All machinery associated with cargo loading, unloading, or cooling shall be capable of being shut down from a remote location. Fix shut down on STBD cargo pump.

Except as provided in paragraph (d) of this section, no person may use any equipment listed in paragraph (c) of this section for transfer operations unless the vessel or facility operator, as appropriate, tests and inspects the equipment in accordance with paragraphs (b), (c) and (f) of this section and the equipment is in the condition specified in paragraph (c) of this section. Complete hose hydrostatic test in accordance with this section.

Spaces containing cargo pumps, compressors, and piping are considered as equivalent to a tank vessel pumproom, and no electrical devices, except Coast Guard approved intrinsically safe devices, shall be installed in these spaces. Fix dead in cable on fwd mast, replace 04 vapor tight packing nuts on FW Delev indicator panels

Each hose also must be marked IAW this part. Mark hoses accordingly.

All exposed and dangerous places such as gears and machinery shall be properly protected with covers, guards or rails in order that the danger of accidents may be minimized. Provide cover for Port cargo pump prime mover turbo.

Each battery charger enclosure must meet § 111.01-9. Additionally, each charger must be suitable for the size and type of battery installation that it serves. Chargers incorporating grounded autotransformers must not be used. Replace battery charger amperage indicator.

Cargo pump relief valves shall be tested at least once each year to determine that they function satisfactorily at the pressure at which they are set to open

Your vessel must undergo an annual inspection within 3 months before or after each anniversary date. Vessel must complete a annual inspection before it can do cargo operations.

Oil tankers and offshore oil barges with an overall length of less than 400 feet must carry appropriate equipment and supplies for the containment and removal of on-deck oil spills of at least 7 barrels. Provide complete SOPEP equipment and inventory list IAW this part.

Cargo pump pressure gage shall be tested at least once a year for accuracy

Every ship of 150 gross tons and above certified to carry Noxious liquid substances in bulk shall carry onboard a shipboard marine pollution emergency plan for Noxious liquid substances approved the administration. MARPOL Annex II/17

Each cargo tank of a tankship must be equipped with an intrinsically safe high-level alarm and a tank overfill alarm. Activate an alarm in the event of loss of power to the alarm system and Activate an alarm during the failure of electrical circuitry to the tank level sensor. Provide proof that these features are installed on system.

No extensive repairs to the hull or machinery which affect the safety of a vessel shall be made without the approval of the Officer in Charge, Marine Inspection (OCMI). Before such repairs are carried out, copies of plans and specifications for the work involved shall be forwarded to the OCMI. Vessel suffered a cargo pump room fire on the starboard side and must work with the local OCMI through the repair process to bring the vessel back to fit for service as required by the COI. A temporary repair plan has been submitted, reviewed, and approved by both ABS and USCG. The temporary plan must be followed in its entirety while conducting cargo operations. Any changes or modifications must have the approval of the [name withheld] final repairs are complete, they must be witnessed and verified by USCG marine inspector prior to deviating from the temporary arrangements.

Each area enclosed by the coaming required must have a mechanical means of closing each drain and scupper in the enclosed deck area. Mechanical plugs missing from all peripheral coaming.

Each vessel vapor connection flange face must have a permanent stud projecting outward that has a 12.7 mm (.5 in) diameter and is at least 1 in long. It must be located st the top of the flange face midway between boltholes, and in line with the bolt pattern. Port and starboard vapor control connections missing required studs.

Port and starboard tank barge liquid overfill protection alarm male plug connectors missing labeling of "Connector for Barge Overflow Control System", total inductance and capacitance of the connected switches and cabling.

Double flame screens of 20 × 20 corrosion-resistant wire mesh with a 1/2-inch corrosion-resistant separator on a single screen of 30 × 30 corrosion-resistant wire mesh shall be fitted on all openings to atmosphere.High velocity PV valve missing flame screens on two lower drain pipe openings.

Vessel has missed the window for the past two annual inspections. The vessel representative need to submit a request for a new COI inspection prior to resuming normal operation to carry any cargo.

In tank vessels carrying Grade A, B, C, or D liquid cargo, cargo pumps shall be isolated from source of vapor ignition by gastight bulkheads. Starboard side cargo pump bellow was not found to be vaportight.

ABS inspected barge DBL76 grounded on January 28th 2014 and requires ABS survey before continuing operations.
