Electrical connections must be suitably installed to prevent them from coming loose through vibration or accidental contact. Electrical wire nut connections not within a junction box are not secured to prevent loosening.

Individuals, who are not crewmembers, on board a towing vessel must receive a safety orientation prior to getting underway or as soon as practicable thereafter, to include:(a) The location, operation, and use of lifesaving equipment;(b) Emergency procedures;(c) Methods to notify crewmembers in the event of an emergency; and(d) Prevention of falls overboard. Non crew orientation has not been established or recorded.

Each towing vessel must perform the applicable tests in Table 143.245(b) of this section. The tests required by this section must be recorded in accordance with part 140 of this subchapter. General alarm not tested and recorded weekly.

A towing vessel of at least 26 feet, but less than 79 feet, in length must carry a minimum of two lifebuoys located on opposite sides of the vessel where personnel are normally present. Lifebuoys must be at least 610 millimeters (24 inches) in diameter. Lifebuoys are located on aft second deck not located where personnel are normally present.

No person may intentionally drain oil or hazardous material into the bilge of a towing vessel from any source. Crew was observed stripping fuel tanks into the engine room bilge. Remove fuel from bilges and conduct training with crews to ensure oils and hazardous materials are not discharged into the bilges.

Paints, coatings, or other flammable or combustible products onboard a towing vessel must be stored in a designated storage room or cabinet when not in use. Flammable liquids found in storage room with ignition sources.

The following systems must be equipped with gauges at the machinery location: (2) Main engine cooling water temperature; (3) Auxiliary generator engine lubricating oil pressure and auxiliary generator engine RPM; Port main engine water temperature gauge not working and port auxiliary generator tachometer not properly working.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Flame shield on starboard Racor fuel filter missing. Install to original condition.

Each escape hatch and emergency exit used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR”. Sign for escape hatch missing.

Each lifebuoy on a towing vessel must be marked in block capital letters with the name of the vessel. Vessel's name is worn off of life buoy on port side of pilot house.

The connections of flexible cable plugs and socket outlets must be designed to prevent unintended separation. Permanently installed extension cord installed on pilot house and going through deck on starboard side.

Each towing vessel must be equipped with a means to detect smoke in the berthing spaces and lounges that alerts individuals in those spaces. Birthing space was not equipped with smoke detector. CORRECTED ON THE SPOT

Each lifejacket must have a lifejacket light approved under approval series 161.012 or 161.112 securely attached to the front shoulder area of the lifejacket. Life jacket light not working. CORRECTED ON THE SPOT

A towing vessel may not be operated without having onboard a valid COI issued by the Coast Guard as required by 136.202. 46 CFR 176.105(a)

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Starboard side propeller is damaged. Repair or replace in kind. Port side shaft indicates wastage. Repair or replace in kind. Visible cracking noted via pictures from operator on starboard shaft strut housing. Provide report of non-destructive testing (i.e. dye penetrant, ultrasound, etc.) to confirm extent of potential internal damage.Provide technician's service report once all repairs are completed. All repairs to be conducted with regards to acceptable marine practices. Vessel may transit to local dry-dock facility only for repairs approximately several hundred yards from vessel's current homeport.

Life float has multiple areas of deterioration but is still serviceable. Repair as original to en sure equipment is properly maintained.

Life float does not have required floating electric waterlight. Provide waterlight in accordance with 46 CFR 161.010 approval series.

Fixed fire pump must be capable of delivering water...at 50psi and a flow rate of at least 80gpm. Pressure guage installed at pump indicates 30psi. Replace guage.

A skiff must be of suitable size for all persons on board the towing vessel. Vessel's current COI requires 04 persons to be provided with survival craft accommodations. Current skiff is only suitable to carry 02 persons. Vessel is restricted to carry 02 persons only.

Port side main engine piping for water cooling is missing lagging or protection from heat by touch. Provide means to protect crew from water cooling line.

Accessibility to emergency fuel shutoff is restricted by the access latches to the containment. Provide quick access to emergency shutoff valve.

Visibility restricted to the general alarm in the engine room. General alarm also held by one bolt. Unrestrict the visibility to general alarm and make permenant.

Provide proof of required general alarm sign in engine room

VESSEL DID NOT HAVE A STANDARD DISCHARGE CONNECTION ON BOARD AS REQUIRED BY33CFR155.410(A)(3).

VESSEL DID NOT HAVE AN APPROVED MARINE SANITATION DEVICE ON BOARD AS REQUIRED BY 33CFR159.7(B)(2). VESSEL'S INSTALLED TOILETS HAD PIPING THAT DISCHARGED RAW SEWAGE DIRECTLY OVERBOARD.

VESSEL DID NOT HAVE A FIXED PIPING SYSTEM ON BOARD FOR DISCHARGE OF OILY MIXTURES TO A RECEPTION FACILITY AS REQUIRED BY 33CFR155.410.

PROVIDE EVIDENCE THAT ALL PORTABLE FIRE EXTINGUISHERS HAVE BEEN PROPERLY SERVICED.
