Propulsion machinery, including main engines must be maintained to ensure proper operation. Vessels Port & STBD engine experienced a failure causing Loss of Propulsion. Vessel must ensure sufficient repairs to the TPO and submit TPO report to the OCMI prior to conduction operations. 46 CFR 143.205(c)(1)

Machinery must be designed and maintained to provide safe operation of the towing vessel under normal and emergency conditions. Vessel reported a failure to the port main engine resulting in a loss of propulsion. Make permanent repairs to the satisfaction of vessel's TPO (TVIB) prior to further towing operations. In accordance with CVC Policy Letter 17-10, the above deficiencies may be cleared by the operating company's TPO. TPO must submit a report to the cognizant OCMI within 72 hours of clearing the deficiencies.

A vessel must be operated in accordance with applicable laws and regulations and such in a manner as tto afford protection against hazards to life, property, and environment. Excessive water was found in the engine room bilge at a rate of 300 gallons per hour. Vessel is to continue de-watering with the fixed bilge system and have a minimum 02 pumps on standby. Conduct rounds at a minimum every 30 minutes. Captain is to update the duty IO every hour or if anything changes from the initial report. Vessel is to proceed with 03 empty red flags to Wood River, Il to fleet the tow and continue to National Maitenace gas free for cleaning and permanent repairs. ***This deficiency must be cleared to the satisfaction of a Coast Guard marine inspector prior to returning vessel to service. In lieu of Coast Guard attendance, the company may use their Third Party Organization, TVIB, to accept/approve the repairs and provide a report to the Coast Guard inspector listed below.****

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Installed electrical power source(s) must be capable of carrying the electrical load of the towing vessel under normal operating conditions. AFT Generator shut down and the secondary gen was not coming online correctly. 46cfr143.400(a)(b). All repairs need to be to the satisfaction of local OCMI or TPO.
