The frame or support of any semi-portable fire extinguisher fitted with wheels must be welded or otherwise permanently attached to a steel bulkhead or deck. Semi-portable fire extinguisher in engine room is not permanently mounted. 46 CFR 142.230(c)

A record of the type, size, and service of each towline, face wire, and spring line, used to make the towing vessel fast to her tow, must be available to the Coast Guard or third-party auditor for review. The following minimum information is required in the record: The dates when examinations were performed, the identification of each item of towing gear examined, and the name(s) of the person(s) conducting the examinations. Towing gear inspections are not being logged as required. 46 CFR 140.820(b)

The portable fire pump must be self-priming and power-driven, with: A minimum capacity of at least 300 LPM (80 gpm) at a discharge gauge pressure of not less than 414 kPa (60 psi), measured at the pump discharge. Gauge fitted to portable fire pump is broken. 46 CFR 142.325(e

An exposed hazard such as gears and rotating machinery, must be protected by a cover, guard or rail. Metal covers over port and starboard shafts are inadequately supported to prevent accidental contact.

Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention. The following alarms must be provided: Low fuel level, if fitted with a day tank. Day tank in engine room does not have a low fuel alarm.46 CFR 143.230(a)(7)

Electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Battery charger on port side of engine room found loose and resting on batteries served. 46 CFR 143.205(a)

Each exhaust pipe from an internal combustion engine which is within reach of personnel must be insulated or otherwise guarded to prevent burns. Lagging on lower portions of main engine exhaust is not in place. 46 CFR 144.830

Closure devices must be provided for deck house which open to the exterior of the vessel and which may allow water to enter the vessel. These devices must be suitable for the expected route. 4 doors (labeled as watertight) on main deck leading to exterior of vessel were fitted with incorrect gasket material and the port engine room door was severely deformed.

Pressure vessels must be equipped with an indicating pressure gauge. Installed gauge increments were of insufficient detail (due to broad range of gauge).

Piping for flammable or combustible liquid, seawater cooling, or firefighting systems must be fitted with isolation valves that are clearly marked by labeling or color coding that enables the crew to identify its function. Piping color coding did not match code key.

Each towing vessel must perform alarm set point testing twice every 5 years, with no more than 3 years elapsing since last test. P & S generators alarms not tested.

No person may operate a non-oceangoing ship of 100 gross tons and above that is fitted with main or auxiliary machinery spaces in the navigable waters of the United States unless (1) The ship has at least one pump installed to discharge oily mixtures through a fixed piping system to a reception facility; (2) The piping system required by this section has at least one outlet that is accessible from the weather deck; (3) Each outlet required by this section has a shore connection that is compatible with reception facilities in the ship's area of operation; and (4) The ship has a stop valve for each outlet required by this section. Vessel not equipped with required system. Install system meeting all requirements of 46 CFR 155.410 by next dry dock exam for credit, but, no later than 31 July 2027.

Individuals, who are not crew members, on board a towing vessel must receive a safety orientation prior to getting underway or as soon as practicable thereafter, to include: (a) The location, operation, and use of lifesaving equipment; (b) Emergency procedures; (c) Methods to notify crew members in the event of an emergency; and (d) Prevention of falls overboard. Crew did not have means to provide safety orientation.

All owners and operators of an existing towing vessel must have onboard a valid COI prior to the specified phase-in period. This vessel has not scheduled an inspection or obtained a valid COI. The vessel shall be made available for a Coast Guard inspection immediately and the vessel may no longer be permitted to operate.

The vessel operator had the renewal decal for the vessel's DOC at the office and not on board the vessel.
