All crew spaces are to be constructed and arranged in a manner suitable to the purpose for which they are intended and so that they can be kept in a clean, workable, and sanitary condition. Inspector observed overhead paneling in crew accommodation space arranged in such a way that it obstructs access to the egress hatch. Remove all egress route obstructions to the satisfaction of the attending marine inspector.

Each cargo vessel must carry the number of lifebuoys prescribed in table 199.271 of this section. Inspector observed 02 lifebuoys installed on vessel. Outfit vessel with lifebuoy requirements prescribed in 46 CFR 199.271 in the presence of attending marine inspector or provide approved equilavency letter from CG-ENG. 46 CFR 199.271 (50c)

All vessels shall have efficient guard rails or bulwarks on decks and bridges. The height of rails or bulwarks shall be at least 39 1/2 inches from the deck except that where this height would interfere with the normal operation of the vessel, a lesser height may be approved by the Commandant. Inspector witnessed sections of missing handrails throughout vessel. Submit repair plan proposal to OCMI for installation of efficient guard rails. 46 CFR 92.25-5(a) (50c)

Portable rails may be used when operating conditions warrant their use. OCMI approval letter for detachable handrails dated 21MAY1998, states vessel may remove handrails while engaged in operations. Inspector witnessed missing handrails in the vicinity of the crane during inspection. Provide detachable handrails while vessel is not engaged in operations. 46 CFR 42.15-75(b)(3)

Operational readiness. Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. Inspector witnessed incorrect configuration of liferaft hydrostatic release unit. Vessel corrected on the spot. 46 CFR 199.190(a)

No repairs or alterations affecting the safety of the vessel shall be made without the knowledge of the OCMI. Drawings of alterations shall be approved before work is started unless deemed unnecessary by the OCMI. The OCMI must be notified before alterations or modifications that deviate from approved plans. In the event of alterations involving the safety of the vessel, the applicable plans shall be submitted for approval covering the proposed work except as modified by 91.45-1. The general scope of the plans shall be as noted in 91.55-5. Plans must be submitted for all modifications associated with the LED installation. Plans must be submitted for the additional Caterpillar genset. The design and arrangements of electric apparatus must afford accessibility to each part as needed to facilitate proper inspection, adjustment, maintenance, or replacement. Locker in machinery space near starboard exit must be moved away from motor controller. Washer and dryer must be moved to a suitable location. 46 CFR 91.25-30, 111.01-7(a)

Each outlet or junction box must have a cover unless a fixture canopy, switch cover, receptacle cover, or other cover is used. 115V outlet in machinery space has a broken cover. 46 CFR 91.25-30, 111.81-1(c)

All relief valves for use on pressure vessels shall be designed to meet the protection and service requirements for which they are intended and shall be set to relieve at a pressure which does not exceed the MAWP of the pressure vessel. Relief valve on center air receiver in machinery space was unable to relieve. 46 CFR 91.25-35, 54.15-10(a)
