Fire pumps must be fitted on the discharge side with relief valves set to relieve at 25 PSI in excess of the pressure necessary to maintain the requirements of paragraph (c) of this section or 125 PSI, whichever is greater. Relief valves may be omitted if the pumps, operating under shut off conditions, are not capable of developing a pressure exceeding this amount. Upon activation of the fire pump relief valve severely leaking. Main fire pump can produce excess of 125 PSI, aux fire pump can not. Barge is to remove relief valve and tag out main fire pump, aux pump will serve as main fire, while barge will also be connected, via their international shore connection to the Tug JAMES K to serve as emergency fire pump until new relief valve can be installed. Barge must stay connected and in communication with tug at all times until def rectified.

No manned ship subject to this part may operator unless it carries on board a shipboard oil pollution emergency plan approved by the Coast Guard. The vessel SOPEP has exercise and training plans located in Section 7, however has no record of training or exercises conducted by the vessel's crew. Complete training and exercises as noted in the vessel's SOPEP or revise section 7 which is non-manditory provisions of the plan and submit changes as required in 33 CFR 151.28(f). 33 CFR 151.27(a)

The bridge-to-bridge radiotelephone station will be inspected on vessels subject to regular inspections pursuant to the requirements of Parts II and III of Title III of the Communications Act. Each vessel must carry a certificate with a valid endorsement while subject to the Bridge-to-Bridge Act. The vessel did not have onboard a valid Vessel Bridge-to-Bridge Radiotelephony Certificate. 47 CFR 80.1005

There shall be at least two means of escape from all general areas accessible to the passengers, if carried, or where the crew may be quartered or normally employed. MI observed that the emergency escape hatch in the MSD room had not been serviced and was difficult to open. 46 CFR 92.10-5

Each vessel must meet the applicable requirements in subchapter S of this chapter. MI observed several watertight doors with dogs that had either not been serviced or missing grease fittings. 46 CFR 93.01-1

The installation of all lifesaving appliances and arrangements must be in accordance with subchapter W (Lifesaving Appliances and Arrangements) of this chapter. Each vessel must have posters or signs displayed in the vicinity of each survival craft and the survival craft's launching controls. The vessel did not have emergency launching instructions posted in the vicinity of the rescue boat. 46 CFR 96.06-1 / 46 CFR 199.90

Lifesaving appliances, instructions to passengers, and stowage locations must be marked in accordance with Subchapter W (Lifesaving Appliances and Arrangements) of this chapter. Each lifeboat and rescue boat must be plainly marked; the number of persons for which the boat is equipped must be clearly marked, preferably on the bow, in permanent characters. The rescue boat was not appropriately marked with the number of persons on the bow of the vessel. 46 CFR 97.37-42 / 46 CFR 199.176

Each vessel must have a line-throwing appliance approved under approval series 160.040. The vessel did not have onboard a line-throwing appliance or auxiliary line. 46 CFR 96.06-1 / 46 CFR 199.170

The boundary bulkheads and decks separating the accommodations and control stations from cargo and machinery spaces, galleys, main pantries and storerooms, other than small service lockers, shall be of “A” Class construction. MI observed the starboard fire door in the engine room would not close properly. Witnessed vessel crew make repairs during the inspection. 46 CFR 92.07-10

Vessel's emergency remote fuel shutoff valve for the emergency generator was siezed and inoperable.

Vessels must be equipped with independently driven fire pumps in accordance with Table 95.10-5(a). Vessels main fire pump was inoperable.

Each machinery space ventilation system must have two controls to stop the ventilation, one of which may be the supply circuit breaker. Vessels ventilation port, stbd and center controls tripped upon operation of fans.

All other openings to a protected space must be capable of being closed. Doors, shutters, or dampers must be installed for openings in the lower portion of the space. Openings in the upper portion of the space must be capable of being closed off either by permanently installed means or by the use of canvas or other material normally carried on the vessel. Vessels ventilation closures did not close properly.

Vessels #3 main generator fuel pump was leaking excessive amount of fuel.

Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. Portable fire extinguisher have not been serviced annually as required by NFPA 10

Each inflatable lifesaving applicance and marine evacuation system must be serviced within 12 months of its initial packing and within 12 months of each subsequent servicing.... Vsl rep not able to provide annual servicing reports for all 6 life rafts.

Each vessel must carry a catagory 1 406 MHz satellite EPIRB meeting the requirements of 47 CFR part 80. The onboard EPIRB was inoperable.

Each vessel must have posted or signs displayed in the vicinity of eahc survival craft and the survival craft's lauching controls that (a) illustrate the purpose of the controls (b) illustrate the procedures for operating the launching device (c) give relevant instructions or warnings (d) can be easily seen under emergency lighting conditions and (e) display symbols in accordance with IMO resolution A.760(18). There were not instructions posted and no emergency lighting in the area.

Efficient guards and rails or bulwarks must be fitted on all exposed parts of the freeboard and superstructure dekcs. The rails around the freeboard deck were being replace and the installation was not completed.

No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. Vessel made 15 repairs on the vessel before Coast Guard completed initial dry-dock and internal structure exam without notifying local OCMI.

A survey shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel. The survey shall be such as to ensure that the necessary repairs have been effectively made, that the material and workmanship of such repairs are in all respects satisfactory. Barge experienced 1/8" x 12" crack in #2 port void approximately 2' above waterline. Barge experienced list and upon further investigation discovered approximately 6' of water in space. Void tank dimensions are 40' x 21' x 12'6". Surrounding voids were free of water. Dewatered space and conducted unauthorized temporary repair on crack by welding several beads to prevent ingress of water. Soundings are being conducted every 6 hours. Rounds shall continue to be conducted throughout voyage until barge is in port. Barge shall return to port immediately to conduct repairs to damage in #2 port void to satisfaction of local OCMI. Vessel rep shall notify local OCMI when in port and submit damage survey/repair proposal to attending Marine Inspector.

Scuttles shall be watertight. Storage room scuttle to main deck was leaking.

Falls are to be renewed every 3 years. No records available for rescue boat davit renewal.

Decks separating machinery and accommodation are to be “A” Class. Penetration in electrical space on quarters deck not properly stuffed.

At each COI the inspector will will determine if the tests of systems have been conducted. Portable and fixed firefighting were last conducted in Jun 2019. Have the fixed and portable extinguishers tested and serviced.

Life-rafts shall be arranged to float free. Painter line on 04 life-rafts were connected to vessel not via weak link.

Launching appliances must be thoroughly inspected every 5 years and subjected to a dynamic load test and release gear must be serviced IAW manufacturer intervals.

Radiotelephony Safety Certificate is expired; must be issued a new current certificate. 47 CFR part 80

Missing A-60 fire protection in CO2 room; must install A-60 insulation in this location.

Starboard vent damper would not properly close. Identify and repair fire damper to operate as intended. Until operational, damper shall be secured in the closed position.

Barge must carry not less than 12 rocket parachute flares approved under 46 CFR 160.136.

Vessel is authorized to operate with temporary repair installed in bottom plating of barge in MSD void space following casualty suffered on Monday, February 5, 2018. Vessel is to complete permanent repairs in dry dock following current dredging contract.

Provide written temporary repair proposal for 2 cracks in bottom plating in MSD void space. Submit dive report and class approval for all repairs.

46 CFR 92.07-10(c)boundary bulkheads separating control rooms from machinery spaces shall be of "A" class construction. The bulkhead separating the switchboard room from the engine room has several penetrations the do not meet the regulations.

The three rafts on each side were all secured to one hydrostatic release. If the largest raft were lost or rendered unservicable , all three would be lost or unservicable and then the vessel would have no life rafts on that side of the vessel.

Rescue boat engine not of approved type. Route has been limited to Coastwise. IAW 199.640(a)

Two of the fire fighting nozzles were not of approved type.

Potable water system is inoperable.

Man overboard drill not observed.

Could not verify Emergency Lighting aboard the vessel.

Bilge Pumps not tested, neither could be started.

Provide annual test of the installed relief valve.

Current install is a single 25 man life raft per side. As the vessel is currently authorized to carry 50 persons the life raft installation must be increased to 50 per side. The vessel is being reduced to 25 until this deficiency is corrected.

Make repairs and demonstrate the proper operation of the General Alarm system.

Install General Alarm bell or bells at the Forward End (opposite the cutter) and Aft End port side.

Install Electrical Breaker "Blank" in the 120/208 volt Forward Starboard interior panel.

Renew CO2 system pneumatic switch cover.

Adjust & Repair Forward Starboard 01 Deck Watertight Door. Adjusted 14 Nov 2012. Renew 01 Deck door, coaming is twisted and proper sealing can't be maintained.

Demonstrate proper operation of the installed Bilge Alarms.

Install Lagging on center generator exhaust piping.

Install CO2 clean-outs for the diffuser's in the Control Room.

Repair/Install Structural Fire Protection in way of Air Receiver, Aft Machinery Space.

Reverse Power relays for 3 generators could not be tested.

Provide documentation proving which air receiver is designated as #1 and which is designated as #2.

Since the air receivers have different MAWPs, and they could not be identified, neither shall be operated above 200 psi (the lower of the two MAWPs).

Relief valves were corroded and past their service life. Install new relief valves and test to the satisfaction of the attending MI.

During next scheduled dry-dock, replace the air receivers in the machinery space with new air receivers, in accordance with 46 CFR 50.30

Vessels master failed to properly test the air recievers within the due date on the vessels COI. 46 CFR 61.10-5

Upon completion of ABS damage survey, make temporary repair to 24" X 04" tear in side shell plate, port quarter, void tank above main deck/above oily water tank.
