46 CFR 91.55-1 The following list of required plans is general in character, but includes all plans which normally show construction and safety features coming under the cognizance of the Coast Guard. In the case of a particular vessel, all of the plans enumerated may not be applicable, and it is intended that only those plans and specifications be submitted as will clearly show the vessel's arrangement, construction and required equipment. Amend Operations Manual to reflect actual operations.

46 CFR 91.25-10 The inspection for certification shall include an inspection of the structure, boilers, and other pressure vessels, machinery, and equipment. The inspection shall be such as to insure that the vessel, as regards the structure, boilers and other pressure vessels, and their appurtenances, piping, main and auxiliary machinery, electrical installations, lifesaving appliances, fire-detecting and extinguishing equipment, pilot boarding equipment, pollution prevention equipment, and other equipment, is in satisfactory condition and fit for the service for which it is intended, and that it complies with the applicable regulations for such vessel and determine that the vessel is in possession of a valid certificate issued by the Federal Communications Commission, if required. The lights, means of making sound signals, and distress signals carried by the vessel shall also be subject to the above mentioned inspection for the purpose of ensuring that they comply with the requirements of the applicable statutes and regulations. The following must be installed -Tilt alarm for jacking operations. -Low hydraulic oil alarm -Install an Anemometer

Leg inspection intervals will be in compliance with CG-543 Policy Letter 07-02 include in the updated operational manual.

The general construction of the vessel shall be such as to minimize fire hazards insofar as is reasonable and practicable. The hose for the CO2 extinguisher installed on the paint locker was cracked. 46 CFR 92.05-1

The inspection shall be such as to insure that the vessel, as regards the structure, boilers and other pressure vessels, and their appurtenances, piping, main and auxiliary machinery, electrical installations, lifesaving appliances, fire-detecting and extinguishing equipment, pilot boarding equipment, pollution prevention equipment, and other equipment, is in satisfactory condition and fit for the service for which it is intended. Hydraulic hoses on the exterior of the vessel were secured with rope. 46 CFR 91.25-10

Except as provided in paragraph (b) of this section, each vessel must have a stability letter issued by the Coast Guard before the vessel is placed into service. This letter sets forth conditions of operation. Vessel's current stability letter (Serial# C2-2200522) dated, February 17, 2022, is only valid while all three jacking legs are installed. Vessel shall not remove the jacking legs until the following requirements are met: 1. Update the operations manual to incorporate the new “legs removed” transit with reference to the Stability Calculation Addendum. 2. Submit the updated ops manual to MSC for verification. 3. Receive new stability from MSC. 46 CFR 170.120(a)

Each vessel must have a stability letter issued by the Coast Guard before the vessel is placed into service. This letter sets forth conditions of operations. Vessel had previously undergone modifications for a jacking system and had a stability letter approved by MSC. Vessel shall be in compliance with Stability letter prior to operations. Vessel has since removed spuds without notification to Coast Guard and the current stability letter is invalid.

To maintain a valid Certificate of Inspection, you must complete your annual and periodic inspections within the periods specified in 46CFR91.27-1(a) and your Certificate of Inspection must be endorsed. The required annual inspection was not completed within 3 months of the November 16th anniversary date.

Each pressure vessel listed on the Certificate of Inspection must be thoroughly examined externally every 5 years. MI observed COI PV inspection date of 09JAN2025.

Each vessel must— (1) Carry not less than 12 rocket parachute flares approved under approval series 160.136. Missing 06 parachute flares.

Electric installations and electric equipment must be inspected at the inspection for certification and periodic inspection to determine mechanical and electrical condition and performance. Vsl removed the STBD generator for rebuild so periodic testing for engine overspeed and low lube oil level couldn't be completed.

Containers, brackets, racks, and other similar stowage locations for lifesaving equipment must be marked with symbols in accordance with IMO Resolution A.760(18) indicating the device stowed in that location. Location of embarkation ladder and flares were not displayed correctly on fire control plan.

Piping, including valves, pipe fittings and flanges, conveying vapors, gases or liquids whose temperature exceeds 150 °F., shall be suitably insulated where necessary to preclude injury to personnel. There was no lagging present on exhaust side of turbo on the no. 1 generator and on the bellow of the exhaust manifold of the main dredge engine.

Each panelboard must have a non-combustible enclosure that meets 111-01-07 and 111.01-9. The knockout was removed with no cable running through.

Piping, fittings, and valves for a fixed fire extinguishing must be in accordance with the manufacturer's approved design and installation. The piping connection between the paint locker and the extinguishing system had an enlarged opening around the connection piping, preventing the space from having a gas-tight closure. 46 CFR 95.16-10(a)(1)

Each cargo vessel must carry an immersion suit approved under approval series 160.171 of an appropriate size for each person on board. Two out of the six required immersion suits on board were oversized for any crew member onboard, which would not provide adequate buoyancy for the wearer.

Each vessel must carry distress signal. The distress signals were expired, replace.

Containers, brackets, racks, and other similar stowage locations for lifesaving equipment must be marked with symbols in accordance with IMO Resolution A.760(18) indicating the device stowed in that location. Stncil PFD/immersion suits and number inside.

The hydraulic hoses used for the jacking system had several leaks. Tighten fitting to prevent leakage.

Means shall be provided for closing all ventilation openings. The new intakes installed for cooling of the radiators need to have closing devices for ventilation shut down incase of an engine room fire.

Port and stbd generator have no lagging on exhaust manifolds. Repair of replace to the satisfaction of attending MI 46CFR56.50(k)

Port engine room door has wastage and a notch missing out of the knife edge. Repair or replace to the satisfaction of attending marine inspector. Deficiency is to be corrected at next drydock. 46CRFR171.111

WT door to lower deck has wastage on bottom. Repair or replace to the satisfation of attending [name withheld] is to be corrected at next DD 46CFR171.111

Ladder to the lower deck has 02 penetrations that are not watertight. Repair to the satisfaction of atteengin marine inspector. 46CFR171.111

There must be ground detection for each ship's service power system. Switchboard located in machinery space had a ground on b phase during inspection.

Stations in the maritime service must be licensed by the FCC either individually or by fleet. Vessel's FCC Radio Station Authorization certificate is expired.

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels. Vessel did not have a valid document stating they use a consortium for random drug testing.

An aggregate of more than 7.6 liters (two gallons) of flammable or combustible liquids stowed outside of an accommodation, control, or service, space (other than a paint locker) or outside of a machinery space must be stowed in a paint locker that is marked with a warning sign indicating flammable or combustible liquid storage. Vessel is storing 4 X 55 gallon drums of combustible liquid on the top deck.

Two gate valves observed leaking diesel on the fuel transfer pipe in way of the fuel transfer manifold.

Starting batteries for the fiesel installation on deck hooked up loose on deck. Place in boxes or trays protected from the weather.

Ventilation openings for the intake supply has no means to be closed in case of fire.

RT01: Permanently install CO2 bottle for newly installed paint locker.

Damage to vessel's watertight bulkhead on starboard side following replacement of piping. Repairs were started prior to notification to [name withheld] affected area available for inspection, to the satisfaction of attending marine inspector prior to operating vessel. 46CFR91.45-1(a)/46CFR91.45-5(a)

Each life buoy is req to be marked w/ name of vessel & port in block letters. FWD life buoy missing markings. Re-mark to satisfaction of CG MI

1/2 of life buoy must be fitted w/ a self igniting light. FWD life buoy light battery not approved by manufacture. Replace to the satisfaction of CG MI.

Side lights shall be fitted w/ mat black inboard screens. Screens are painted white. Paint mat black to satisfaction of CG MI.

Suitable guards shall be installed IWO all exposed dangers. Both service generators turbo lagging missing. Replace to the satisfaction of CG MI.

Vessel less than 400' must carry appointed spill equipment. Spill kit incomplete. Replace to satisfaction of CG MI.

CG MI assured safety of vessel. Gear and equipment not secure. Secure to satisfaction of CG MI.

Vessel's 480V sub-pump electrical connection is spliced and wrapped with electrical tape, not in an enclosure. The vessel has all the necessary parts to properly secure. Vessel is restricted from conducting dredging operations until this electrical hazard is eliminated.

Vessel is required to have 12 rocket parachute flares, but has six parachute and six hand flares. Vessel must provide six additional rocket parachute flares. 46CFR199.60

Vessel is required to have not less than 12 distress signals. Vessel only has 11 rocket parachute flares. Provide additional rocket parachute flare.

Distress signals are not in compliance with regulations. Provide 12 rocket parachute flares (approval 160.136) prior to operating more than 30 minutes from shore.

Containment for fuel vent has drain with no means of closing. Provide means to seal containment.

Replace gasket on watertight door between bow and center holds

Install dripshield on main switchboard.

Repair exhaust leak on stbd generator diesel engine.

Replace plastic Raycor plastic bowls with approved type.

Provide vessel with line throwing appliance.

Provide vessel with distress signals.

Port and starboard running light side screens were faded and required painting matte black

Repaint name of vessel and hailing port on the stern.

Repalce stern lifering waterlight battery with an approved battery.

Repair starboard running light.

Provide a battery box and cover for the 12 volt batteries powering the stern and port running lights.

The navigation lights are not working due to low battery voltage, repair or replace a necessary, portable lights are temporarily authorized.

Properly label the storage location for the lifejackets.

The emergency light in the lower deck center storage room is not working make repairs as necessary.

Provide a globe cover for the light fixture in the forward stairway to the lower deck.

Provide markings for the storage location of the fire extinguishers as needed.

Remove wire tie from the water light and properly attach with 3 - 6 ft. line.

Remove improper petcocks from the fuel filters on the prime mover for the on deck air compressor and replace with petcocks that will accept plugs.

Repair or replace inoperative battery charger and stbd generator alternator for the generator batteries.

Install lagging where missing on the prot and stbd generator turbochargers.

Provide onboard the following hand portable fire extinguishers: a) One (1) A-II for each Storeroom. b) One (1) BC-II for the compressor / waste oil tank room. c) One (1) B-II for the Jacking Air Compressor enclosure mounted outside the space.

Relocate the battery box installed under the electrical switch gear, or, make the switch gear suitable for a hazardous location.

Remove the portable lube oil containers from the jacking system air compressor space.

Provide oil transfer procedures for the barge that complies with 33 CFR 154, 155, and 156.

Mark the two (2) Class I WTDs in the below deck storeroom - "KEEP CLOSED".

Provide on board the vessel's Certificate of Documentation.

Provide the Manufacturer's Data Sheet for the air receiver.
