Safety and relief valves for steam shall be provided with a substantial lifting device so that the disk can be lifted from its seat when the pressure in the vessel is 75 percent of that at which the valve is set to blow.
The valve seat was seized. Repair or replace valve and test.

Vent outlets from all tanks which may emit flammable or combustible vapors must be fitted with a single screen of corrosion resistant wire of at 30x30 mesh or two sections of at least 20x20 mesh. Flame screen on the cargo tank vent riser was not secured and had a 2' gap in the screen mesh.
