Each towing vessel must be maintained and operated so as to minimize fire hazards. Murphy switch that senses fuel in the fwd fuel tank in E/R has rubber hosing that is not protected by a remote fuel shutoff and could melt in a fire. Reroute or replace with hard piping within 30 days.

Items must be recorded in the TVR or official logbook. Vessel has not been in operation since Dec 2024 and several maintenance items were not recorded including: L/S equipment checks, towing gear checks, engine alarm set points, and relief valve testing of unfired pressure vessels. Prove satisfactory record keeping within 30 days.

No person may transfer oil or hazardous material to or from a vessel (with a capacity of 250 barrels/10500 gallons as per 33 CFR 156.100 Applicability.) unless each person in charge, designated under 33 CFR 154.710 (facility requirement) and 33 CFR 155.700 (vessel requirement) of this chapter, has filled out and signed the declaration of inspection (DOI) form described in paragraph (c) of this section. Crew on board during inspection was not knowledgeable of requirement. Prior to conducting next regulated transfer, and within the next 30 days, provide objective evidence showing crew's ability to comply with regulated transfer requirements including completion of the DOI.

Suitable closure devices must be provided for deck house or hull penetrations which may allow water to enter the vessel. Two main deck doors leading to the vessel's exterior on the starboard side and two deck hatches leading to voids on the port side did not have adequate gasket material to maintain a weather tight integrity.

Each towing vessel must have a reliable means to provide notification when an emergency condition exists, or an essential system develops problems that require attention. An alarm for a generator engine low lube oil pressure must be provided and the specific alarm condition must be indicated at the operating station or machinery location. During set-point test, found that the generators did shut down for a low oil pressure condition, but did not indicate the specific alarm.

Each towing vessel must have a reliable means to provide notification when an emergency condition exists, or an essential system develops problems that require attention. An alarm must be provided for low fuel level, if fitted with a day tank. Low level day tank alarm not operating.

Each towing vessel must be equipped with a means to detect smoke in the berthing spaces and lounges that alerts individuals in those spaces. There was no smoke detection in Captain’s and port main deck berthing.

Emergency lighting must be provided for all internal crew working and living areas. Emergency lighting sources must provide for sufficient illumination under emergency conditions to facilitate egress from each space. Inspection found emergency lighting in engine room and berthing passageway on 02 deck not working.

Each lifebuoy on a towing vessel must be approved under approval series 160.050 or 160.150, which includes manufacturing. Lifebuoy located at bow was missing the fill cap installed during manufacturing.

The original COI must be framed under glass or other transparent material and posted in a conspicuous place onboard the towing vessel. Inspection found the COI onboard was a copy. 46 CFR 136.220

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Exhaust fans in E/R were not operable.

Towing vessels must be inspected in conjunction with the issuance of a Certificate of Inspection (COI). Vessel has been issued a COI to reflect change of ownership. Schedule vessel for inspection within 30 days.

No person may intentionally drain oil or hazardous material into the bilge of a towing vessel from any source. Engine room has an accumulation of approximately 3" of oil in bilge. Pump down bilges as required.

Each towing vessel must be maintained and operated so as to minimize fire hazards and to ensure the following: (a) All bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquids insofar as practicable. Fuel oil transfer pumps and emergency fuel cutout valve leaking into engine room bilge. Repair leaks as required.

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. 220 volt electrical panel in engine room has missing breaker/breaker blanks. Cover exposed shock hazards as required.

An existing vessel may be deemed by the OCMI, or TPO, to be in compliance with this subpart provided that either: (b) The vessel has been both in satisfactory service insofar as structural adequacy is concerned and does not cause the structure of the vessel to be questioned by either the OCMI, or TPO engaged to perform an audit or survey. The following structural non-conformities were identified: 1. Weld seem to water tank in forward line locker fractured and leaking water into space. 2. Hull plate wasted and holed on port side of line locker under exterior rub rail. 3. Hull plate wasted and holed on starboard side engine room under exterior button cleat.

Vessel reported stbd shaft was entagled with buoy chain.

VESSEL IS MISSING OIL TRANSFER PROCEDURES.

FOG BELL ON BOARD THE VESSEL WAS MISSING THE STRIKER.

REPLACE BOTH RING BOUYS WITH SERVICABLE COAST GUARD APPROVED ONES.
