The installation of all systems of an electrical engineering or interior communication nature, together with the details of design, construction, and installation, shall be in accordance with the requirements of subchapter J (Electrical Engineering) of this chapter. Found a dead end wire in the forward compartment above the rack in the top of the port aft section and in the top of the starboard which is in a connection box.

For vessels assigned a load line, see subchapter E (load lines) of this chapter, for special requirements as to strength, closure of openings, etc. The port 1 and 5 gooseneck vents are missing their physical closures.

The installation of all systems of a marine engineering nature, together with the details of design, construction, and installation, shall be in accordance with the requirements of subchapter F (Marine Engineering) of this chapter. High jackwater temperature shutdown testing must be completed on all installed engines.

No Repairs or alterations affecting the safety the vessel with regard to the hull, machinery, or equipment shall be made with out the knowledge of the Officer in Charge, Marine Inspection. The aft 3" bilge suction pipe in the port no.5 void is corroded and holed. Renew the pipe to the satisfaction of the attending USCG Marine Inspector.

06199-A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its life-saving appliances, fire-fighting or other equipment. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in Subchapter I. Nothing in this subpart shall be construed as limiting the inspector from making such tests or inspections as he deems necessary to be assured of the safety and seaworthiness of the vessel. A discharge of oil allegedly originated from the machinery equipment on board the barge HALEAKALA, discharging into the navigable waterway and upon equipment and/or cargo on board the vessel. The vessel and its cargo pose a harmful risk to the environment while underway due to the oil contaminated vessel hull and cargo. Provide proof to the marine inspector that the vessel, its equipment and its cargo no longer pose a risk to the marine environment prior to sailing.

Crew was unable to open the starboard engine room emergency escape hatch. Escape hatch shall be operational for personnel to escape engine room.

Port engineer could not satisfactorily test the port generator high temperature alarm and crane generator high temperature alarm. Prove high temperature sensor alarms are operational for each.

Thruster engine flexible non metallic expansion joints on the fresh water piping found materially wasted. Replace in kind.

Provide records for non metallic expansion joints placed into service on salt water piping for thruster cooling, or replace expansion joints if installed more than 10 years.

Found a severed O-ring at the lube oil delivery pipe flange connection located after the pump on the upper deck crane engine.

Provide datasheet for industrial 3-way valve, and provide updated plans, copies of USCG Type approval certs., & copies of the DIOM manuals for both installed fire fighting systems.

Properly install six 250 degree quartzoid bulbs (spec #42276) on Ansul CO2 system, which protects the machinery space for the industrial crane.

provide a different means to secure the auto actuation of the Halon system for the lower machinery space, as the arrangement they have now creates a hazard for personnel working on the vessel as noted in

Prove Proper operation of oily water separator for completion of IOPP Cert.

Complete fixed fire extinguishing system function test in the presence of a CG Marine Inspector.

Provide date at which the pneumatic detection/actuation of the fixed gas fire extinguishing systems can be tested/maintained IAW their USCG approved design, installation, operation, & maintenance manuals.

The testing for prime mover of generator in the crane house has not been tested. Company allowed to prepare for tests and contact USCG to witness.

The last hydrotest was done in 1997, NFPA and CGA allows for 5 year external visual inspection. Sought to have company provide results of last inspection IAW CGA 6.

USCG issued wrong IOPP or missed verifying that the Oil Content meter was a 60 vs what was on the IOPP as 393. Having company provide 1 year of receipts of proper disposal of oil waste.

The detectors in the crane space appeared to be damaged from open flame, vsl rep stated earlier fire servicing company had done tests and left scorch marks. Issued 835 to have current fire servicing company to provide statement of evaluation of detectors.

A 12"-16" Inset was found on the Port FWD bow rake, resulting from collision with tug while taking into alongside tow. Inset was observed to approx 4"-6" in depth. Inset did not appear to penetrate hull plating. A 1/2" fracture was found on internal side of inset. Perm repair to be made at next available port to satisfaction of ABS class surveyor.

During periodic exam testing of reverse power relay on #1 generator, reverse power relay failed to operate.

Submit permanent repair plan for temporary drill stop/weld repair located portside on #6 blkhd in #1 rake to OCMI. Repair plan to be approved by OCMI prior to start of repairs. Include NDT method for weld repair.

Submit plans and make repairs to deck inset and overhead fractures on forward aft crane support.

Repair/renew lower deck railing on port quarter of vessel.

Renew port fuel tank flame screen as original.

Renew fuel shutoff valve labels and posted transfer procedures. Also, renew fixed firefighting system labels on deck.

Properly dead end wiring beneath tank level indicators in generator space.

Repair/renew cover on port generator exhaust and provide lagging on exposed exhaust piping for both generators.

Make all emergency lighting in the generator space operational.

Renew cracked flex coupling on stern thruster.

Provide complete copy of SOPEP on board vessel.

Repair ground indicator light in forward storage area.

Install a bolted cover on flanged connection of the standard discharge connetion.

Forepeak; Bay 6 aft bulkhead outboard vertical stiffener (port side) "V" and reweld crack to the satisfaction of the attending marine inspector.

Provide permanent fuel oil containment for the crane diesel generator fuel tank fill hose and tank vent in accordance with 33 CFR 155.320 (a).

Replace inoperable emergency light in main machinary space by storage locker.

Provide a pressure gauge with markings in increments of "psi" for the cranediesel generator air receiver and prove proper operation of the pressure relief valve.

Provide permanent splice to wiring in way of battery charger, port side main machinary space.

Prove proper operation of the #1 S/S diesel generator start air receiver pressure relief valve.

Effect permanent repairs to the exhaust leak located inside the main machinery space in the overhead approx 3 feet inboard of the horizontal turn of the exhaust piping of the #2 S/S diesel generator.

Effect repairs to starboard running light and prove operation prior to night time operations.

Provide a Standard Discharge Connection as required by MARPOL 73/78 Regulation 19, and by 33 CFR 155.430.

Provide an Oil Record Book as required by MARPOL 73/78 Regulation 20, and by 33 CFR 151.25.

Repair or lubricate the ventilation louvers in the deck of the crane machinery room to allow full closure.

Renew the International Oil Pollution Prevention Certificate (IOPP)(expired09NOV96), or limit vessel's route to "not on an international voyage" on the Certificate of Inspection (COI).

Hydro test all CO2 bottles and their flex loops for the fixed extinguishingsystem serving the crane generator space by next drydock but not later than06JAN97.

REINSTALL MISSING 2'x 2' SECTION OF A-60 INSULATION AND SHEATHING AS ORIGINAL ON THE PORT FORWARD BULKHEAD IN THE HALON ROOM.

PRIOR TO CARRYING FREIGHT FOR HIRE PROVIDE EVIDENCE THAT THE VESSEL'S ONBOARD SOPEP HAS BEEN APPROVED BY THE U.S. COAST GUARD.

REPAINT AND PERMANENTLY MARK LOADLINE MARKS.

PRIOR TO CARRYING FREIGHT FOR HIRE PROVIDE COPY OF SERVICING REPORT INDICATING FIXED AND SEMI PORTABLE SYSTEMS ARE IN SATISFACTORY OPERATING CONDITION.

LABEL THE EMERGENCY ESCAPE SCUTTLE IN THE MACHINERY SPACE ON THE PORT SIDE.

PERFORM HYDROTESTS ON SHIPS SERVICE AIR RECEIVERS 1 AND 2.

DEMONSTRATE PROPER OILY WATER SEPARATOR AND MONITOR OPERATION.

ENGINE ROOM SEMIPORTABLE CARBON DIOXIDE FIRE EXTINGUISHER BOTTLES ARE TO BE HYDROSTATICALLY TESTED AND REFILLED BY AN AUTHORIZED FACILITY.

PROVIDE BLANK IN OPEN CIRCUIT BREAKER SPACE IN THE DISTRIBUTION PANEL TOP OF MACHINERY SPACE LADDERWAY (PANEL P-12).

DEMONSTRATE PROPER OPERATION OF THE EXISTING RADIO CONTROL SYSTEM FOR THE STERN THRUSTER. THE STERN THRUSTER IS NOT TO BE OPERATED BY RADIO CONTROL UNTIL THIS REQUIREMENT IS SATISFIED. PRIOR TO INSTALLATION OF ANY NEW STERNTHRUSTER RADIO CONTROL SYSTEM, SUBMIT NECESSARY PLANS TO MSC FOR APPROVAL.

DEMONSTRATE PROPER OPERATION OF THE RADIO CONTROL SYSTEM FOR THE STERN THRUSTER. THE STERN THRUSTER IS NOT TO BE OPERATED UNTIL THIS REQUIREMENT IS SATISFIED.

PROVIDE A GUARD FOR THE STERN THRUSTER DRIVE SHAFT AND FLYWHEEL.

REMOVE ALL DEAD END WIRES ON BOTH GENERATORS LOCATED IN THE MACHINERY ROOM.

RENEW NUMBER 4 STBD AFTER BALLAST TANK VENT.

RENEW NUMBER 6 STBD FORWARD BALLAST TANK VENT.

PROVIDE A SUITABLE GUARD FOR THE MAIN BILGE/BALLAST PUMP DRIVE SHAFT.

PRIOR TO INSTALLATION OF ANY NEW STERN THRUSTER RADIO CONTROL SYSTEM, SUBMIT NECESSARY PLANS TO THE COAST GUARD MARINE SAFETY CENTER FOR APPROVAL.

PROVIDE SERVICE REPORTS FOR THE PORTABLE FIRE EXTINGUISHERS.

PROVIDE THREADED PLUGS FOR ALL FUEL AND CARGO CONTAINMENT PRIOR TO TRANSFEROF FUEL OR CARGO.

MAKE PERMANENT REPAIRS TO PORT BALLAST TANK, FRAME 45 AT NO. 3 SIDE LONG- ITUDINAL, IN LIEU OF TEMPORARY REPAIRS.

INSTALL METAL LIGHT GUARDS ON CRANE LIGHTS.

INSTALL RUBBER MAT AT FORECASTLE ELECTRICAL CONTROL PANEL.

RENEW VENT AND FLAME SCREENS AT MAIN DECK.

INSTALL PLUGS AT JUNCTION BOXES AT BOARDING LIGHTS PORT AND STARBOARD FORWARD.

RENEW STUFFING TUBE CONNECTION AT BARGE CRANE TIE.

REINSTALL PORT LIGHT ASSEMBLIES AT FORECASTLE WHERE MISSING.

EFFECT PERMANENT REPAIRS TO MAIN DECK STARBOARD SIDE BETWEEN FRAMES 54-55 #4 STARBOARD BALLAST TANK.

OPEN AND MAKE READY FOR INTERNAL INSPECTION DURING NEXT DRYDOCK THE CARGO HOLD. PARTICULAR ATTENTION TO BE PAID TO DENTED SECTION OF MAIN DECK APPROXIMATELY 7'X8' LOCATED 15' FORWAQRD OF VOID SPACE, AFTER PORT HATCH AND 14' FROM SIDESHELL PORT.

PROVE PROPER OPERATION OF VENTILATION AND MACHINERY SHUTDOWNS OF CRANE ENGINEROOM WHEN CO2 FIXED SYSTEM IS ACTIVATED. CO2 PORTION OF SHUTDOWN SYSTEM PROVEN, ELECTRICAL PORTION INOPERATIVE.

PROVIDE ANNUAL PROOF OF SERVICING FOR ALL FIRE EXTINGUISHERS.

REPAIR TO OPERABLE CONDITION THE RING BUOY WATERLIGHT ON THE STARBOARD STERN RING BUOY.
