Survival craft must be appropriate for the intended route; vsl is missing a life float. Item is on order. Vsl possesses an EPIRB and appropriate number of distress signals; is restricted to Rivers route until def. is cleared.

Frequency of inspections. After a towing vessel receives its initial COI, the OCMI will inspect a towing vessel subject to this subchapter located in his or her jurisdiction at least once every 5 years. The OCMI must ensure that every towing vessel is of a structure suitable for its intended route. The vessel must come in compliance with the following to receive the USCG COI: -Vessel name does not match COI/COD -No valid Drug/Alcohol consortium -Expired FCC certificates -No logs present - training, drills, maintenance, safety orientation -Charts require updates -Port generator fuel lines found wasted -Port main raw water line found wasted -Satisfactory operation of the bilge suction -All deck hatches shall be opened for inspection -Pilothouse ladder well emergency lighting - exposed electrical hazards

A towing vessel subject to this subchapter and choosing the Coast Guard inspection option, or required to have the Coast Guard inspection option, must undergo an annual inspection within 3 months before or after the COI anniversary date. The vessel's window for the annual inspection ended on 01SEP2022. The vessel must complete an annual inspection prior to operation.

Each towing vessel must be fitted with a general alarm that can notify persons onboard in the event of an emergency. The general alarm is inoperable and incorrectly wired so that it shuts down radios and vessel alarm systems when activated.

Electrical systems must be designed and maintained to provide for the safe operation of the towing vessel and safety of persons onboard. Vessel had unsafe electrical installation with permanent usage of extension cords and power strip. Remove power strip and terminate connections in appropiate power sockets.

All owners or managing operators of an existing towing vessel are required to a COI under Subchapter M by 20 July 2020. Vessel does not have COI. Meet requirements under Subchapter M and complete COI process.

A towing vessel must carry adequate and up-to-date charts, maps, and publications for the intended voyage. Vessel charts were not updated and publications had not been updated since 2014. Update all charts and publications.

No later than July 22, 2019, the owner or managing operator must implement a health and safety plan. The vessel did not have a health and safety plan. Implement health and safety plan in accordance with regulatory requirements.

The officer in charge of a navigational watch must conduct a navigation assessment for the intended route and operations prior to getting underway. Vessel has not been keeping adquete records of navigation assessments of pre-underway operational checks.

Each towing vessel must have a low hydraulic steering fluid level alarm provided. Vessel does not have an alarm. Install a low hydraulic steering fluid alarm.

Excessive oil accumulation throughout engine room bilge. Oil level was almost to engine room deck plates. Uncovered, 2'x2'x3' portable container 3/4 full of used oil also located in engine room. All bilges are to be kept free from accumulation of combustible and flammable material. Remove oil from bilge to the satisfaction of the OCMI and rectify the cause of oil accumulation.

Excessive shaft packing gland leakage. Vessel must be maintained and operated so the watertight integrity and stability of the vessel are not compromised. Make effective repairs to the satisfaction of the OCMI.

Multiple electrical deficiencies posing safety risks to vessel and crew including: exposed wires and connectors and electrical switch box that had water dripping out of it due to leaking pump mounted above switch box. Electrical systems must be maintained to provide safe operation of vessel and safety of persons onboard. Make effective repairs to the satisfaction of OCMI.

Starboard Main Diesel Engine inoperable due to crankcase explosion. Machinery must be maintained to provide safe operation of vessel. Make effective repairs to the satisfaction of OCMI.
