Propulsion machinery, including main engines, reduction gears, shafting, bearings, and electrical equipment and systems, must be maintained to ensure proper operation. The starboard shaft and propeller of the vessel have been fouled by debris (tire). The deficiency must be rectified prior to the vessel resuming normal operations. The Coast Guard will accept a class report from the ABS surveyor attesting to the successful rectification of the deficiency in order to clear this CG-835V.

Immediately after the addressing of resultant safety concerns, the owner, agent, master, operator, or person in charge shall notify the nearest Sector office, Marine Inspection Office or Coast Guard Group Office whenever a vessel is involved in a marine casualty consisting of a loss of main propulsion. The relevant parties failed to report the marine casualty of the fouling of the starboard propeller. Submit to the Coast Guard proper procedures for the reporting of a marine casualty.
