Every oil tanker of 150 gross tons and above and every ship other than an oil tankerof 400 gross tonnage and abover shall carry on board a shipboard oil pollution emergency plan approved by the Administration. Vessel did not have an approved SOPEP from current Flag Administration. - MARPOL ANNEX 1 Reg. 37.1

A Certificate, called a Safety Management Certificate, shall be issued to every ship by the Administration or an organization recognized by the Administration. The Administration or organization recognized by it shall, before issuing the Safety Management Certificate, verify that the company and its shipboard management operate in accordance with the approved safety management system. Vessel did not have approved Safety Management Certificate issued by the Administration or organization. -SOLAS 74(20) Ch.IX Reg 4.3

Fire doors in main vertical zone bulkheads, galley boundaries and stairway enclosures other than power-operated watertight doors and those which are normally locked shall satisfy the following requirements: the doors shall be self-closing and be capable of closing with an angle of inclination of up to 3.5° opposing closure. Vessel "A" class division doors were unable to fully self-close to maintain division specified on Fire Control Plan. - SOLAS 74(20) Ch.II-2 Reg. 9.4.1.1.5.1

Ships shall be fitted with a system to automatically transmit the information specified in paragraph 5. Vessel’s LRIT does not have a conformance test required by MSC.1/Circ.1307. - SOLAS 74(20) Ch.V Reg. 19-1.4.1

Machinery spaces of category A shall be adequately ventilated to ensure that when machinery is operating at full power in all weather conditions, an adequate supply of air is maintained to the space. Ventilation to the main machinery space was found inoperable due to a bad breaker. - SOLAS 74(04) Ch.II-1 Reg.35

At least three two-way VHF radiotelephone apparatus shall be provided on every passenger ship and on every cargo ship of 500 gross tonnage and upwards. Such apparatus shall conform to performance standards not inferior to those adopted by the Organization as per resolution A.809(19). Vessel did not have spare batteries for VHF radios. - SOLAS 74(20) Ch.III Reg. 6.2.1.1

Fire extinguishers shall be situated ready for use, which can be reached quickly and easily at any time in the event of a fire, and in such a way that serviceability is not impaired. Portable extinguishes within the CO2 room were piled onto one another and not properly secured. - SOLAS 74(09) Ch.II-2 Reg 10.3.2.4

No repairs or alterations to the hull or machinery, or to equipment that affects the safety of the OSV, may be made without notice to the cognizant OCMI in the inspection zone where the repairs or alterations are to be made. Vessel suffered damage to their starboard side shell and starboard fuel day tank due to collision with DB THOR (IMO-8639455) during Hurricane ZETA. Vessel currently is aground in the mud at 29.09.7N 090-3.1W at Bayou Cochon. Master/Rep shall make reports to OCMI prior to movement to Bollinger Fourchon for repairs with plans to refloat the vessel, and transit plans to include timeline of movement, towing vessels, and configurations. Point of contacts shall also be provided for the towing vessels and any vessels used in refloating the HARVEY SEAS. Timely reports shall be made to the OCMI during the process and once the vessel has safely made it to Bollinger Fourchon. Vessel is only authorized to transit to Bollinger Fourchon for repairs and shall provide the local OCMI with a damage survey and repair proposal prior to effective repairs. The local OCMI will oversee the repair process and final examination of satisfactory repairs.

Noted several instances of white-out used to alter entries in Garbage Log Book. Likely attributable to simple mistakes but nevertheless advised Captain that lining out/initialing was preferrable editing method. Check at next annual inspection

No expiration date marked on HRUs for four (4) inflatable liferafts

Expired backup battery for portable VHF radio

Connection missing required 6 bolts and nuts, as well as oil-proof gasket

Pinhole leak in one fire hose

Company added centrifuge system to existing fuel manifold. Informed Vsl rep that plans must be sent to MSC for approval.

Ensure new directions for making entires are inserted into Oil Record Book. New MARPOL regulations came into effect January 1, 2011.

Make wiring penetrations in bridge deck compliant to deck fire protection rating. Materials must be CG approved for A-class penetration in 3 locations identified: under main console fwd and in locker amidships within 90 days. Verify approval of materials and installation with attending inspector prior to installation.

Fwd bulkhead CO2 room space is identified as A-15 on plans, but should be A-60. Verify A-60 construction by next credit drydock.

Periodic Safety Test Procedures call for use of simulation for testing of sensors. Provide test procedures to OCMI (ACTEUR) for approval that do not rely on simulation within 60 day.*** PSTPs are amended and reviewed. Must be verified at next scheduled CG attendance.***
