Machinery and electrical systems must be maintained to provide for safe operation of the towing vessel and safety of persons on board under normal and emergency conditions. Vessel experienced a loss of propulsion in the center engine due to the clutch locking up and shutting the engine down. Make repairs to the satisfaction of the TPO. In accordance with CVC Policy Letter 17-10, the above deficiencies may be cleared by the operating company's TPO. TPO must submit a report to the cognizant OCMI within 72 hours of clearing the deficiencies.

Piping and tanks exposed to the outside of the hull must be made of metal and maintained in a leak free condition. While underway a fuel leak was found from the #1 tank into the engine room bilge. The #1 tank fuel was transferred to the #2 tanks to stop leak. Permanent repairs will be conducted at local ship yard to the satisfaction of the TPO and or the local OCMI.

While transiting upriver from MM 85, vessel experienced a reduction in propulsion after discovering that the engine was damaged internally. Prior to movement, submit an approved transit plan to a confirmed repair facility to the cognizant [name withheld] approval, proceed to repair facility according to the stipulations laid out in the transit plan and affect permanent repairs to the satisfaction of the TPO.
