Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI. Vessel experienced damage to the starboard bow and side shell plating during an allision with an offshore well head. Vessel shall provide a repair proposal to the OCMI for approval, and complete repairs to the satisfaction of the attending marine inspector.

The owner or master shall instruct each crew member at least once every three months as to the duties the crew member is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard. Crew training shall be logged or otherwise documented. The training entry shall include the date of the training and the general description of the training topics. The vessel could not provide logs or evidence of training covering all emergency instruction topics, specifically rough weather at sea/flooding. Vessel shall incorporate this aspect of training as part of vessels and company training procedures. The vessel shall provide evidence to the MI that this issue is addressed company wide since this has been a re-occuring event onboard the company vessels. 46 CFR 185.420(a) & (c)

Each hatch used for emergency escape must be capable of being opened by one person, from either side, in both light and dark conditions. After securing power to the vessel, emergency light illumination was not strong enough to adequately light a path to, and area of, the emergency escape in the fwd stbd side engine room emergency escape hatch. Vessel shall provide adequate lighting to properly illuminate area for emergency escape from said space. 46 CFR 177.500(h)

Provisions shall be made for closing all supply duct cowl openings for spaces protected by a fixed gas fire extinguishing system. All closure devices must be readily available. Pins holding covers in place were missing and not properly attached. Vessel shall make necessary repairs to ensure proper installation can be achieved prior to departure. 46 CFR 182.465(h)
