Each class 1 watertight door must have a quick action closing device operable from both sides of the door. The portside class 1 watertight door going into the 7P stairwell has damaged hardware rendering the door inoperable. Make repairs within 30 days and prior to any voyages beyond the boundary line.

All vessels shall have efficient guard rails or bulwarks on decks and bridges. The hand rails on the starboard and port stern corners are damaged and need repair. Additionally approximately 50 feet of hand rail on the fish loading deck is missing/damaged from the stbd bow aft. Repair and reinstall all handrail iaw 46 CFR 92.25-5.

Each vessel must meet the requirements in subchapter S; Each vessel must have a stability letter...The stability letter sets forth the vessels conditions of operation. MI observed a substantial amount of water in the bilge in the vicinity of the space, 6c. Part 5 of HANNAH's stability letter states "The vessel’s bilges and voids shall be kept pumped to a minimum content at all times consistent with pollution prevention requirements." Pump bilge accumulation consistent with the requirements of the issued stability letter.

All auxiliary machinery must be designed, constructed, and installed in accordance with the provisions of this subchapter. MI observed the #6 and #9 generator sets were undergoing trouble shooting at the time of inspection. Perform functional tests with attending marine inspector during follow on inspection with SEC Western Alaska.

Construction of vessel shall be such as to minimize fire hazards. Two penetrations in the engine rm port side and one on the main deck centerline adjacent to the hotel need to be properly sealed. Verified during follow on inspection with SEC Western Alaska.

Lifesaving appliances and arrangements are subject to tests and inspections described in paragraph (b) of this section during vessel boardings to ensure that the appliances and arrangements comply with applicable requirements, are in satisfactory condition, and remain fit for the service. Vessel unable to launch life saving appliances, nor conduct drills due to crew complement being onboarded upon arrival in Alaska. Tests/Drills to be completed upon arrival.

There shall be at least two means of escape from areas where the crew may be quartered or normally employed. Design of deck coaming and scuppers permits emergency escape hatches from below deck spaces in the vicinity of the fish loading area (aft of midship; port and starboard) tow be submerged. Escape routes must be unobstructed. 46 CFR 92.10-5

A space used in the sorting or processing of fish in which water is used must be fitted with dewatering system capable of dewatering the space under normal conditions of list and trim at the same rate as water is introduced. The dewatering system must be interlocked with the pump(s) supplying water to the space, so that in the event of failure of the dewatering system, the water supply is inactivated. Main Deck Fish House above 6C and 7C dewatering pump was inoperable due to a ground fault. This failure resulted in the water introduced into the space to spill outside of the conveyor belt system, leading to water cascading down from upper decks within the fish house to the main deck where 6 inches of pooled water on main deck within the fish house. System not fitted with interlock to prevent water accumulation. 46 CFR 28.255 & 46 CFR 91.27–1(a)(4)

Each inflatable buoyant apparatus must meet the liferaft stowage requirements in § 199.130(c), being arranged for float-free launching. The arrangement must ensure that the inflatable buoyant apparatuses, when released and inflated, are not dragged under by the sinking vessel. A hydrostatic release unit used in a float-free arrangement must be approved under approval. Vessel's IBAs are lashed down and not float free. Hydrostatic release does not cut cradle strap. 46 CFR 199.640(j)

Every crewmember must participate in at least one abandon-ship drill and one fire drill every month. Additionally, drills must be held before sailing when a vessel enters service for the first time, after modification of a major character, or when a new crew is engaged. Drills were conducted when crew arrived in early June, but monthly drills were not conducted until attending marine inspector came in late July. 46 CFR 199.180(c)

Every crewmember must be given instructions that include the operation and use of fire-extinguishing appliances. Cook on-board was unfamiliar with the location of designated fire-fighting appliances in the galley and the operation of the manual galley hood-extinguishing system installed. 46 CFR 199.180(g)

Each lifejacket must be marked - In block capital letters with the name of the vessel. Lifejackets not currently marked. 46CFR199.70(b)(3)(i)

Machinery compartments containing equipment for ammonia must be fitted with a sprinkler system providing an effective water spray and having a remote-control device located outside the compartment. Vessel has approved plans, but has nto finished installation of required equipment. 46CFR58.20-15(b)

After any survey of the vessel made under 46 CFR 42, no change shall be made in the vessel's structure, equipment, arrangement, or scantlings without prior specific approval of the issuing authority. Vessel removed unauthorized portions of the bulkhead on the weather deck, affecting their watertight integrity after issuance of a temporary Load Line Certificate.

The weather deck must have freeing ports, open rails, and scuppers, as necessary, to allow rapid clearing of water under all weather conditions. The vessel accumulates standing water in compartments on the weather deck and has no approved means of drainage.

No person may operate an oceangoing ship of 400 gross tons and above but less than 10,000 gross tons unless it is fitted with approved 15 parts per million (ppm) oily-water separating equipment for the processing of oily mixtures from bilges, or fuel oil tank ballast and an oil residue (sludge) tank or tanks of adequate capacity to receive oil residue that cannot be dealt with otherwise. The vessel has no permanent means of retaining bilge water, there was excessive oily water in the bilge, the temporary tank on board was not utilized and is of insufficient capacity for vessel operations.

Electrical installations on vessels must ensure maintenance of services necessary for safety under normal conditions and the protection of crew, other persons, and the vessel from electrical hazards. The vessel has exposed electrical wiring from a previous fire in a compartment that regularly produces condensation and has standing water.

Cylinders must always be secured and, when not in use, they must be stowed in a rack in an upright position. Oxygen and acetylene tanks were secured to a pillar with line, not in a rack, and in standing water.

Non-conducting deck coverings, such as non-conducting mats or gratings, suitable for the specific switchboard voltage must be installed for personnel protection at the front and rear of the switchboard and must be sufficient width to suit the operating space. The switchboard in the emergency generator space did not have non-conducting deck coverings.

Electric equipment must be arranged, as far as practicable, to prevent mechanical damage to the equipment from the accumulation of dust, oil vapors, steam, or dripping liquids. Condensation and/or a water leak from piping was observed actively dripping onto an energized charging appliance in the generator space.

Piping systems shall be installed so that under no condition will the operation of safety or relief valves be impaired. The vessel removed all of the motor operated valves from the ballast system due to them being inoperable.

Vessels of 500 gross tons or more on an international voyage, must be provided with at least one international shore connection complying with ASTM F 1121. The international shore connection on deck was missing appropriate hardware components.

Alterations or repairs of machinery or piping systems shall be done under the cognizance of the Officer in Charge, Marine Inspection and in accordance with applicable regulations. The water recirculation piping system is actively leaking and has been temporarily repaired with electrical tape.

The ring, lines and letter shall be painted in white or yellow on a dark ground or in black on a light ground. They shall also be permanently marked on the sides of the vessels to the satisfaction of the assigning authority. Load line is not permanently marked on vsl.

Each vessel subject to load line requirements shall carry on board a valid certificate attesting to compliance with such requirements. Vessel did not have approved final load line certificate and corresponding stability documentation on board. 46 CFR 42.09-1(b)

All applicable electrical plans under 46 CFR 110.25-1 must be submitted for Coast Guard review, addressing all outstanding comments when returned. Vessel has resubmitted revised plans as per MSC correspondence E2-2401833, but has not received approved plans. Once plans are approved, address outstanding comments to the satisfaction of OCMI (if applicable). 46 CFR 110.25-1

Flexible non metallic hoses and fittings shall conform to SAE J1942 and J1475 specifications. Vessel generators (C18) are using non-compliant hoses for their generator cooling return systems. (02) generators are using 3" hoses that are marked w/J-1942, but not fully tested/approved to the standard. (06) generators are using non-J1942 hoses designed for turbocharger aftercooling. 46 CFR 56.60-25(b)

Each cable installation must meet IEEE 45.8-2016 if not constructed to IEC 60092-353. Such installations should provide support by means of metallic banding to prevent undue sag, but in no case should exceed the distance between frames or 610 mm (24 in.), whichever is less. Locations throughout the vessel are not provided with metallic banding spaced 24 inches apart, which in some areas permitted sagging of cable installations. 46 CFR 111.60-5(a)

All general alarm bells shall be identified by red lettering at least 1/2 inch high: “GENERAL ALARM—WHEN BELL RINGS GO TO YOUR STATION.” General alarm speakers are not identified with required signage. 46 CFR 97.37-7

There shall be at least two means of escape from areas where the crew may be quartered or normally employed. 5P, 7P, and 7S vertical emergency escape ladders were unable to be opened because they were either blocked on deck (5P or 7P) or was stuck closed (7S). 46 CFR 92.10-5

Coamings and hatchway covers to exposed hatchways on decks above the superstructure deck shall comply with the requirements of the assigning authority. 11 Hatch covers were found to be missing a stud or multiple studs, preventing it from being completely secured to the deck and watertight. Grind and repair as original.

Inset observed on Port side, just aft amidships, FR 36-37, approx 10'x8'x8".

Void space covers on deck missing studs and broken gaskets, impairing water tight envelope.
