Flammable and combustible liquids are being stored in starboard push knee. Push knees are completely open to atmosphere. Current storage arrangements do not provide equivalent level of safety.

The entry must include the date and time of assessment, the name of individual making assessment, and the starting and ending points of the voyage or trip that the assessment covers. There are no logs indicating that navigation assessments are being conducted. Provide objective evidence that person in charge of the navigation watch is conducting navigation inspections in accordance with all requirements listed in 46 CFR 140.635.

Rails or equivalent protection must be installed near the periphery of all decks accessible to the crew. Equivalent protection may include lifelines, wire rope, chains, and bulwarks that provide strength and support equivalent to fixed railed. Aft deck does not have any fall overboard protection. Install stanchion and chain system as discussed with Marine Inspector. If a different system than discussed is going to be installed, contact Marine Inspector for approval prior to its instillation.

Each towing vessel must be equipped with the following alarms: Maine engine and generator engine high cooling water temperature. Main engine and generator engine low lube oil pressure alarm. High bilge level alarm. Low hydraulic steering fluid level alarm. Low fuel level alarm if fitted with a day tank. Additionally, in accordance with table 143.245(b), alarm set points must be tested twice every 5 years, with no more than 3 years elapsing since last testing. Crew was unable to test generator alarm set points or steering fluid level alarm.

46 CFR 141.340 Each life jacket must have a light approved under under approval series 161.012 or 161.112 securely attached to front shoulder area of life jackets. Life jacket must be removed from service until it can be fitted with light. Vessel is limited to carrying no more than 07 total persons until life jacket is placed back into service.
46 CFR 140.430: When worn at night, work vest must be equipped with light that meets requirements of 141.340. Found 04 work vest without lights. Work vest must not be worn at night until equipped with approved lights.
