Vessel is authorized to install doubler plates as a temporary repairs in aft void space. All new doubler plates are to be permanently marked to be identified. The Port and STBD aft voids in way of rudder stock voids are taking on water at various places in bottom hull plating. One dewatering pump is in place and operating continuously and various other pumps are in place and operating intermittently to mitigate flooding and maintain stability. Survey the vessel to determine extent of the damage and provide thickness readings of all bottom plating prior to conducting repairs. Make repairs and conduct satisfactory NDT prior to returning to commercial service.

Each towing vessel must be maintained and operated so as to minimize fire hazards and to ensure the following: (a) All bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquids insofar as practicable. There is an excessive amount of lube oil accumulated in the engine-room bilges below each main engine. In addition, there is an excessive amount of fuel accumulated in the fuel-filter room bilge and the fwd-adjacent space's bilge. The vessel is fitted with a dedicated slop tank to store this fuel/lube oil. Remove the oil and fuel from all bilge spaces.

All seals and gaskets must be properly maintained to prevent leaks of combustible liquid into the machinery space. Observed fuel being collected in a 5-gallon bucket from a leak under the Stbd Generator fuel filters. In addition, there is a fuel leak on the fuel supply line located in the overhead, centerline of the engine room. The crew is using sorbent material to collect the leak. Repair the leak and remove saturated rags in the vicinity. Provide verification of the pipe's integrity while at operating pressure after repairs are made.

Pressure vessels over 5 cubic feet in volume and over 15 pounds per square inch maximum allowable working pressure (MAWP) must be equipped with an indicating pressure gauge (in a readily visible location) and with one or more spring-loaded relief valves. The total relieving capacity of such relief valves must prevent pressure from exceeding the MAWP, as established by the manufacturer, by more than 10 percent. Engine room Portside air receiver with a MAWP of 150 psi found with a 200psi relief valve installed. An exposed hazard such as gears and rotating machinery, must be protected by a cover, guard or rail. Port and Starboard gear cooler pumps, as well as Port and Starboard Main engine Fuel oil pumps are missing protective covers around the shafts. Reinstall safety guards on all pumps with exposed rotating shafts.

A towing vessel must be maintained and operated so the watertight integrity and stability of the vessel are not compromised. During an inspection after the vessel grounded, CG Marine Inspectors determined the Stb aft, Port/Stbd Centerline, Port Aft, and Fwd Port voids are taking on water. Make the voids safe for entry and provide a Marine Chemist certificate for further inspection. In addition, ensure the vessel's hull is watertight prior to returning to commercial service to the satisfaction of attending marine inspector.
