Propulsion machinery, including main engines, must; be suitable for route and service. Vessel reported the cam shaft on the starboard main engine failed, resulting in a grounding/barge breakaway. After the tow has been rebuilt and passed off to another vessel, it is to proceed to a location to repair the cam shaft, without a tow. A TPO report attesting to satisfactory repairs and operation is to be submitted to the Coast Guard for consideration for clearance. 46 CFR 143.205(c)(2)

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Vessel experienced a crankcase explosion on the starboard main diesel engine. Subsequent engine room fire occurred. The following items were initially identified as damaged due to the fire: - STBD main diesel engine, #4 cylinder/piston assembly - Extensive charring/blistering of surrounding structure - Extensive charring/blistering of surrounding panels - Broken glass/window for engine room door/watch room - Excess oil accumulation in the engine room bilge - Engine room ventilation inoperable - Multiple electrical panels with heat damage - Multiple LED light fixtures with heat damage - One of each inoperable: heat & smoke detectors - Inoperable CO2 discharge strobe - Installed CO2 system complete discharge - Multiple portable fire extinguishers expended (See 835 for transit plan)

STBD LIFE RING WAS FITTED WITH AN INOPERABLE MARKER LIGHT.
