Each towing vessel must be equipped with a means to detect smoke in the berthing spaces and lounges that alerts individuals in those spaces. This may be accomplished by an installed detection system, or by using individual battery-operated detectors meeting UL 217 (incorporated by reference, see § 136.112 of this sub-chapter). Detection systems or individual detectors must be kept operational at all times when the crew is on-board the towing vessel. Smoke detector was removed in passageway between berthing spaces. Replace missing smoke detector to satisfaction of Coast Guard MI. 46 CFR 142.330(b)

Lifejackets must have the following attachments and fittings: Each lifejacket must have a whistle firmly secured by a cord to the lifejacket. Whistles were missing from all life jackets onboard. Replace whistles to satisfaction of Coast Guard MI. 46 CFR 141.340(g)(2)

Rails or equivalent protection must be installed near the periphery of all decks accessible to crew. Equivalent protection may include lifelines, wire rope, chains, and bulwarks that provide strength and support equivalent to fixed rails. Coast Guard MIs found Starboard bow 2nd deck handrails to be wasted. Crop and renew handrails to the satisfaction of a Coast Guard MI. 46 CFR 144.800(a)

A towing vessel subject to this subchapter and choosing the Coast Guard inspection option, or required to have the Coast Guard inspection option, must undergo an annual inspection within 3 months before or after the COI anniversary date. Vessel is operating with expired COI, vessel to stop work until a valid COI can be obtained. 46 CFR 137.200

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Vessel reported a loss of power to Starboard Generator, due to a inoperable battery charge. Vessel tried to start port Generator but could not get it to prime. moved port battery to starboard Generator and got Starboard Generator to start. installed a temporary charger to maintain battery. Vessel is authorized a one time Transite with an assist tug to old river. Vessel shall enact permanent repairs to the satisfaction of the Coast Guard. Cite 46 CFR 143.205(a) Action 701c

Each lifebuoy with a floating electric water light must have a lanyard of at least 910 millimeters (3 feet) in length, but not more than 1,830 millimeters (6 feet), securing the water light around the body of the lifebuoy. The floating electric water light was clipped to the lifebuoy without a lanyard. The vessel is limited to daytime operation until lanyard is provided. 46 CFR 141.360(c)(4)

Each lifejacket must have a lifejacket light approved under approval series 161.012 or 161.112 securely attached to the front shoulder area of the lifejacket. The Type I PFDs throughout the vessel did not have a light as required. The vessel is limited to daytime operation until lights are provided. 46 CFR 141.340(g)(1)

A 40-B portable fire extinguisher must be located near the storage room or cabinet. Vessel did not have an extinguisher for the flammable cabinet. Provide new extinguisher within 14 days. 46 CFR 142.225(d)

The following systems must be equipped with gauges at the machinery location. (1) Main engine lubricating oil pressure and main engine RPM. (3)Auxiliary generator engine lubricating oil pressure and quxiliary generator engine RPM. STBD main engine and both generators were not equipped with engine RPM gauge. Vessel shall install RPM gauges for above mentioned machinery at their location to the satisfaction of a Coast Guard MI.

Vessel failed to obtain an annual inspection within 3 months of the anniversary date. The vessel is not allowed to operate until a successful COI inspection is completed and a new COI is issued.

The following systems must be equipped with gauges at the machinery location: (1) Main engine RPM; (3) Auxiliary generator engine RPM. No RPM gauges were installed on any engine.

towage positions for lifejackets stowed in a berthing space or stateroom and all lifejacket containers must be marked in block capital letters and numbers with the minimum quantity, identity, and, if sizes other than adult or universal sizes are used on the vessel, the size of the lifejackets stowed inside the container. The equipment may be identified in words or with the appropriate symbol from IMO Resolution A.760(18) All lifejacket stowage locations aboard the vessel were unmarked.
