Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Vessel experienced a loss of propulsion on Starboard Main engine. The shaft and propeller have fallen off just aft of the cutlass bearing. Repair/replace starboard shaft and propeller to the satisfaction of the attending Marine Inspector.
Condition: Improper/Lack of Maintenance
Action required: 16 - Rectify deficiencies w/in 14 days
