Electrical systems must function properly and minimize system failures and fire. Port side red gear had exposed wires. Complete repairs to satisfaction of MI.

Pressure vessels over 5 cubic feet in volume and over 15 lbs per square inch maximum allowable working pressure (MAWP) must be equipped with an indicating pressure gauge and with one or more spring loaded relief valve. The total relieving capacity of such relief valves must prevent pressure from exceeding the MAWP. MI observed the pressure went past the MAWP and gauge did not exceed 200 psi. Complete repairs for gauge to satisfaction of MI.

Electrical equipment and electrical cables must be suitably protected from wet and corrosive environment. MI observed power cord plugged into outlet and cable ran across bulkhead to charge back up battery. O/O to provide plans for installing a way to charge back up battery to the satisfaction of MI.

An exposed hazard such as gears and rotating machinery, must be protected by a cover, guard or rail. MI observed starboard generator did not have guard around the belt. Complete repairs to satisfaction of MI.

Where practicable each space where crew may be quartered or normally employed must have at least two means of escape. MI observed door on second deck to the crew accommodation space was not able to open. Provide plans and complete repairs to satisfaction of MI. Vessel may continue to operate, if a second crew is provided after 12 hours so crew may change and continue operations.

Closure devices must be provided for hull penetrations, which open to the exterior of the vessel and which may allow water to enter the vessel. MI observed several 1 inch holes along the exterior bulkhead of the machinery space. Complete repairs to satisfaction of MI.

Each towing vessel must be maintained and operated so as to minimize fire hazards. All fuel tank sight glasses are missing protective guards. Install guards to the satisfaction of the attending MI.

Electrical components that pose an electrical hazard must be in an enclosure. 02 dead end wires found in FWD Peak and Engine room. Remove old wiring or properly secure in an enclosure.

Electrical conductors passing through watertight bulkheads must be installed so that the bulkhead remains watertight. Multiple penetrations on the bulkhead in the fwd peak are not properly sealed and watertight. Repair to the satisfaction of the attending MI.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Found breaker panel in pilot house missing several blanks, two outlets with no cover/protection & discovered multiple extension cords being used throughout both berthing areas. Install blanks in breaker panel, add cover/protection on exposed outlets & remove all extension cords from berthing areas. 46 CFR 143.205

Each towing vessel must be equipped with an industrial type first aid cabinet or kit, appropriate to the size of the crew and operating conditions. Found expired aspirin in first aid kit. Replace aspirin. 46 CFR 140.435

Each towing vessel must have either a self-priming, power-driven, fixed fire pump, a fire main, and hoses and nozzles in accordance with paragraphs (a) through (d) of this section; or a portable pump, and hoses and nozzles, in accordance with paragraphs (e) and (f) of this section. No spanner wrenches available at fire stations. Provide spanner wrench for each fire station. 46 CFR 142.325

46 CFR 137.200 & 46 CFR 136.212 - A towing vessel subject to this subchapter and choosing the CG inspection option must undergo an annual inspection. Annual inspections will be similar in scope to the inspection for certification. The inspection will ensure that the vessel is in satisfactory condition and fit for the service for which it is intended, and that it complies with the applicable statutes and regulations for such vessels. In order to complete the annual inspection and complete credit, the following items must be carried out: -Provide a properly credentialed crew that will be normally assigned to the vessel. -The assigned crew is required to demonstrate satisfactory emergency drills (fire, man overboard, abandon ship, etc.). -Provide objective evidence of safety orientation, Health and Safety Plan initial training, nav safety training for deckhands, etc. for the assigned crew. -All portable fire extinguishers must have up-to-date monthly inspections. -Oil-soaked rags and standing fuel/lube oil in the bilge underneath both main engines must be cleaned up. -Prove dewatering capability, suitable for all spaces, via portable pump. -Provide 6 person IBA (reportedly currently ashore for servicing)

46 CFR 143.230 - Each towing vessel must have a reliable means to provide notification when an emergency condition exists. The following must be provided: High bilge levels. Alarm must: be visible and audible at the operating station. Bilge alarm at the operating station was inoperable, no audible alarm was present and bilge alarm could not be cleared.

33 CFR 155.720 - The operator of a vessel with a capacity of 250 or more barrels (10,500 gallons) of oil shall provide transfer procedures that meet the requirements of this part. The vessel has a combined fuel and lube oil capacity of >10,500 gallons, but the transfer procedures available onboard are relatively generic procedures from Vane Brothers (a previous operator) that do not meet all the requirements of 155.750.

46 CFR 143.410 (b) - Emergency lighting must be provide for all internal crew working and living areas. E-light in engine room is stuck in the on position and crew berthing needs sufficient illumination to facilitate egress.

46 CFR 142.215 - All portable fire extinguishers required by this part must be approved by the Commandant (CG-ENG). The two 10BC portable fire extinguishers in the fidley/upper level of the engine room do not have the appropriate bracket, as listed on the extinguisher label (example: port side extinguisher requires a 810 bracket, but a 20 bracket is presently installed).

46 CFR 143.720 - Each towing vessel must be equipped with navigation lights, shapes, and sound signals in accordance with 33 CFR part 84. Vessel's upper forward mast light was inoperable. Vessel is limited to operating in daylight hours only with no restricted visibility.

46 CFR 140.605 - A towing vessel must be maintained and operated so the watertight integrity and stability of the vessel are not compromised. Port and starboard watertight hatches for lazarette space found with wasted knife edges and securing straps. To be repaired as original, or replaced, by next dry dock.

Each towing vessel must carry on board and maintain currently corrected editions of charts of the areas to be transited, and currently corrected editions (or extracts) for relevant publications (Light List, NTM, Tide tables, Tidal-current tables, U.S. Coast Pilot). Up-to-date nautical charts and publications not available onboard at time of inspection. 33 CFR 164.72(b)

The master must ensure that all hatches, doors, and other openings designed to be watertight or weathertight function properly. Doors on port and starboard side of galley leading to exterior of vessel do not properly seal. Most dogs on the two watertight hatches for aft void are seized. 46 CFR 140.610(b)

Each towing vessel must be maintained and operated so as to minimize fire hazards and to ensure the following: all bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquids. Excessive standing oil in bilge pockets below both port and starboard main engines and in ~5 gallon bucket in engine room. 46 CFR 142.220(a)

All fire detection systems must be inspected or tested at least once every 12 months as prescribed in paragraphs (a)(1) through (8) of this section [142.240 (a)]. All smoke and fire detection systems, including detectors and alarms, must be tested [142.240 (a)(8)]. No record available of annual inspection of fire detection system in past 12 months. 46 CFR 142.240(a)

Each towing vessel must be equipped with a means to detect smoke in the berthing spaces that alerts individuals in those spaces. Smoke detector in starboard berthing space near engine room is missing batteries. 46 CFR 142.330(b)

Each towing vessel must carry at least one appropriate-sized immersion suit for each person onboard. Vessel did not have required number of immersion suits on board. 46 CFR 141.350(a)

Each towing vessel must carry at least one appropriate-sized lifejacket for each person onboard. Vessel did not have required number of lifejackets on board. 46 CFR 141.340(a)

Machinery must be maintained to provide for safe operation of the towing vessel. Several flex hoses on PORT engine outboard side chafed through and fuel leak identified on PORT engine near fuel filters. 46 CFR 143.205(a)

A crew work space must be of adequate construction to provide for the protection and accommodation of the crew in a manner practicable for the size, facilities,service, route, and modes of operation of the vessel. The deck is corroded and wasted through inside the fidley space in way of the port side watertight door. 46 CFR 144.700(a)

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels. Evidence of crewmembers being enrolled in a random drug testing program that meets the requirements of 46 CFR 16.230 could not be provided because no crew is employed while vessel is not in service. Marine employer must provide evidence of crewmembers being enrolled in a random drug testing program that meets the requirements of 46 CFR 16.230 prior to operation of the vessel. 46 CFR 16.230

The master of a towing vessel must ensure that drills are conducted and instructions are given to ensure that all crewmembers are capable of performing the duties expected of them during emergencies. Prior to operations vessel crew is to demonstrate abandoning the vessel, recovering persons from the water, and responding to onboard fires to the satisfaction of attending CG Marine Inspector. 46 CFR 140.420(a)

No marine employer shall engage or employ any individual to serve as a crewmember unless the individual passes a chemical test for dangerous drugs for that employer. Evidence of preemployment drug testing could not be provided because no crew is employed while vessel is not in service. Marine employer must provide evidence of preemployment drug testing prior to operation of the vessel. 46 CFR 16.210

The owner, managing operator, master, or officer in charge of a navigational watch of a towing vessel must ensure the strength of each component used for securing the towing vessel to the tow and for making up the tow is adequate for its intended service. Repair prior to towing operations. 46 CFR 140.801(a)

A ship of 100 gross tons or more constructed before July 1, 1974 must have a fixed container or enclosed deck area under or around each fuel oil tank vent, over-flow, and fill pipe that has a capacity of one-half barrel (21 gallons); or equip each fuel oil or bulk lubricating oil tank vent, overflow, and fill pipe during oil transfer operations with a portable container of at least 5 U.S. gallon capacity. The fixed containment under the port and starboard fill pipes have been filled with cement and reduced the containment volume to below 16 gallons. Wastage on starboard fill pipe containment patched with tape. Permanently repair prior to fuel/lube oil transfer operations. 33 CFR 155.320(b)

A vessel of less than 50 meters in length may exhibit an all-around white light where it can be seen. Anchor light damaged and inoperable. Vessel is limited to operating in daylight hours only with no restricted visibility. 46 CFR 140.720 33 CFR 83.30(b)

Emergency lighting must be provided for all internal crew working and living areas. Emergency lighting in engine room and galley not operational. 46 CFR 143.410(b)

A towing vessel that is certificated for lakes, bays, and sounds must be equipped with either an approved 160-B semi-portable fire-extinguisher system or a fixed fire-extinguishing system installed to protect the engine room. Vessel is not equipped with an installed fixed fire-extinguishing system and the 160-B semi-portable extinguisher was not onboard or part of the June 2021 annual servicing performed by Harris Fire Protection. 46 CFR 142.315(a)

All portable fire extinguishers and semi-portable fire extinguishers systems must be inspected or tested once every 12 months as required by NFPA 10. 10-B:C (15lb CO2) portable fire extinguisher located in upper crew cabin was not included in the June 2021 annual servicing performed by Harris Fire Protection. 46 CFR 142.240(a)(1)

Propulsion machinery including main engines must be maintained to ensure proper operation and be suitable for route and service. 2 coolant leaks, one each on port and starboard side, observed on port main engine. 46 CFR 143.205(c)

Electrical components that pose an electrical hazard must be in an enclosure. Within utility space below wheel house, dead end conduit wire not enclosed within an enclosure in (forward/stbd) and junction boxes and light switches missing cover plates. 46 CFR 143.400(g)

Means must be provided for closing, in case of fire, each doorway, ventilator, and annular space around funnels and other openings into such spaces. Port and starboard ventilation systems to the machinery space, inboard of the ventilation fans, are wasted and holed beyond the coverage area of the ventilation cover bags. 46 CFR 144.605

Each operator of a U.S. towing vessel of 26 feet or more in length must hold a restricted operator’s license or higher. Vessel operator did not possess marine radio operator permit. 46 CFR 140.715(a) 33 CFR 164.72(b)(3)

The inspection will ensure that the vessel is in satisfactory condition and fit for the service in which it is intended, to include inspections of the structure. Exterior second level deck is holed in several locations, to include starboard side near amidships, and port side aft. Bulkwarks in aft quarter of vessel, and associated support members, found to have significant wastage. To be permanently repaired to the satisfaction of the attending Marine Inspector at next dry-dock. 46 CFR 136.212(b)

A towing vessel choosing the Coast Guard inspection option must undergo an annual inspection within 3 months before or after the COI anniversary date. Vessel did not complete first annual inspection within 3 months of COI anniversary date. 46 CFR 137.200

Fire pump cannot be activated at the pump and can only be activated outside the engine room. A fixed fire pump must be capable of being energized from a safe place outside the engine room and at the pump.

Standby generator "starboard" has an active fuel leak. Identify and correct fuel leak.

During inspection, fuel oil shutoffs were not in working order and it was not clear if both are broken or if only one is broken. Prove proper operation of fuel shutoff valves.

Shaft seal is leaking excessively on both shafts. Starboard rose box for catching water has soft piping leading out a windo because existing piping is corroded. Conduct repairs to shaft seal to bring it back into good working condition.

Vessel has fuel leaks in piping near the port side emergency shutoff and from the fuel tanks around the site glasses. Correct all fuel leaks in the engine room.

Vessel is not equipped with a survival craft. Install a survival craft to meet 46 CFR 141.305.

No documentation for the servicing and testing of the fire control panel located in the pilot house was provided to the Marine Inspector. Provide documentation to prove the control panel is approved and maintained with the manufacturer's manual.

Drills were not conducted during the inspection due to condition on vessel. Conduct man over board drill and fire drill to the satisfaction of attending USCG Marine Inspector.

Alarms and guages on main engines and generator engines required by 46 CFR 143.230 were not installed or not able to be tested. Prove proper operating of alarms, gauges, and testing procedure.

Identified hanging electrical wires that were improperly spliced together in the engine room. Remove spliced wires.

Exposed wire conductors (dead end wiring) was identified in various locations around the engine room. Remove exposed wires.

In the engine room an extension cord was found hard wired into an electrical receptacle. Remove electrical hazard.

A significant amount of fuel is located in on of the vessels fuel tanks that is not being used nor is it even pipped into the vessels fuel system. Remove unused fuel to prevent fire and pollution hazard.

Remove excessive oil accumulation in engine room bilge.

High bilge alarms do not have a visual indicator on the bridge. Install visiual indicator for the high bilge alarm on the bridge that shows when the alarm is activated and which alarm it is.

Provide accurate manufacturer data for the pressure vessels and have that information visible on the pressure vessel. Old data plate on pressure vessel says working pressure is 200 PSI, P-touched on the pressure vessel says 135 PSI.

Install relief valve on pressure vessel to be in compliance with 46 CFR 143.300. Relief device is labeled as having a range or 150-200 but the set point is unknown.

The generators and main engines have exposed piping that experiences high temperatures with no insulation. Provide insulation on hot piping in the engine room.

Flammable products are stowed in the engine room near the pressure vessel. Flammable and explosive products shall be stowed in accordance with 46 CFR 143.220(c).

Vessel is not equipped with an EPIRB. Install an EPIRB in accordance with 46 CFR 141.380.

Echo depth sounder is not functioning properly and does not show the current depth. Prove proper operating of echo depth sounder.

Vessel is not equipped to display the navigation light arrangement to meet Rule 27 of 33 CFR 83.27. Install navigation lights required to meet 33 CFR 83.27.

Vessel has five ventilation openings into the engine room, each one does not have a means to secure it for fire fighting purposes. All crew members must be familiar with their fire-fighting duties, and specifically how to effectively seal all natural openings to the space to prevent leakage of the extinguishing agent.

No documentation was on board to prove crew is in compliance with 46 CFR 16. Provide evidence that crew members are meeting chemical testing requirements outlined in 46 CFR 16.
