In addition to the radiotelephone required by paragraph (b) of this section, each vessel described in paragraph (a) of this section while transiting any waters within a Vessel Traffic Service Area, must have on board a radiotelephone capable of transmitting and receiving on the VTS designated frequency in Table 161.12(c) (VTS and VMRS Centers, Call Signs/MMSI, Designated Frequencies, and Monitoring Areas). VTS reported that vessel cannot transmit but can receive radio transmissions. Submit technicians report identifying root cause and attesting to sat operations of the inoperable communication equipment.

Each towing vessel equipped with rudder(s) must have a means to monitor and control the position of the rudder. Damaged steering gear components preventing control of rudder.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Aft steering station rudder lever not working properly. 46CFR143.205(a)

The owner, managing operator, master or officer in charge of a navigational watch of a towing vessel must ensure the following:(a) The strength of each component used for securing the towing vessel to the tow and for making up the tow is adequate for its intended service. Tow line not spooled properly. 46CFR140.801(a)

Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention. The following alarms must be provided:(3) Auxiliary generator engine low lubricating oil pressure; (4) Auxiliary generator engine high cooling water temperature; Alarms for generator low lubricating oil pressure and engine high cooling water not installed on at the operating station. 46CFR46CFR143.230(a)(3)&(4)

Storage of flammable or combustible products. (d) A 40-B portable fire extinguisher must be located near the storage room or cabinet. No fire extinguisher in vicinity of locker. 46CFR142.225(d)

Each towing vessel must be equipped with an industrial type first aid cabinet or kit, appropriate to the size of the crew and operating conditions. Replace all expired medicine in first aid kit. 46CFR140.435

For vessels other than those covered in paragraphs (b) and (c) of this section, the name of the vessel must be marked on some clearly visible exterior part of the port and starboard bow and the stern of the vessel. Vessel's name not marked on the stern. 46CFR67.123

The crew of each towing vessel must demonstrate the ability to operate the primary and auxiliary machinery systems for which they are responsible, and to do so under normal and emergency conditions. This includes, but is not limited to, restoration of steering in the event of failure. The vessel's steering system failed and cannot be operated correctly. Your vessel is prohibited from departing any port within the Sector Honolulu COTP Zone until satisfactory repair of the vessel is completed, and the proper operation of the propulsion system is demonstrated to the satisfaction of the attending Marine Inspector. Your vessel is prohibited from transiting any port within the Sector Honolulu COTP Zone without the assistance from a towing vessel of adequate horsepower and maneuverability. 46CFR143.200(b)

All machinery and equipment that is not in proper working order (including missing or malfunctioning guards or safety devices) must be removed; made safe through marking, tagging, or covering; or otherwise made unusable. The towing winch hydraulic motor piping is wasted and has rubber with hose clamps to seal the leaking hydraulic piping. Complete the repairs to the satisfaction of the attending Marine Inspector. 46 CFR 140.505(c)

Towing vessels must meet the communications requirements of 33 CFR part 26. Vessel does not have a Bridge-to-Bridge Radiotelephone Certificate. Provide valid Bridge-to-Bridge Radiotelephone Certificate the satisfaction of the attending Marine Inspector. 46 CFR 140.715(a)

Need one additional Immersion suit. Vessel had nine of the ten required per COI.

Provide approved and serviceable survival craft as required for vessel route. Survival craft currently not on board. 46 CFR 141.305

Provide two approved firefighter outfits and two self contained breathing apparatus with 30 minute air supply that are readily accessible and ready for use. Ensure all crew is properly trained in use and proper donning of equipment and incorporate into training program, drills and documentation of training and drills. 46 CFR 142.226 46 CFR 140.420 46 CFR 140.650

Demonstrate proper operation of steering system following repairs to steering hydraulic assembly. 46 CFR 143.205

Provide documentation showing the Fire-detection systems is certified and inspected by a registered PE with experience in fire-detection system design, by a technician with qualifications as a NICET Level IV tech or or by an authorized classification society.

Provide operable smoke detectors for all berthing spaces meeting the requirments of 46 CFR 142.330 (b).

Renew expired emergency eye wash solution.

Provide amended International Tonnage Certificate showing dual tonnage notation.

Provide two firefighter outfits and self contained breathing apparatus meeting the standards listed in and required by 46 CFR 142.226.

ON AUGUST 18, 1994 THE VESSEL HENRY SR. OPERATED BY SAUSE BROS OCEAN TOWINGCO. WAS OPERATING IN HONOLULU HARBOR EQUIPPED WITH INSTALLED TOILET FACILITIES WHICH WERE CONNECTED TO A NOT OPERATIONAL TYPE III MARINE SANITATION DEVICE. THE MSD WAS BYPASSED IN THE OVERBOARD DISCHARGE POSITION
