Each towing vessel must be equipped with navigation lights, shapes, and sound signals in accordance with the International Regulations for Prevention of Collisions at Sea (COLREGS) or 33 CFR part 84 as appropriate to its area of operation. Vessel reported the ship's whistle as inoperable. Rectify within 7 days.

Vessel number one steering pump is leaking hydrualic fluid. Failure to comply with steering requirements. Make repairs to the satisfaction of the attending Coast Guard marine inspector.

Bilge piping does not have screens at suction point. Failure to comply with piping systems. Make repairs to the satisfaction of the attending Coast Guard marine inspector.

Vessels port engine alarmed in th wheel house indicating an overheating condition. Engine was shut down, vessel pushed in to investigate. Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons on-board under normal and emergency conditions. Vessel is restricted from getting underway until repairs are complete to satisfaction of the local Coast Guard Marine Inspector.

Vessel had steering casualty during fleet operations and stern quarter of vessel superstructure received damage when it struck a barge rake. Master stated the vessel lost steering and the breaker tripped but when mechanic conducted trouble shooting he could not recreate problem or find anything wrong with steering pumps. Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safe and safety of persons onboard. Vessel shall proceed to C&B Hebron KY under tow to affect permanent repairs. Conduct troubleshooting of steering gear and make repairs to the satisfaction of the attending Coast Guard marine inspector.

One bilge valve was seized and two bilge vales required maintenance in the forward deck gear compartment. There must be installed bilge pump for watering. Replace or repair bilge valves to the satisfaction of the attending Coast Guard Marine inspector.

Vessel pilothouse alerter system did not have an alarm in the pilothouse. Towing vessels shall have an alarm in the pilothouse distinct from any other alarm. Repair or install pilothouse alterer system in the pilothouse to the satisfaction of the attending Coast Guard marine inspector.

Generator front seal has an excessive leak. All seals and gaskets must be properly maintained to prevent leaks of flammable or combustible liquids. Repair generator front seal leak to the satisfaction of the attending Coast Guard Marine inspector.

Unable to properly test detcetion system, prove service and testing of detection system, COTP Order issued..

Master lacked proof of radar endorsement and/or refresher course w/in past 5-yrs.
