All vessels shall be provided with not less than two avenues of escape from all general areas accessible to the passengers or where the crew may be quartered or normally employed, so located that if one is not available the other may be. Both emergency escape hatches for the port and starboard engine room were inoperable and could not be opened from the interior of the vessel. Vessel shall make escape hatches operable prior to departure.
