be marked—(i) In block capital letters with the name of the vessel; and(ii) With Type I retro-reflective material approved under approval series 164.018. The arrangement of the retro-reflective material must meet IMO Resolution A.658(16).(4) Attachments and fittings. Lifejackets must have the following attachments and fittings:(i) Each lifejacket must have a lifejacket light approved under approval series 161.112 securely attached to the front shoulder area of the lifejacket.(ii) Each lifejacket must have a whistle firmly secured by a cord to the lifejacket. Lifejackets throughout the vsl need proper stenciling, lights are expired, Clips not fully engaging. Correct to the satisfaction of the attending Marine inspector.

All lifesaving appliances and arrangements must be in accordance with subchapter W (Lifesaving Appliances and Arrangements) of this chapter. Additional equipment. Each vessel must carry the following equipment for the line throwing appliance 1) The equipment on the list provided by the manufacturer with the approved appliance; and(2) An auxiliary line that—(i) Is at least 450 meters (1,500 feet) long;(ii) Has a breaking strength of at least 40 kiloNewtons (9,000 pounds-force); and(iii) Is, if synthetic, of a dark color or certified by the manufacturer to be resistant to deterioration from ultraviolet light. Auxiliary line not proved to have a breaking strength of 9,000 lbs. Satisfy to the attending marine inspector.

All lifesaving appliances and arrangements must be in accordance with subchapter W (Lifesaving Appliances and Arrangements) of this chapter. Each vessel must have posters or signs displayed in the vicinity of each survival craft and the survival craft's launching controls that—(a) Illustrate the purpose of controls;(b) Illustrate the procedures for operating the launching device;(c) Give relevant instructions or warnings;(d) Can be easily seen under emergency lighting conditions; and(e) Display symbols in accordance with IMO Resolution A.760(18). Rescue boat needs operating instructions. Satisfy to the attending marine inspector.

Portable fire extinguishers and their stations must be numbered in accordance with § 97.37-23 of this subchapter. Portable fire extinguishers not properly numbered in accordance with fire plan. Correct to the satisfaction of the attending marine inspector.

Diesel Fuel Tanks: There must be no openings in the bottom, sides, or ends of a tank. The vessel's emergency generator independent deisel fuel tank contains an inspection port on the aft-facing side of the tank. Provide sufficient documentation to indicate compliance. If the vessel cannot provide compliance documentation, plans shall be submitted to the Marine Safety Center (MSC) IAW MSC Guidelines for Independent Fuel Tanks plan review guide. Rectify deficiency within 90 days. 46CFR90.20(a) 46CFR58.50-10(a)(4)

At each inspection for certification, periodic inspection, and at other times necessary: on all fire-extinguishing systems, all piping controls, valves, and alarms must be checked to ascertain that the system is in operating condition. During the CO2 testing, the 88 second time delay was found to provide the delay for only 70 seconds. Rectify deficiency prior to the vessel's departure. 46CFR91.25-20(a)(3)

Each general emergency alarm signal must be an electrically operated bell, klaxon, or other warning device capable of producing a signal or tone distinct from any other audible signal on the vessel. General alarms in the following spaces were found to be inoperable: (01) engine room ivo fuel tank; (01) in the aft/center of machinery space. 46CFR90.25-1(a) 46CFR113.25-12(a)

Where mechanical ventilation is provided for spaces other than cargo and similar spaces which are protected by a carbon dioxide extinguishing system, provision shall be made so that the ventilation system is automatically shut down with the operation of the system to that space. Both aft port/starboard fire dampers located adjacent to the galley were found inoperable. The starboard damper in the main engine room did not completely seal. Rectify deficiencies prior to the vessel's departure. 46CFR95.15-35

Fire hydrants, nozzle, and other fittings must have threads to accommodate the hose connections. Both #5 and #6 hydrant/hose connections were leaking excessively during operation. Rectify deficiency prior to the vessel's departure. 46CFR95.10-10(m)

Each switchboard must be fitted with a drip shield unless the switchboard is a deck to overhead mounted type which cannot be subject to leaks or falling objects. Half of the emergency generator switchboard was missing a drip shield. 46CFR90.25-1(a) 46CFR111.30-5(b)

The following emergency lighting and power loads must be arranged so that they can be energized...At least one light in each space where a person may be maintaining, repairing, operating equipment, or transiting. Emergency lighting was found inoperable ivo aft bulkhead accommodation space. 46CFR90.25-1(a) 46CFR112.15-1(f)

All crew spaces are to be constructed and arranged in a manner suitable to the purpose for which they are intended. Mounting bracket holes were found on the forward trunk leading to the storage space which obstructed the space's watertight integrity. 46CFR92.10-15

Means shall be provided for closing all other openings to the space protection from outside the space of a CO2 protected area. The construction shall be such that openings in the upper portion of the space can be closed off either by permanently installed means or by the use of canvas or other material which is normally carried by the vessel. The forward bulkhead of the vessel's main engine room contains a large opening where the dredge cables run from inside to outside of the space which disrupts the integrity of the CO2 protected zone.

The owner of a vessel subject to inspection under section 3301 shall-(1) ensure that such vessel is equipped with a vessel master key control and Signage requirements regarding the Coast Guard's Sexual Assault and Sexual Harassment Prevention and Response programs for commercial vessels. Follow guidance in implementation IAW Coast Guard Policy Letters 23-04 and 23-06. 46USC3106 46USC111101

All installations shall be of marine type engines suitable for the intended service, designed and constructed in conformance with the requirements of this subchapter. Marine Inspector identified fuel leak on priming pump seal and oil leak on centrifuge filter, STBD Forward Generator. 46 CFR 58.10-10 46 CFR 58.10-5(a)

Fire pumps shall be fitted with a pressure gauge on the discharge side of the pumps. No gauge on discharge piping #1 fire pump and a non-functional gauge #2 fire pump. 46 CFR 95.10-5(e)

An electric apparatus must be designed, constructed, and installed to prevent any person from accidentally contacting energized parts. Marine Inspector identified dead-end wires/junction box hanging from HPU for Aft dredge pump, open panel back of switchboard, an opening in the 480V shore tie power panel and loose LP-2 panel. 46 CFR 111.05-3(a)

All installations shall be of marine type engines suitable for the intended service, designed and constructed in conformance with the requirements of this subchapter. Jacket Water leak on piping coming from Generator Head-tank and water pump leak port forward generator. 46 CFR 58.10-10 46 CFR 58.10-5(a)

Weekly inspections and test: Each survival craft rescue boat and launching appliance must be visually inspected to ensure its readiness for use. Monthly Inspections: Each lifesaving appliance,... must be inspected monthly using the checklists required under paragraph (b)(1)(i) of this section to make sure the appliances and the equipment are complete in good working order. Vessel needs to provide a maintenance records for all lifesaving appliances. 46 CFR 199.190(b)(d)(e)

Fuel filters must be approved to ASTM 1201; Bowls for fuel filters were plastic; replace at the next drydock.

On vessels other than motorboats, a fixed CO2 or other approved system must be installed on paint lockers; paint locker nozzle was not secured insde the paint locker.

1. Tighten/repack glands on general service/fire pumps.

Repair ventilation louvers in EDG room- not closing completely.

3. Repair starboard forward Watertight Door on spud house.

4. Repair/renew flammable locker in spud house room.

Discovered a pinhole in the bilge piping on the discharge side of the pump. Required a two foot section of piping to be cropped and renewed.

The battery for emergency power to the bridge control station located outside the pilothouse was corroded and needed to be replaced.

The #1 and #2 ship's service generators were being serviced and were unavailable for required testing. The marine inspector will return to conduct testing.

An electronics transformer in the electronic room was plugged in to a power strip. Required the transformer to be wired into the electrical system.
