Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI. Vessel experienced damage to the starboard bow and side shell plating during an allision with an offshore well head. Vessel shall provide a repair proposal to the OCMI for approval, and complete repairs to the satisfaction of the attending marine inspector.

The owner or master shall instruct each crew member at least once every three months as to the duties the crew member is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard. Crew training shall be logged or otherwise documented. The training entry shall include the date of the training and the general description of the training topics. The vessel could not provide logs or evidence of training covering all emergency instruction topics, specifically rough weather at sea/flooding. Vessel shall incorporate this aspect of training as part of vessels and company training procedures. The vessel shall provide evidence to the MI that this issue is addressed company wide since this has been a re-occuring event onboard the company vessels. 46 CFR 185.420(a) & (c)

Each hatch used for emergency escape must be capable of being opened by one person, from either side, in both light and dark conditions. After securing power to the vessel, emergency light illumination was not strong enough to adequately light a path to, and area of, the emergency escape in the fwd stbd side engine room emergency escape hatch. Vessel shall provide adequate lighting to properly illuminate area for emergency escape from said space. 46 CFR 177.500(h)

Provisions shall be made for closing all supply duct cowl openings for spaces protected by a fixed gas fire extinguishing system. All closure devices must be readily available. Pins holding covers in place were missing and not properly attached. Vessel shall make necessary repairs to ensure proper installation can be achieved prior to departure. 46 CFR 182.465(h)

Cracks observed on internal members in forward machinery space on forward partial bulkhead prot side and steering compartment port side. Gouge and reweld.

Gouge/Reweld 20 cracks in lazerette.

5 cracks on deck to be Gouge/Reweld.

Gouge/Reweld 1 crack in the Foc'sle.

Gouge/Reweld 2 cracks above the generator exhaust.

9 Hoses to be replaced on the Reduction gear.

1 hose to be replaced on the fire monitor engine

Prove proper operation of the galley bilge alarm light in the wheelhouse.

Firepump guage was only showing 50 psi. Firepump must maintain 60 psi.

Vessel was granted a 30 day dry-dock extension due to Hurricanes Gustav and Ike.

Up-to-date corrected nautical charts were not onboard

The bilge high water alarm for the lazarette was inoperable.

One eye wash bottle was expired.

One master did not have a marine radio operator permit.

Create and maintain a log documenting vessel drills, training, and tests as directed in 46 CFR 185.520, 185.524, 185.320 etc.

Provide a copy of a valid Communications Act Safety Radiotelephony Certificate to [name withheld] DESC: Safety Radiotelephony Certificate

Provide proof of reinspection as requried by 46 CFR176.500 (b), or provide written documentation (from the OCMI your vessel was operating during the anniversary date) that the vessel was granted relief of such requirement. Conduct pressure test of fire pump in the presence of a USCG Marine Inspector after effective repairs.

Repair fractured welds on starboard side inset three feet aft of sewage overboard discharge(void behind crewmember closet).

Replace plastic dryer hose with metal dryer hose.

Lable forepeak bilge suction valve.

ENSURE AT LEAST ONE CREW MEMBER ON BOARD RECEIVES A RADIO OPERATORS PERMIT.

MAKE VESSEL READY FOR INSPECTION OF WORKLIST ITEMS NOTED DURING DRYDOCK EXAM ON 18SEP98 AT SEACRAFT SHIP YARD IN AMELIA, LA. COMPLETE ABOVE REQUIREMENT TO SATISFACTION OF [name withheld], LA OR COGNIZANT MARINE INSPECTOR.

HAVE SSB RADIO CALIBRATED TO ENSURE 2182 CAPABILITIES.

PROCEED DIRECTLY TO OFFSHORE SHIPYARD IN VENICE, LA. VIA THE ICW FOR REPAIRS TO PORT SIDESHELL ADJACENT TO UPPER ACCOMODATION SPACE. COMPLETE VOYAGE BY 12 NOON 20MAR97

ADJUST FWD. AND AFT WATER TIGHT DOORS OF ENGINE ROOM SO THAT KNIFE EDGES SEAT PROPERLY ON GASKETS TO THE SATISFACTION OF ATTENDING CG MARINE INSPECTORE.

INSTALL BUSHINGS IN WIRE PENETRATIONS OF WATERTIGHT BKHD., PORTSIDE FWD. ENGINE ROOM.
