Doors in “A” Class bulkheads shall meet the following requirements:Doors shall have a latch with a minimum throw of 3/4 inch which can be operated from either side of the door. Class a door on A-end of passenger lounge did not latch.

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. Damage present to gel coat on keel of rescue boat at stowage cradle location. Investigate if only gel coat damage or goes into fiberglass. Swapping of spare rescue boat authorized with notification to MSD Nags Head.

All emergency lights shall be marked with a letter “E” at least 1/2 inch high. Emergency lights on 01 deck not properly marked.

Each lifejacket container must be marked in block capital letters and numbers with the quantity, identity, and size of the equipment stowed inside the container. Lifejackets stowed on bridge were not in the marked container.

For nontank vessels with an oil capacity of 250 barrels or greater, a vessel owner or operator must comply with the VRP exercise requirements of 33 CFR 155.1060. A vessel owner or operator required to have a response plan shall conduct quarterly qualified individual notification exercises. The quarterly qualified individual notification exercises were not conducted. Conduct quarterly qualified individual notification exercises as required.

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Marine Vessel Rules (incorporated by reference, see § 58.03-1), except as otherwise provided by this subchapter. During the test of the emergency generator, it failed to automatically come on line. Complete repairs as needed.

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Marine Vessel Rules (incorporated by reference, see § 58.03-1), except as otherwise provided by this subchapter. The keel cooler boot of the A end main engine ruptured. Make permanent repairs to the keel coolant boot of the A end maine engine.

Fire control. (1) Fire control diagram showing location and type of all required fire-screen insulation, including main fire zone and subdivisions, stairway and elevator enclosures, control space enclosures, etc., and type of all doors in such subdivisions and enclosures.(2) Comprehensive typical details of fire-screen insulation of both vertical and horizontal surfaces, including deck coverings where used, keyed by reference numbers to the “fire control diagram”.(3) Ventilation diagram including dampers and other fire control features.(4) Alarm systems.(5) Detecting systems.(6) Extinguishing systems, including fire main, carbon dioxide, clean agent, foam, and sprinkling systems.(7) Supervised Patrol Route. MI observed the vessel did not have fire control plan onboard.

An emergency means of communication required by this subpart must—(1) Be comprised of either fixed or portable equipment; and(2) Provide common talking means of two-way voice communication and calling among the navigating bridge, emergency control stations, muster stations, embarkation stations, and other strategic positions listed in § 113.30-5. MI observed that the A end ringer is not working as designed.

Means shall be provided for closing all openings to the space protected from outside such space. In this respect, relatively tight doors, shutters, or dampers shall be provided for openings in the lower portion of the space. The construction shall be such that openings in the upper portion of the space can be closed off either by permanently installed means or by the use of canvas or other material which is normally carried by the vessel. MI observed the door gasket not sealing.

The material, design, construction, and workmanship, and arrangement of main propulsion machinery and of each auxilary, directly connected to the engine... must be at least equivalent to the standards established by the ABS Steel Vessel Rules. A- end main engine lost power temporarily.

Remote shut-off valve for the emergency bilge pump piping for the B-end forepeak suction is inoperable due to a snapped reach rod. Conduct permanent repairs to the satisfaction of the attending marine inspector, prior to carrying passengers.

Passenger lounge door on the 01 deck (A-end) will not close due to excess paint on door jam. Conduct repairs to ensure space is weather tight.

Life jacket stowage number on 01 deck IVO passenger lounge is peeling. Relabel stowage compartment.

Each self-propelled vessel must have at least two electric generating sources in addition to the emergency generator required by 112 of this chapter. The #1 Genset of the W. STANFORD WHITE is inoperable due to a leak on the vessel's keel cooler. Effect permanent repairs to the satisfaction of the attending Marine Inspector, prior to carrying passengers.

Bulkheads or decks of the "A" Class shall... be capable of preventing the passage of smoke or flame for 1 hour. The gasket has failed on the EDG space sliding door.

Each machinery space must be ventilated to ensure that,...an adequate supply of air is maintained for the operation of the machinery and the safety, efficiency, and comfort of the crew. MSD space fan is OOC.

Vessel's A-end main engine would not go in reverse power. Complete permanent repairs to prior to carriage of passengers. Vessel's repairs must be cleared by a marine inspector from DD Nags Head IAW 46CFR77.01. Please conduct sea trials after repair prior to operations in addition, please provide the Masters attestation to the repairs and sea trial, and his/her comfort in running the vessel.

Voith unit losing oil into the water. Vessel may transit to yard for repairs. All repairs are to be made prior to carrying passenger and prior to using voith unit due to pollution potential. All repairs are to be made to satisfaction of attending [name withheld] 46CFR71.25-35

Submit updated PSTPs to MSC for review and approval. IAW 46CFR61

Radiotelephony Certificate and Bridge-to-Bridge Radiotelephony certificate are expired. IAW 47CFR80

Operator reported that the B end voith unit started making a loud knocking noise and they took the unit off line to prevent further damage during their trip back to Hatteras. Provide causal factors attributing to cause and corrective actions taking to rectify. Vessel must be cleared by a marine inspector from this office before returning to service.

The main steering gear must be operable from the pilothouse by means of mechanical, hydraulic, electrical or other means accepted by the Commanding Officer, Marine Safety Center. Vessel's master reported that the steering lever in the pilothouse is causing the vessel to "pull," but is still able to maintain control of the vessel. Make repairs and prove proper operation of the steering system.

Rescue boat grab handle on hull port forward broken in half replace IAW 46CFR71.25-20.

A-end MDE oil differential pressure hose damaged replace IAW 46CFR71.25-35.

Gasket on A & B end forward peak is leaking, replace with new IAW 46CFR71.25-25.

Vessel crew reporting engine knock on A-end MDE, believed to be hydraulic electronic unit injection system. NCDOT to verify HEUI system is cause/damaged equipment and make repairs to the satisfaction of the attending Marine Inspector iaw 46 CFR 58.05-1.

Make permanent repairs to A End MDE and conduct satisfactory sea trials in the presence of a Coast Guard Marine Inspector IAW 46CFR50.05-10.

Identify cause for engine casualty on A-End MDE, make permanent repairs and conduct sea trials prior to carrying passengers, IAW 46CFR50.05-10.

Vessel liferafts are not properly secured to the vessel. liferaft cradles are also loose. Mount on liferaft #3 is loose. Life raft #5 has wasted bolt. Liferaft #5 and #22 have loose straps (fixed on spot). Spacers are missing on all liferaft mounts. Repair/modify in accordance with 46CFR70.28-1

Various cracks found through out liferaft mounts. Will monitor on future inspections. Will be repaired in accordance with 46CFR70.28-1 during drydock in May 2022

Cracked ore on rescue boat. Repair/replace in accordance with 46CFR70.28-1

B end voith unit has various hydraulic oil leaks at fittings. Repair in accordance with 46CFR70.20-1

Fuel tank supply valve is leaking. Repair in accordance with 46CFR70.20-1

#5 fixed CO2 bottle found loose. Repair in accordance with 46CFR76.15-20

Rescue boat davit wire is corroded and beginning to deteriorate. replace wire and conduct weight test in accordance with 46CFR199.45(b)(1)

Watertight Door on Main Deck leading to engine room is miss aligned with open/close indicator. Indicator is not registering alarm. Ensure proper alignment and indicator works in accordance with 46CFR170.270(a).

Vessel's public restroom doors do not fasten, createing a hazard for passengers/crew. Ensure door latches/secures in accordance with 46CFR71.25-40

Remove new Co2 fire extinguishers that do not meet updated standards in accordance with 46CFR71.25-20.

Fixed Co2 bottles are loose in mounting brackets. Tighten to prevent shifting in accordance with 46CFR71.25-20.

A-end voith space emergency lighting is inoperable. Replace/repair to ensure proper operation in accordance with 46CFR111.75-15(c)(2)

A-end engine room hand rail is missing course. replace with new/existing in accordance with 46CFR72.40-20

Lifering throwline is deteriorated and lifering waterlight line is deteriorated. Both on portside main deck. Replace with new in accordance with 46CFR199.70(a)(3)

Main engine control room alarm panel has fault on blank causing false alarm replace circuit to prevent repeat in accordance with 46CFR61.40-6

B-end ladder leading from main deck to 01 deck deteriorated at connection. repair to ensure properly connected, crop and renew in accordance with 46CFR71.25-10

B-end voith unit has high oil temp and bearing temp. NCDOT removed vsl from service to rebuild voith unit with CG oversite.

Replace leaking shaft input seal on A and B end voith unit iaw 46 CFR 46 CFR 58.05-1.

Replace expired batteries on lifering lights iaw 46 CFR 199.211(b).

Vessel is unable to deploy lifeboat for MOB recovery due to davit seized. Repair and test underway prior to carrying passengers.

CO2 system is not operating when using remote activation. Vessel crew and NCDOT were able to identify cause as failed/deteriorated O-ring/seals on the time delay. Parts replaced with new, tested sat.

Vessels main engines are giving both low lube oil and overspeed alarm during testing of low lube oil alarm. System/engines sill provide audible/visual alarm and shut engine down. Identify cause for additional (overspeed) alarm in EOS and rectify.

Replace petcock valves with valve plug on lube oil system for MDE and generators.

Troubleshoot and repair sound powered phone on Bravo end of vessel. Phone lines were miss matched and did not call selected line.

Voith unit on both A and B ends have minor leaks and deteriorated fastener bolts. Replace deteriorated bolts and mitigate water ingress iaw 46 CFR 72.01-15.

Weathertight hatch at FR 44A is leaking. Tighten/Replace to prevent iaw 46 CFR 72.30-1.

Ensure all portable fire extinguishers are weighed and recordings are accurate iaw NFPA 10 and 46 CFR 76.05-25 / 76.01-2.

Provide high temp alarm verification and calibration certificate for Calibrator MTC 320-A cal.

Sound powered phone on "A" End main deck is not working properly, repair iaw 46 CFR 78.35-1.

Each credentialed crewmember must become familiar with the relevant characteristics of the vessel appropriate to his or her duties and responsibilities prior to assuming those duties and responsibilites. Vessel engineers present for the Certificate of Inspection were unfamiliar with bilge suction configuration.

Drills, musters, and training sessions shall be properly logged IAW 46 CFR 199.180(h).

Number one bilge pump power supply is inoperable.

Properly configure CO2 fixed system to comply with onboard diagram. Currently valve release is after ventilation shutdown pressure switch.

During dockside testing the rescue boat davit accumulator bladder ruptured. Replace accumulator bladder and test davit.

Steering system does not respond to helm commands correctly. Recalibrate system and prove proper operation.

Both local digital displays at the A and B voith units read ahead when the vessel is traveling astern and read astern when the vessel is traveling ahead.

Found A/C cooling water discharge pipe leaking iwo ovbd check valve. Repair leak.

Found control oil pressure alarm constantly active/ inop. Repair alarm circuit; gauges reflect actual pressure.

Upon entering the sewage space a significant vacuum was observed and both CG Marine Inspector's gas detectors alarmed for low O2 levels; the spaces vent fan seems to be exhausting more air than the vents can keep up with causing a negative pressure environment. Identify and repair condition; ensure crew safety measures are in place while working to correct condition.

Identified area of minor damage on fiberglass rescue boat and approved request for repair of gel coast ONLY to be conducted by NCDOT Hatteras Field Maintenance Shop Supervisor in accordance with manufacturer (Fassmer) repair instructions. Provide proof that repair has been examined and assessed for future repairs by authorized repair facility within 30 days.

Found incorrect batteries installed in both floating waterlights. Install correct batteries.

Found the B-end masthead and A-end stern lamps out. Repair navigation lights.

Found the B-end remote fuel shut-off valve leaking at the packing gland. Adjust or replace valve.

Unable to test MDE high J/W temp alarms. Prove proper operation of high temp alarm.

Found the EOS door handle/latch missing. Install door handle.

Vessel suffered a loss of voith control system 1. Complete repairs to the voith control system.

Found the engine room ventilation supply fan inoperable. Repair/ replace fan.

Vessel's "B-end" MDE is hunting at all RPM levels excessively. Repair and prove proper operation of "B-end" MDE to the satisfaction of the attending CG Marine Inspector.

NCDOT is required to develop a plan for the installation of required emergency lighting for rescue boat launching station.

Repair passenger accommodation space heater and pilot hosue heater.

Address procedures for prevention of blocking fuel shut-off valves while loaded.

Provide kill-switch lanyard for rescue boat.

Make Repairs to Leaks on #2 Ship Service Generator, #1 and #2 Fuel Nozzle.

Remove all flamables from Engine Room Lockers.

Install flame screen in E/R oil tank vent pipe.

Install steel fuel valves as per Sector NC letter at next yard period or drydock.

Repair lavatory drain valves in "A" end aux. space IVO mufflers

Fix seat backs for 03 Passenger seats in 01 deck passenger lounge

Renew COFR (expired)

Repair 01 deck doors (not closing).

Install a suitable, J standard, fuel oil flex hose (x4).

Install a suitable plug for oil tank.

Adjust watertight doors from the main deck down (28A, 12B, and 28B)

Repair "A" end lobby weather tight door.

Renew fuel pressure switch, "B" ship service generator.

Remote fuel shut-down on "B" end leaks, repair.

Adjust engine room WTD (main deck access).

Provide approved automation test procedures

Secure microwave and refrigerator for sea.

Renew captive chains IWO fire nozzles.

Adjust watertight door IWO engine room main deck access.

Provide complete instructions IWO local engine room CO2 system.

Install a painter cleat aboard the vessel in a suitable location (providing for proper fairlead while the rescue boat is along side) and provide proper rescue boat equipment IAW 46 CFR table 199.620(j) to include: bailer, boathook, painter, and towline.
